Shri Nirotilal Buddha, Sansthan (SNBS), Agra has been primarily working in the areas of health
and its determinants, safe drinking water, hygiene, sanitation, HIV/AIDS awareness, family planning &
welfare, child labor & literacy, education, livelihood, plantation, community awareness and development,
with especial focus on marginalized sections of the societies, slum dweller women and children through
various development programs, for the last 20 years. Besides, SNBS has been supporting DMHFW, Agra
closely.
SNBS has conducted Mapping and Assessment of Agra slums as key resource organization, jointly
facilitated by Department of Health and Family Welfare, Agra and USAID-EHP during February-April
2004. During this process, SNBS has conducted qualitative health vulnerability assessment in slums of
Agra and helped USAID-EHP and Department of Health and Family Welfare, Agra in plotting of slums
and existing health facilities on the map of Agra and gap identification. Thus, SNBS is one of the founder
partners of Urban Health Resource Center (UHRC) operation, formally known as EHP.
Subsequently, the Agra Urban Health Program (AUHP) has been implemented as response to
request of Government of Uttar Pradesh, suggesting UHRC to initiate PPP models in Agra. UHRC
initiated public-private partnership (PPP) process in consultation with the Department of Medical, Health
and Family Welfare, Agra. Based on its experience in the area of reproductive and child health, the
exceptional contribution rendered in proposal development phase and its credibility in the district, the
Screening Committee constituted under the chairpersonship of Chief Medical Officer, DMHFW, Agra
has recommended SNBS as potential NGO for partnership with UHRC operation and our partnership
came into existence.
Under this partnership, SNBS had mobilized approximately 40,250 populations in 20 slums and
facilitating outreach camps through linkage with Rakabganj North D-Type Health Centre.
Apart from this, since March 2006 SNBS had been facilitated a multi-stakeholder approach for
optimizing local resources and improving reach of services to approximately 46700 underserved urban
poor populations residing in 15 slums under Bundukatra D-Type Health Centre catchment area. Since
February 2009, SNBS has been demonstrating two approaches at Yamuna Par. Under one, SNBS has set
up and managing Urban Health Centre at Naraich, Yamuna Par for providing regular OPD and outreach
services to approximately 55000 population residing in 35 slums in Trans Yamuna area. The other
demonstrates community mobilization for demand generation of health services and community linkages
with Govt. health facility and mobilizing approximately 27000 populations in 9 slums and facilitating
outreach camps through linkage with government D-Type Health Centre, Yamuna Par. We are working in
80 slums of the Agra City under health programmes.
SNBS started 4 special child labor schools(with 200 child labor boy as well as girls students) from 1 st of
August 2005 in four different slums i.e. Nala Kaji Pada,Ram Nagar,Jagdishpura and Chipitola of Agra
with the objective of providing protection and education to working children(child labor) in coordination
with Labor Department, Agra under National Child Labor Project. Up till June,2013 a total of 742
children from Special Child Labor Schools of SNBS has been promoted and integrated with main stream
of education.
In continuation UHI-CARE funded Urban Health Initiative (UHI) program had been implemented
by SNBS since March,2011.Under this program eligible couples were counselled for adopting modern
family planning methods in our intervention area of 33 slums in Bunduktra. We covered population of
92984 with eligible couples of 14568 in 15521 house holds.We have counselled 14560 elibile couples and
3673 couple adopted modern methods so far i.e. 489 adopt Female sterilization,45 NSV,1419
IUCD,294,OCP and 624 Condom.

Along with UHI we are implementing USAID-PFI funded Health of the Urban Poor (HUP)
program in 24 slums of Agra city in the area of Jagdishpura,Bodla and Sikandra with population of 52263
in 10974 families since October,2011.We are covering 642 pregnant women,1140 infants and 7556
eligible couples in our intervention under HUP.We have formed 20 MAS in our intervention area to
strengthen the community for ownership.We have visited 113058 house holds for counseling in our
intervention i.e 6298 counseling visits to pregnant women, 5944 counseling visits to lactating
women,9126 visits to infants,20944 visits to 1-5 yrs children and 70476 visits to eligible couples.During
UHND we have facilitated beneficiries to avail health and nutrition services ever since we have facilitated
services to 714 pregnat women,1202 lactating women,1210 infants,2064 1-5 yrs children and 3782
eligible couples.
Despite this, SNBS has organized several international and national visits of USAID as well
as other organizations. On 18th April 2009 the visit of The United States Congress Delegation to
observe and understand the lives of urban poor and slum dwellers and their access to health services has
appreciated our work and devotion towards the disadvantaged community.
Most recently on 21th February 2012 the visit of The Ethiopian Delegation to observe and
understand the lives of urban poor and slum dwellers and their access to health services has acclaimed our
work.
Thus, SNBS have substantial experience in mobilizing and empowering communities for
accessing and utilizing better health care facilities and also have strong experience in working with state
and district level government counterparts for implementation of different projects and community
based activities.
SNBS at Kanpur :
SNBS will implement targetted intervention for Destination Migrant at Kanpur.Target population will be
10000 since February 2014 .We will provide them Information services i.e IPC,Mid media campaign
etc,Condom related services,Health care services i.e. STI treatement & management,health camps
etc,Treatement & care package and enabling environment services.

