Adult Service Application

for Childrens and Student Ministry



Thank you for your interest in serving children or students at Frisco Bible Church. It is exciting what God is doing
to build these ministries. You will never regret investing time in the lives of the children and students. We want to
be a congregation that passes His blessings on to others.

It is mandatory for all volunteers in our Childrens and Student Ministries to complete and return an application.
Please be as thorough as possible, date each area where you sign.

This application consists of two parts, the first of which is the information needed by our background processing
vendor. The second part is a basic spiritual questionnaire. This application will be used for the express purpose of
screening volunteers for the Childrens or Student Ministries. Completed applications and information derived
from personal background checks will be held in the strictest confidence. Upon completion, please return the
application in an envelope with a return address or drop it off at the church.

Again, thank you for your interest in serving the children or student of Frisco Bible Church. We trust that Jesus will
richly bless your efforts and that you will truly make a positive and eternal impact on the lives of this next

FRISCO BIBLE CHURCH DOCTRINAL STATEMENT
The following is the doctrinal statement of Frisco Bible Church. Agreement with it represents one of the qualifications
for serving on a ministry team or the Elder Board.
















* * FRISCO BIBLE CHURCH CHILDRENS/STUDENTS MINISTRY CODE OF ETHICS AND RULES * *
1. Smoking or use of tobacco products in the presence of children is prohibited.
2. Using, possessing, or being under the influence of illegal drugs will not be tolerated.
3. Losing integrity of character or sobriety by being under the influence of alcohol will not be tolerated.
4. Ministry Volunteers shall not engage in inappropriate behavior with children/student/adults including but not limited to:
a. Physical - strike, spank, shake, slap
b. Verbal/Mental - humiliate, degrade, threaten
c. Sexual - including inappropriate touching and exposure.
5. Ministry Volunteers must treat children, youth and adults of all races, religions, and cultures with respect and consideration.
6. Ministry Volunteers shall not use nor tolerate profanity in the presence of children or student.
7. Ministry Volunteers must be free of physical and psychological conditions that might adversely affect the health of children
and student, including but not limited to fever or contagious diseases.
8. Ministry Volunteers will portray a positive role model for children/student by maintaining an attitude of respect, loyalty,
patience, courtesy, and maturity.
9. Ministry Volunteers will do everything in their power to avoid being put in a situation where they are alone with a
child/student other than their own.
10. Ministry Volunteers will be expected to act and react with Christian love and understanding in all situations.
11. I understand that as a Ministry Volunteer for Frisco Bible Church I will be subject to a background check, including criminal
history.

Scripture: We believe the scriptures of the Old and New Testaments,
that every word in the original manuscripts is inspired by God,
without error, and that the scriptures in their completeness are of
supreme and final authority in all matters of faith and life. (Cf., Exodus

Godhead: We believe in one God, eternally existing in three equal
persons: Father, Son, and Holy Spirit. (Cf., Deuteronomy 6:4; Psalm

