Operating Guidelines

This procedure establishes minimum requirements for safe use of rigging and
hoisting equipment used in material handling. The procedure is based on OSHA
regulations in 29 CFR 1926.251 and on U.S. Army Corps of Engineers Safety
and Health Requirements Manual EM385-1-1, Revised September, 1996.

Responsibilities
2.1

Project Superintendents are responsible for selecting and purchasing or renting


lifting equipment. They are also responsible for confirming that lifting operations are
performed safely and in compliance with OSHA regulations and Haselden
Construction procedures.

2.2

On-site supervisors are responsible for inspecting lifting operations and verifying
that operations are conducted in compliance with this procedure and other safety
regulations and safe work practices. They are also responsible for identifying new
hazards and developing control measures to prevent injuries and property damage.

2.3

Qualified employees are responsible for complying with this procedure and for
inspecting all rigging and hoisting equipment for damage and defects before
each use. Employees must immediately report any damaged or defective
equipment to their supervisor.

Operational Procedures
3.1

Inspection and Use


3.1.1

The use of ropes, slings, and chains must be in accordance with safe
work practices recommended by their manufacturers. Rigging equipment
for material handling must be inspected for damage and defects prior to
each use. At the minimum, rigging shall be inspected for the following
failure modes:
3.1.1.1 Broken wires; for strand laid and single part slings, ten randomly
distributed broken wires in one rope or five broken wires in one
strand in one rope lay; for cable laid and braided slings.

3.1.1.2 Severe localized abrasion or scraping.


3.1.1.3 Kinking, crushing, birdcaging, protruding core, or any other
damage resulting in distortion of the rope structure.
3.1.1.4 Severe corrosion of the rope or end fittings.
3.1.1.5 Evidence of electric arc or heat damage.
3.1.1.6 Excessive pitting or corrosion, or cracked, distorted, or broken
fittings.
3.1.1.7 Diameter reduction:
3.1.1.7.1

Reductions of 0.4 mm (1/64 in) for diameters up to and


including 8 mm (5/16 in);

3.1.1.7.2

Reductions of 0.8 mm (1/32 in) for diameters 10 mm


(3/8 in) up to and including 13 mm (one-half inch);

3.1.1.7.3

Reductions of 1 mm (3/64 in) for diameters 14 mm


(9/16 in) up to and including 19 mm (3/4 in);

3.1.1.7.4

Reductions of 2 mm (1/16 in) for diameters 22 mm (7/8


in) diameter up to and including 29 mm (1-1/8 in);

3.1.1.7.5

Reductions of 2 mm (3/32 in) for diameters 22 mm (7/8


in) up to and including 38 mm (1-1/2 in).

3.1.1.8 Significant stretching of the wire rope beyond the initial


construction stretch.
3.1.1.9 Any sign of metal fatigue or other visible damage that causes
doubt as to the strength of the wire rope.
3.1.1.10 Evidence of failure is cause for replacement of the rigging.
Rigging degradation not only indicates that the rigging is
becoming unsafe and requires replacement, it also often
indicates problem(s) with the rigging setup, use, or maintenance.
3.1.1.11 Damaged or defective equipment must be repaired or replaced
immediately. Rigging equipment removed from service must be
clearly tagged "DANGER - DO NOT USE" or else cut or damaged
and disposed of to prevent reuse. Hooks, shackles, rings, pad
eyes and other fittings that show excessive wear or that have been
bent, twisted or otherwise damaged, must be removed from
service.
3.1.1.12 When not in use, rigging equipment must be removed from the
immediate work area and properly stored and maintained in a safe
condition.
3.1.1.13 Loading rigging equipment in excess of its recommended safe
working load is prohibited. Safe working limits are defined in
Tables H-1 through H-20 attached to this procedure.