A.
The goal of the Agra Urban Health Program is to carry out demonstration and learning activities and
develop new interventions in urban slums that lead to synergistic participation of different government
officials in urban slums to improve water supply, sanitation and hygiene services to urban poor.
B.
SNBS is implementing the Agra Urban Health Program activities to reach out to underserved urban poor
population in Rakabganj North, Yamuna Par and Bundukatra area covering approximately 1,70,000
population. SNBS was selected as a partner NGO by a Screening Committee under the chairpersonship of
the Chief Medical Officer, Agra for implementation of the Agra Urban Health Program. The program
implementation activities were initiated in November 2005 with 14 slums (approximate population 29,000)
and gradually scaled up to cover 20 slums (40,250 population) in Rakabganj North area,15 slums with
46,700 population in Bundukatra area and 44 slums with 79,000 population in Yamuna Par area.
SNBS has conducted Mapping and Assessment of Agra slums as key resource organization, jointly

implementation from time to time.

C.
Key Activities:
Strengthen capacity of community link volunteers and Mahila Aarogya Samities to coordinate with
health service providers for outreach camps, tracking immunization coverage at slum level to minimize
leftouts and dropouts, and to promote health behaviours.
Strengthen capacity of the link volunteers and MAS members negotiate effectively with water and
sanitation service providers for the related services in their respective slums whenever required.
Established linkages with Govt. Health department for regular outreach sessions (including visits of
LMO for ANC checkups during these sessions).
Regular participation in health rallies and pulse polio campaign as and when requested by CMO and
encourage visits of CMO/Dy.CMO and other relevant Govt officials in the city/region to program slums
and program activities, seek feedback and support them in disseminating best practices in different
government meetings at city and region level.
Facilitated regular meetings of UHC level coordination committee to improve access of urban poor to
health services at Bundukatra and influence CMO/Dy.CMO to replicate such a multi-stakeholder approach
for optimizing local resources and improving reach of services to underserved urban poor population in
other UHCs areas.
Strengthen project staff capacity to collect, compile and analyze service coverage data and use it for
improving coverage rates, document program learning and share at appropriate forums.
Established linkages with other department like DUDA, Nagar Nigam, ICDS and education for
addressing other development issues in the intervention slums.
Facilitated program site visits of Govt. officials and non government organizations to observe and
understand the lives of urban poor and slum dwellers and their access to health services and learning
different program approaches to reach underserved urban poor population and its replication in other areas.

Community mobilization
SNBS has promoted 24 Mahila Arogya Samities (MAS) and also opened 19 bank accounts in the
Rakabganj area. The capacities (Institutional, Program, Linkage and Financial) of MAS have also been
strengthened. Capacity building sessions including exposure visits were also conducted both for MAS and
CLVs to develop their knowledge on the different components of RCH and health fund management. BCC
and IEC activities are regularly carried out in the slums to improve health and hygienic practices among
the slum community improved, and generate demand for services. In Bundukatra area CLVs were selected
and trained for providing community mobilization support.
Linkages with Govt. D-Type Health Centre (Rakabganj North) for health service provision
Demand generation activities were undertaken in all 35 slums. Services were strengthened in 20 slums
from DTHC Rakabganj which is a government run first tier facility. List of eligible beneficiaries updated
regularly, planned outreach clinics were organized and follow-up activities carried out especially in case of
left outs and resistant households. SNBS coordinated with CMO/ACMO and facilitated visit by these
Govt. officials successfully. SNBS developed linkages with the other stakeholders and local elected
persons by facilitating Coordination committee meeting. Services were also strengthened from the
Bundukatra DTHC to the 15 slums in the area. This was all achieved with support and mobilization of the
government.
Capacity building of the program staff
Capacity building sessions for MAS, CLVS, Community Organizers and ANMs were conducted to
enhance their skills on counseling and RCH components. Ongoing review meetings at various levels were
conducted to assess program progress, address program issues and concerns. Review feedback and inputs
from SNBS management, other stakeholders visiting the program and UHRC helped strengthen the
program.
Coordination with the government department
Coordination with district health department was given special focus from the program inception.
Regular coordination meetings were carried out right from DTHC level to District level for organizing
regular outreach sessions. District Health Department officials were invited to for providing capacity
building inputs to staff and assess community mobilization activities. The support to National Programs
continued.
Meetings of Coordination committee at Bundukatra were held with multiple stakeholders such as the
ward representative, CHV, Deputy CMO, AWW etc. Regular outreach clinics were held in 15 Slums in the
area. The SNBS staff also supported in routine immunization in Bundukatra. For improving service
coverage in catchments area regular meetings with coordination committee were organized.
D.
SNBS is implementing the Agra Urban Health Program to fulfill the aforementioned objectives, in 20 slum
neighborhoods (or bastis) in Rakabganj North area under Approach II, covering an approximate slum
population of 40,250, 44 slums in Yamuna Par under Approach I & II covering 79,186 population and 15
slums in Bundukatra under Approach III covering 46,695 population.
At Yamuna Par, SNBS is demonstrating two approaches under the Agra Urban Health Program (AUHP),

