Genome
In natural host: papillomas; In other hosts, epithelial-derived cancers
MOST common viral causing venereal disease (STD)
Most subclinical/latent; Few become CA
Penile/anal warts can progress to dysplasia & then carcinoma
Equal prevalence of infection in M & F; M have less severe Sx
HPV (Human wart)
Papillomavirus
Papovaviruses
Papillomavirus HPV
Pathogenesis Virus attach R on basal cells
Penetrate by endocytosis
Uncoat at nuclear mem
E6 binds p53; E7 binds Rb -
Prolif of basal & suprabasal epithelial cells -
Transcribe & translate E genes & prot
Switch to L genes & form virion when cells differentiate into
keratinocytes
Mature virions shed when infected cells shed from skin surface
Causing AbNL prolif of kerotinocytes
Sx Verrucae=warts on cutaneous surf
Condyloma acuminata=genital & anal warts
Epidermodysplasia verruciformis=often become cancerous
Dx
HPV DNA radiolabeled & cross-hybridized to reference HPV
sequence

Fully baseparied=identical in sequence

<50% match-->new type of HPV
Direct sequencing of short, highly conserved regions present in
all HPVs: if differs >10%, new genotype

Genotyping=detect lvl of specific genes in DNA

Tx
With noninfectious capsid shells to protect 90% of genital warts
4 genotypes of HPV: 6, 11, 16, 18
Gardasil (1st CA vaccine) & Cervarix
Important!!! HPV 6 & 11 found in CIN but not in malignant cancers
HPV 16, 18 31 found in invasive carcinomas of cervix
Fatal demyelinating Ds of CNS
Later in life & especially affect pt with Ds of reticuloendothelial sys
JC can cause progressive multifocal leucoencephalopahty (PML)
Rarely causes overt Ds
BK reactivate in immunocompromised pt
Historically important for developing key concepts in oncogenic viruses
Tumor antigen (T-Ag)
Viral proteins that cause host cells to become immortal
SV-40 & mouse polyoma viruses=1st tumor viruses discovered
Naked
Icosahedral
dsDNA, C
T-Ag binds p53 & Rb-->inactivate p53 (TSG)-->induce cell prolif
Turn on L genes
Turn off E genes
1transcript spliced into T-Ag
t-Ag binds ___ -->stim Pol II & III transcription
1transcript not spliced would result in t-Ag
E genes copied by RNA Pol giving a large 1transcript
SV40 (simian virus)
Polyomavirus
Polyoma JC BK
Ds Multifocal leukoencephalopathy
(Junky cerebrum)
Mild respiratory illness
Hemorrhagic cystitis
(Bad kidneys)
Pathogenesis 80% adults have Ab to JC and BK
Can be latent lifelong OR
1infection in childhood
reactivate during pregnancy
Sx & Dx Inapparent infection in natural hosts
Polyoma-assoc nephropathy--emerging
Dx: PCR of genome DNA; ELISA of Ab
Can be oncogenic in different hosts
complication of renal transplant
to virus