3.1.1.14 Special custom design grabs, hooks, clamps, or other lifting


accessories are sometimes present on units such as modular
panels, prefabricated structures, and similar materials. These lifting
accessories must be marked with their safe working loads. They
must also be proof-tested up to 125 percent of their rated load,
prior to initial use.
3.1.1.15 When hoisting loads, a positive latching device shall be used to
secure the load and rigging. Only closed hooks will be used. Open
hooks are prohibited.
3.1.1.16 Eye splices will be made in an approved manner. Rope thimbles of
proper size will be fitted in the eye, except that the use of thimbles
in slings is optional.
3.1.1.17 Running lines located within 6.5 feet (2 meters) of the ground or
working level must be boxed off, guarded, or the area restricted
from unauthorized access.
3.1.1.18 Drums, sheaves, and pulleys must be smooth and free of surface
defects that could damage ropes and lines.
3.1.1.19 Suspended loads must be kept clear of all obstructions. Personnel
must be kept clear of suspended loads and those about to be
lifted.
3.1.1.20 Shock loading is prohibited.
3.1.1.21 Pulling a sling from under a load while the load is resting on the
sling may damage the sling. This action is prohibited.
3.2

Alloy Steel Chains


3.2.1

Only alloyed chain will be used in rigging. Chains must be inspected for
damage and defects before each use. In addition, a formal annual
inspection must be performed on each chain. Annual inspections must
be documented in writing with copies kept at the job site.

3.2.2

New, repaired, or reconditioned alloy steel chain slings must be prooftested before initial use, in accordance with the manufacturer's
instructions. A certificate of the proof test must be retained for the
service life of the sling. Copies of proof test certificates must be
available at the job site.

3.2.3

Chains must be normalized or annealed periodically as recommended


by the manufacturer.

3.2.4

Hooks, rings, oblong links, pear-shaped links, welded or mechanical


coupling links, or other attachments used with alloy steel chains must
have a rated capacity at least equal to that of the chain.

3.2.5

The use of job or shop hooks and links, makeshift fasteners, or other
such attachments formed from bolts, rods, etc. is prohibited.

3.2.6

Alloy steel chain slings must have a rated capacity (working load limit)
at least equal to the values shown in Table H-1. Welded alloy steel
chain slings must have permanently attached, durable identification
showing their size, grade, rated capacity, and manufacturer.

3.2.7

Alloy steel chain assemblies must be replaced when wear at any point
of any chain link exceeds the values shown in Table H-2.

3.2.8

Chains must also be replaced if they are heated to 1000 degrees


Fahrenheit and when they show cracks, nicks, lifting of any link weld,
more than 10 percent elongation of any link or section, or when the
diameter of any link is worn down 20 percent.

3.2.9

Alloy steel chain slings must be replaced if they have cracked hooks;
hooks opened more than 15 percent of the normal throat opening,
measured at the narrowest point; or hooks twisted more than 10
degrees from the plane of the unbent hook.

3.2.10

Use of mechanical coupling links or low carbon steel links to repair


broken chains is prohibited.

Wire Rope
3.3.1

Tables H-3 through H-14 must be used to determine the safe working
load of improved plow steel wire rope and wire rope slings. For sizes,
classifications, and grades not included in these tables, the safe
working load recommended by the manufacturer for specific identifiable
products must be followed. A minimum safety factor of five must be
maintained at all times.

3.3.2

Protruding ends of strands in splices on slings and bridles must be


covered or blunted.

3.3.3

Securing wire rope by knots is prohibited.

3.3.4

An eye splice made in any wire rope will have at least five full tucks.
Forming eyes by wire rope clips or knots in wire rope bridles, slings, or
bull wires is prohibited. Wire rope clips will not be used to splice rope.

3.3.5

Wire ropes used in hoisting, lowering, or pulling loads must be one


continuous piece without knots or splices; except for endless rope
slings and eye splices in the ends of wires.

3.3.6

Wire rope must be replaced if the total number of visible broken wires in
any length of eight diameters exceeds 10 percent of the total number of
wires.

3.3.7

Wire rope must also be replaced if it shows any of the following


conditions:

3.3.7.1 Evidence of heat damage;


3.3.7.2 Kinking, crushing, bird-caging, or other distortion in lines;
3.3.7.3 Six randomly distributed broken wires in any one lay of running
rope;
3.3.7.4 Three broken wires in any one strand of running rope;
3.3.7.5 More than two broken wires in any one lay of standing rope in
sections beyond the end connections;
3.3.7.6 More than one broken wire in any one lay of standing rope at an
end connection; and
3.3.7.7 Individual outside wires worn down to 2/3 or less of their original
diameter.
3.3.8

Wire ropes must be replaced if they are worn down more than the
amounts specified below.

