13.

For every problem listed on the Chart of Patient Needs, state the long term goals for dental
hygiene treatment that have been established by the clinician and the patient. Under each long
term goal, state at least 2 short term goals that will help the patient attain the long term goals.
Plaque/ Brushing:
LONG TERM GOAL: Patient will reduce plaque score by 0.3 at every
appointment until she reaches a plaque score of 0.76.
SHORT TERM GOALS: 1) Patient will be able to describe plaque and be able to
explain how it forms by the second patient education session. 2) Patient will correctly use
the Bass brushing technique by the third patient education session. 3) Patient will choose
to floss three times per week by the fourth appointment.
Periodontal Disease/ Flossing:
LONG TERM GOAL: Patient will eliminate gingivitis by an improved gingival
description and bleeding score by the end of treatment.
SHORT TERM GOALS: 1) Patient will describe periodontal disease by the third
patient education session. 2) Patient will correctly use the Bass brushing
technique by the third patient education session. 3) Patient will choose to floss
three times per week by the fourth appointment.
Smoking Cessation/ Quitting:
LONG TERM GOAL: Patient will choose to successfully quit smoking by the
next six month appointment.
SHORT TERM GOALS: 1) Patient will choose to seek help from a physician to
collect information about starting nicotine replacement therapy before the last
appointment. 2) Patient will decrease cigarette usage to 3 times a week by the last
appointment. 3) Patient will choose to utilize nicotine replacement therapy by the
last appointment.

15. Outline the information to be presented in the three patient education sessions. Include the
specific information that will be taught, the methods of presentation, and what visual aids will be
used.
Session 1:
Plaque/Brushing:
Define plaque. Sticky, colorless film of bacteria and sugars that constantly forms on
teeth.
Discuss patients long and short term goals.
Explain that plaque develops when food is left on the teeth. Plaque then turns to calculus.
Show patient pictures of plaque coated teeth from my patient ed. Flip chart.
Explain plaque score and how it is calculated.
Show patient what disclosing solution looks like and explain how it shows you where all
of the plaque is in your mouth.
Ask patient to brush at the sink
Tell patient which brushing technique should be used.
Show patient how to brush on the typodont, and let patient brush on typodont.
Session 2:
Periodontal Disease/ Flossing:
Define periodontal disease. Gingivitis-inflammation of gingival=infection in your mouth.
First stage of periodontal disease. Gingiva is red and bleeding. If gingivitis is not
reversed, it turns into periodontitis. Periodontitis- loss of bone that surrounds your teeth.
Pockets higher than 4 mm. Second stage of periodontal disease.
Discuss patients short and long term goals.
Ask patient to floss.
Show patient how to floss on typodont
Tell patient about bleeding score and what it means.
Show patient pictures of periodontal disease.
Session 3:
Smoking Cessation/ Quitting:
Discuss options for quitting cold turkey, medications, smoking cessation counseling,
Quit hotlines or websites, nicotine gums, nicotine patches.
Discuss short and long term goals.
Effects that smoking has on oral health
- ALL forms of tobacco are harmful and can lead to chemical dependence, one form
should never be substituted for another.
- Smoking accounts for 77% of all oral and pharyngeal cancers.
- Development of leukoplakia (precancer)
- Tooth loss, staining, abrasion, periodontal disease, gingival bleeding, calculus,
halitosis, hairy tongue.
Cigarette smoke contains more than 4,000 toxins and over 60 carcinogens (agent directly
related to causing cancer).
Smoking affects cardiovascular system, respiratory system, skeletal motor, and
gastrointestinal system. Even leads to death.
