NATURE OF THE CASE:
Reckless Imprudence Resulting in Damage to Properties

DATE/TIME/PLACE OF OCCURRENCE:
June 09, 2014 /OOA 9:00 PM/ along Manila-North Rd, Brgy Sto Domingo 2nd,
Capas, Tarlac

INVOLVED VEHICLES:
VEH-1 One (1) Fuso Fighter Truck, blue in color, bearing license plate number
RJT 524, registered to Meladio Gonzales Jr # 254 Feliciano Homes Pulong Gubat
Balagtas (BIGAA) Bulacan under Certificate of Registration number 9885731-2 and
Official Receipt number 545753773 driven by Carlos Caballero y Martin, 64 years old,
married of 53 U Rincon City of Valenzuela Metro, Manila and a holder of a Professional
Drivers license # C08-08018260 valid until 2014-07-16.

VEH-2 One (1) Toyota HiAce GL Grande Van,white in color, bearing license
plate number TID 554, registered to Maria Luisa O Cabawatan of Bramonte Subdivision
P3 Tinajero Lucban Quezon City under Certificate of Registration number 13695375-1
and Official Receipt number 128325662, driven by Bernard Garon y Pangan, 48 years
old, married of 573 Sardonyx Street. GLS Mabalacat City, Pampanga and a holder of a
Professional Drivers license # C01-84-009061 valid until 2014-11-30.

FACTS OF THE CASE:
Investigation conducted shows that prior to the incident, both vehicles were
traveling along said road towards north direction with Veh-1 ahead of Veh-2. Upon
reaching the place of incident near at Digitel Post TX-0403, Veh-1 decelerated his
driven vehicle until it slowed down due to traffic ahead of it and while in the process, it
was hit and bumped by the tailing Veh-2. As a result thereof, both vehicles incurred still
undetermined amount of damages with Veh-1 on its rear portion while Veh-2 on its front
portion. No injuries happened during the incident.

ACTION TAKEN: Case was investigated and recorded at the police blotter.

June 10, 2014 /OOA 02:30 AM/ along Manila-North Rd, Brgy Sto Domingo 2nd, Capas,
Tarlac

INVOLVED VEHICLES:
VEH-1 One (1) Isuzu Tractor Head Truck, blue in color, bearing license plate
number UVS 740, registered to BPI Leasing Corporation Leased of 8/f Bank of PI
Building Ayala Avenue City of Makati NCR under Certificate of Registration number
13571209-1 and Official Receipt number 471047216 driven by Moises Razonado y
Noval, 38 years old, married of Letre RD- Malabon City and a holder of a Professional
Drivers license # N25-06-002288 valid until 2014-08-23.

FACTS OF THE CASE:
Investigation conducted that prior to the incident shows that vehicle involved was
traveling along said road towards north direction. Upon reaching the place of incident
near the traffic light situated at Capas Caltex Junction unidentified vehicle suddenly
crossed the road from south to east direction and to avoid hitting it, the driver of the
involved vehicle swerved to the right to avoid hitting it, but in the process the vehicle
involved almost turned-around and its trailer was almost detached from its tractor head.
As a result of the incident, the involved vehicle incurred still undetermined amount of
damages. No injuries happened during the incident.

ACTION TAKEN: Case was investigated and recorded at the police blotter.

OR ABOUT 11:30 AM JUNE 16, 2014, HE PARKED HIS DRIVEN VEHICLE AND LEFT
UNATTENDED INSIDE THE PARKING LOT OF THE SAN NICHOLAS DE
TOLENTINO PARISH CHURCH, BRGY STO ROSARIO, CAPAS, TARLAC FACING

SAID PLACE. UPON RETURNING, HE DISCOVERED THAT THE RIGHT FRONT
DOOR HANDLE OF HIS DRIVEN VEHICLE WAS ALREADY MISSING WHILE THE
LEFT FRONT DOOR HANDLE WAS ALMOST DETACHED. THAT HE BELIEVED
THAT SAME WAS TOOK/CARTED AWAY BY STILL UNIDENTIFIED SUSPECT/S.