3.3.9

When two wires are broken or rust or corrosion is found adjacent to a


socket or end fitting, the wire rope will be removed from service or
resocketed. Special attention will be given to the inspection of end fittings
on boom support, pendants, and guy ropes.

3.3.10

Wire rope removed from service due to defects will be cut up or plainly
marked as unfit for further use as rigging.

3.3.11

Table H-20 must be used to determine the number and spacing of clips
when using U-bolt wire rope clips to form eyes. Please note that ONLY
drop forged, American made wire rope clamps are allowed to be used on
Haselden projects. The use of malleable clamps or non-American made
drop forged clamps is prohibited.

3.3.12

Wire rope clips attached with U-bolts will have the U-bolts on the dead or
short end of the rope. Retighten the clip nuts immediately after initial load
carrying use and at frequent intervals thereafter. U-bolts used for eye
splices must be applied so that the U section is in contact with the dead
end of the rope.

3.3.13

If a wedge socket fastening is used, the dead or short end of the wire
rope will have a clip attached to it or looped back and secured by a clip.
The clip will not be attached directly to the live end.

Natural and Synthetic Fiber Rope


3.4.1

Tables H-15 through H-18 prescribe safe working loads for natural rope
and synthetic fiber slings.

3.4.2

Use of wet frozen ropes and slings is prohibited. Dry, fiber rope slings
may be used in a temperature range from minus 20 degrees to plus
180 degree Fahrenheit without decreasing the working load limit.

3.4.3

Ropes that have been subjected to acids or excessive heat will not be
used for load carrying.

3.4.4

Fiber rope slings must be replaced if they show abnormal wear;


powdered fiber between strands; broken or cut fibers; variations in the
size or roundness of strands; discoloration or rotting; or distortion of
hardware in the sling.

3.4.5

Fiber rope must be protected from cuts and abrasion by padding where
it is fastened or drawn over square corners or sharp or rough surfaces.

3.4.6

Splices in rope slings must be made in accordance with fiber rope


manufacturers' recommendations.

3.4.7

Eye splices in manila rope must contain at least three full tucks. Short
splices must contain at least six full tucks, three on each side of the
centerline of the splice.

3.4.8

Eye splices in layed synthetic fiber rope must contain at least four full
tucks. Short splices must contain at least eight full tucks, four on each
side of the centerline of the splice.

3.4.9

Strand end tails will not be trimmed flush with the rope surface,
immediately adjacent to the full tucks. This applies to both eye and
short splices and all types of fiber rope. For fiber ropes that are less
than 1 inch (2.5 cm) in diameter, the tails will project at least six rope
diameters beyond the last full tuck. For fiber ropes that are 1 inch (2.5
cm) in diameter or larger, tails must project at least 6 inches (15 cm)
beyond the last full tuck. Tails may be tapered and spliced into the body
of the rope using at least two additional tucks, if projecting tails are
objectionable. However, this will require a tail length of about six rope
diameters beyond the last full tuck.

3.4.10

Eyes in all eye splices must be large enough to provide an included


angle of 60 degrees or less at the splice, when the eye is placed over
the load or support.

Use of knots instead of splices is prohibited.

Slings
3.5.1

A sling is an assembly used for lifting that has its upper end connected
to the lifting mechanism and its lower end supporting the load that is to
be lifted. Slings and their fittings and fastenings will be inspected before
use on each shift and as necessary during use. Use of slings with knots
or kinks is prohibited.

3.5.2

Slings will be protected between the sling and any sharp, unyielding
surfaces of the load to be lifted. The use of slings will be such that the
entire load is positively secured. Slings must be set to avoid slippage.

3.5.3

Placing hands and fingers between the sling and its load is prohibited
while the sling is being tightened or is under tension.

3.5.4

Employees are prohibited from exceeding the rated capacity of any


sling.

3.5.5

A basket hitch involves passing the sling under the load to be lifted and
placing both ends of the sling on the hook or on a single master link.
Slings used in basket hitches must have the loads balanced to prevent
slippage. The term basket hitch means loading with the sling passed
under the load with both ends, end attachments, eyes, or handles on
the hook or a single master link.

3.5.6

Cable laid grommets, strand laid grommets, and endless slings must
have a minimum circumference length that is 96 times their body
diameter.

3.5.7

Fiber core wire rope slings of all grades must be removed from service
if exposed to temperatures of 200 degrees Fahrenheit or higher.