5:3-4)
Jesus Christ: We believe that Jesus Christ is the eternal Son of God
who was conceived by the Holy Spirit and born of the virgin Mary. We
believe that during His life on earth He was true man while retaining
His absolute deity, that He might reveal God and redeem sinful
man.(Cf., John 1:1-4, 14-18; John 14:8-12; Hebrews 1:8; Isaiah 7:14;
Matthew 1:16, 20-21; Luke 1:34-35; Philippians 2:5-8; 1 Timothy 2:5-6)
We believe in the literal, physical resurrection from the dead of our
Lord Jesus Christ. We believe that the Lord Jesus Christ was bodily
exalted to the right hand of God. As His peoples high priest, He fulfills
the ministry of intercessions and advocacy, and He provides our
access to God. (Cf., Matthew 20:19; 1 Corinthians 15:1-8; Ephesians
1:19-21; Hebrews 12:2; Hebrews 4:14-16; Hebrews 7:23-25; Ephesians
2:17-18)
The Holy Spirit: We believe the Holy Spirit is the third person of the
Godhead. He convicts the world of sin, regenerates those who receive
Christ as Savior, baptizes them into the body of Christ, indwells them
permanently, seals them unto the day of redemption, bestows spiritual
gifts on each one, and fills and empowers those yielded to Him. (Cf.,
John 16:8-11; Titus 3:5; 1 Corinthians 12:13; 1 Corinthians 6:19;
Ephesians 1:13; Ephesians 4:30; 1 Corinthians 12:4-11; Romans 12:4-8;
Ephesians 5:18; Acts 1:8)
Angels: We believe angels are spiritual beings who were created as
sinless personalities. They presently exist in both fallen and unfallen
states, the former including Satan and his angels. (Cf., Hebrews 1:13,
14; Jude 5; Ezekiel 28:11-15; Isaiah 14:12-20; 2 Peter 2:4) We believe
that Satan, by choice the enemy of God and the people of God, led
our first parents into sin and now rules as the god of this world. We
believe he was judged at the cross, and that ultimately he will be cast
into the lake of fire.(Cf., Ephesians 2:2; Hebrews 2:14; Revelation
20:10)
Mankind and Sin: We believe that man was created in the image of
God, but in Adams sin the human race fell and incurred not only
physical death, but also spiritual death, which is separation from God.
Man inherited a sinful nature causing all human beings to be born
with a sinful nature.(Cf., Genesis 1:26-27; Genesis 9:6; Genesis 3:1-
Romans 5:12-14; Psalm 51:5; Romans 3:9-12, 23; Romans 4:12-14;
24; Ephesians 2:1-3)
Salvation: We believe that the Lord Jesus Christ died for our sins
according to the scriptures as a substitutionary sacrifice acceptable
by God; and that all who receive Him by grace through faith are
justified on the basis of His shed blood.(Cf., Matthew 20:18; 1
Corinthians 15:3-4; 2 Corinthians 5:21; 1 John 2:2; Ephesians 2:8-
9; 1 Peter 1:18-19; Romans 3:23-26; Romans 5:8-10) We believe
that all true believers, once saved, are forever secure in their
salvation; and that the abiding presence of the Holy Spirit as a seal
and pledge is ones guarantee of his inheritance. (Cf., John 10:27-
30; Ephesians 1:13-14; Ephesians 4:30; 1 Peter 1:3-5)
Sanctification: We believe that sanctification, which is a setting
apart unto God, is positional, practical, and permanent. Positional,
it is already completed since the believer is in Christ. However,
since he retains his sinful nature in this world, there is need for
practical sanctification in his daily experience, whereby the
Christian grows in grace by the power of the Holy Spirit.
Permanent sanctification will occur in the presence of the Lord
with the absence of the sin nature, where position and practice
will be consistent. (Cf., John 17:17; 2 Corinthians 3:18; Galatians
5:16; 1 Thessalonians 5:23; Hebrews 10:10; 1 Peter 1:13-16)
The Church and Ordinances: We believe that the church in its
invisible form is a universal and spiritual organism, the true body
and bride of Christ. All who are born again are members
regardless of organizational affiliation. We believe the Lord
instituted and ordained water baptism and the Lords Supper. (Cf.,
Ephesians 1:22-23; Romans 12:4-5; 1 Corinthians 12:12-14;
Colossians 1:18; Matthew 26:26-28; 1 Corinthians 11:23-26)
Future Things: We believe in the personal, imminent coming of
the Lord Jesus Christ for His Church. We believe in a tribulation on
earth which will culminate in the return of Christ to earth to set up
His millennial kingdom. (Cf., 1 Thessalonians 4:13-18; Revelation
3:10; Matthew 24:15-31; Zechariah14:4-11; Revelation 20:1-6)
The Eternal State: We believe Christians at death immediately pass
into the presence of Christ, and there remain in conscious bliss
until the resurrection of the body at the coming of Christ for the
Church. Unbelievers remain after death in conscious misery in
Hades until the final judgment at the close of Christs millennial
reign, when they are cast into the lake of fire. (Cf., Luke 16:19-26;
Luke 23:43; 2 Corinthians 5:8; Philippians 1:23; 2 Thessalonians
1:7-9; Revelation 20:11-15)

company hired by Frisco Bible Church will verify all or part of the information I have given Frisco Bible Church . I understand that this verification may
include any inquiry into my motor vehicle driving record, criminal and civil records, as well as other public record information. I authorize the release of such
information as may be necessary to verify the information I have provided. I release and hold harmless from all liability Frisco Bible Church , the background
check investigation company and any individual or entity requesting or supplying information with respect to my application to volunteer.