3.5.8

Synthetic web slings must be marked or coded to show the name or


trademark of manufacturer; rated capacities for the type of hitch; and
the type of material.

3.5.9

Synthetic webbing must be of uniform thickness and width and selvage


edges must not be split from the webbing's width. Fittings must be of a
minimum breaking strength equal to that of the sling and must be free
of sharp edges that could damage the webbing.

3.5.10

End fittings must be sewn onto the sling. Sewing must also be used to
form eyes in webbing. Thread must be in an even pattern with enough
stitches to develop the full breaking strength of the sling.

3.5.11

Synthetic web slings must be replaced if they show acid or caustic


burns; melting or charring of any part of the sling surface; broken or
worn stitches; distortion of fittings; snags, punctures, tears, or cuts.

Use of nylon web slings is prohibited where acid or phenolic fumes,


vapors, sprays, mists, or liquids are present. Use of polyester and
polypropylene web slings is prohibited where caustic fumes, vapors,
sprays, mist or liquids are present. Aluminum fittings are also prohibited
where caustics are present.

3.5.13

Polyester and nylon web slings are prohibited at temperatures of 180


degrees Fahrenheit and higher. Polypropylene web slings are
prohibited at temperatures of 150 degrees Fahrenheit and higher.

Rigging Hardware
3.6.1

Connections, fittings, fastenings and attachments used with rigging


must be of good quality, and of proper size and strength for the loads to
be lifted. These devices must be installed in accordance with their
manufacturers recommendations.

3.6.2

Drums, sheaves and pulleys must be smooth and free of surface


defects that may damage rigging. The ratio between the diameter of the
rigging and the drum, block, sheave or pulley tread diameter, must be
such that the rigging will adjust itself to the bend without excessive
wear, deformation or damage.

3.6.3

If drums, blocks, sheaves or pulleys are replaced during rigging and


hoisting operation, the diameter of the replacement devices must be at
least as large as the original devices. Smaller diameter drums, blocks,
sheaves or pulleys can only be used if the type of rigging and the safe
loading limits are changed to fit the size of the new devices.

3.6.4

Drums, sheaves or pulleys that have eccentric bores or cracked hubs,


spokes or flanges must be removed from service.

Shackles
3.7.1

Table H-19 must be used to determine the safe working loads of


shackles. Higher safe working loads are permissible when
recommended by the manufacturer for specific identifiable products. A
safety factor of 5 or greater must be maintained at all times. Eccentric
loading of shackles is prohibited.

Manufacturers' recommendations must be followed in determining safe


working loads for hooks. Where no applicable manufacturer's
recommendations are available for specific hooks, those hooks must be
tested to twice the intended safe working load before their initial use.
Written records of this testing must be maintained at the job site.

Drums
3.9.1

A sheave is the grooved wheel of a pulley or block over which rope or


cable is passed. Reeving is a rope system in which the rope travels
around drums and sheaves. Drums must have sufficient rope capacity
with the recommended rope size and reeving, to perform all hoisting
and lowering functions involved in each lifting activity.

3.9.2

At least three full wraps (not layers) of rope must remain on the drum at
all times. A clamp that is securely attached to the drum in an
arrangement approved by the manufacturer must anchor the drum end
of the rope.

3.9.3

Grooved drums must have the correct groove pitch for the diameter of
the rope. The depth of the groove must be correct for the diameter of
the rope.

3.9.4

The flanges on grooved drums must project beyond the last layer of
rope at least 2 inches (5 cm) or twice the diameter of the rope,
whichever is greater. The flanges on ungrooved drums must project
beyond the last layer of rope at least 2.5 inches (6.3 cm) or twice the
diameter of the rope, whichever is greater.

Sheaves
3.10.1

Sheaves must be compatible with the size of rope in use, as specified


by the manufacturer.

3.10.2

Sheaves must be inspected to ensure that they are the correct size, are
in good condition, and are properly aligned and lubricated.

3.10.3

Sheaves must be equipped with cable-keepers where the rope is


subject to riding or jumping off the sheave.

Eye Bolts
3.11.1

Employees are prohibited from loading shoulderless eyebolts at an


angle.

3.11.2

Eyebolts will only be loaded in the plain of the eye. They will not be
loaded at angles of less than 45 degrees from the horizontal.