Summary of Your Rights Under the FCRA
The Federal Fair Credit Reporting Act (FCRA) is designed to promote accuracy, fairness, and privacy of information in the files of every "consumer reporting
agency" (CRA). You can find the complete text of the FCRA at 15 U.S.C. 1681-1681u. The FCRA gives you specific rights, as outlined below. You may have
additional rights under state law. You may contact a state or local consumer protection agency or a state attorney general to learn those rights.
You must be told if information in your file has been used against you. Anyone who uses information from a CRA to take action against you -- such as
denying an application for credit, insurance, or employment -- must tell you, and give you the name, address, and phone number of the CRA that provided
the consumer report.
You can find out what is in your file. At your request, a CRA must give you the information in your file, and a list of everyone who has requested it recently.
There is no charge for the report if a person has taken action against you because of information supplied by the CRA, if you request the report within 60
days of receiving notice of the action. You also are entitled to one free report every twelve months upon request if you certify that (1) you are unemployed
and plan to seek employment within 60 days, (2) you are on welfare, or (3) your report is inaccurate due to fraud. Otherwise, a CRA may charge you up to
eight dollars.
You can dispute inaccurate information with the CRA. If you tell a CRA that your file contains inaccurate information, the CRA must investigate the items
(usually within 30 days) by presenting to its information source all relevant evidence you submit, unless your dispute is frivolous. The source must review
your evidence and report its findings to the CRA. (The source also must advise national CRAs -- to which it has provided the data -- of any error.) The CRA
must give you a written report of the investigation, and a copy of your report if the investigation results in any change. If the CRA's investigation does not
resolve the dispute, you may add a brief statement to your file. The CRA must normally include a summary of your statement in future reports. If an item is
deleted or a dispute statement is filed, you may ask that anyone who has recently received your report be notified of the change.
Inaccurate information must be corrected or deleted. A CRA must remove or correct inaccurate or unverified information from its files, usually within 30 days
after you dispute it. However, the CRA is not required to remove accurate data from your file unless it is outdated (as described below) or cannot be
verified. If your dispute results in any change to your report, the CRA cannot reinsert into your file a disputed item unless the information source verifies its
accuracy and completeness. In addition, the CRA must give you a written notice telling you it has reinserted the item. The notice must include the name,
address and phone number of the information source.
You can dispute inaccurate items with the source of the information. If you tell anyone -- such as a creditor who reports to a CRA -- that you dispute an
item, they may not then report the information to a CRA without including a notice of your dispute. In addition, once you've notified the source of the error
in writing, it may not continue to report the information if it is, in fact, an error.
Outdated information may not be reported. In most cases, a CRA may not report negative information that is more than seven years old; ten years for
bankruptcies.
Access to your file is limited. A CRA may provide information about you only to people with a need recognized by the FCRA -- usually to consider an
application with a creditor, insurer, employer, landlord, or other business.
Your consent is required for reports that are provided to employers, or reports that contain medical information. A CRA may not give out information about
you to your employer, or prospective employer, without your written consent. A CRA may not report medical information about you to creditors, insurers, or
employers without your permission.
You may choose to exclude your name from CRA lists for unsolicited credit and insurance offers. Creditors and insurers may use file information as the basis
for sending you unsolicited offers of credit or insurance. Such offers must include a toll-free phone number for you to call if you want your name and
address removed from future lists. If you call, you must be kept off the lists for two years. If you request, complete, and return the CRA form provided for
this purpose, you must be taken off the lists indefinitely.
You may seek damages from violators. If a CRA, a user or (in some cases) a provider of CRA data, violates the FCRA, you may sue them in state or federal
court.

Bible Church permission to photograph or video my image and the image of my minor
children participating in such program and consent to the use of such voice, likeness and
images by Frisco Bible Church and its employees, agents and contractors for any and all
church- sponsored communications (including but not limited to newsletters, brochures,
worship folders, video productions, church website and advertisements) and to make changes
or alterations to such voice, likeness and images as Frisco Bible Church deems proper. The
permission granted hereby shall pertain to any such image taken prior to the receipt by Frisco
Bible Church of written notice from me revoking the consent herein granted.