MAS member training:We planned one day training on March, 23, 2012 and March, 24, 2012 of the members of MAS. Members of
MAS from 7 and 8 slums participated respectively in the training. The period of the training was 9:30 am to
5:30 pm. Introduction of the project Health of The Urban Poor (HUP) was given by Mr. Ravi Kashyap,
President, SNBS in a detail to the participants. We discussed about the Goal, Objectives of the programme.
Ravi Kashyap took the session on Mahila Arogya Samiti (MAS).He built capacity of the participants on
role and responsibility of MAS and need to form the MAS.
QTR MAS meeting:We have planned 8 meeting in the month of March, 2012.The meetings were planned in Rajeev Nagar,
Mahaur Nagar, Geetapuram, Satnam Nagar, Vikram Nagar, Agra Ber, Nagla Ajita and Shyam Nagar. Points
for the discussion were:-objective to form MAS and benefits of MAS formation.

Community Meeting:We have organized 25 community meetings in the intervention area. Community was made aware of the
programme and its intervention. We discussed MCHN and WATSAN issues with the community women.
Participants in the meetings were app. 600.
World Water Day was celebrated on March, 22; 2012.We organized a rally to mark the day. The rally
covered 5 slums of our project area Nagla Ber, Nagla Ajeeta, Kishorepura, Jagdishpura and Ram Nagar
Rally was flagged off by Mr.Bahadur Singh, Local Ward Member. The rally was leaded by the Dhol and
National Flag. Band cart was following the procession. Link Workers and community women were holding
the slogan templetes.They were raising the slogans in the rally. The rally was started at 9:30 am. It was
started from Nagla Ajita and end up at Community Centre, Jagdishpura.Ms Vidhya Devi Ward Member
addressed the rally and hailed the community women to save water. Buddhist religious leader Vimal Sagar
drew attention for the griming situation of water availability. He emphasized on water conservation as the
water availability will be minimized in upcoming years. He hailed the participants to stop misuse of water.

MAS formation:We have formed 8 MAS in our intervention area in the month of March,12.The MAS were formed in Pathar
Ghoda,Rajeev Nagar,Nagla Ajeeta,Ram Nagar,Mahal,Kabir Nagar,Baipur and Bodla Sarai.
Resource mapping:We have organised stakeholders meetings in February and March,12 to prepare the resourse map of the
slum.Stakeholders from the different Govt. deptt and community members participated in the meetings.They
draw a map of the slum and display resources i.e. health centre,AWC,Panchayti bhawan,schools etc in the
map.We have organised 25 stakeholder meetings in the 25 slums and prepared a resource map of the slum.
Dr.Anupam Bhasker ACMO participated in the meeting.Health visitors,Mukhya Savika,Teachers,Sanitory
inspector and others stakeholders have participated in the meetings.
Review meeting of CCs and Link Workers:We have organised 4 review meetings during December,11 to March,2012 to analyse the progress of the
project activities. Programme activity plan of LW s on the monthly babis was prepared in the
meetings.Different programme activities were included i.e.Community meeting ,HH visit,MAS meeting.We
would ensure participation in the Community Health Day in the intervention area. We discussed about
organising community meetings,Identification of active women to form MAS. We have instructed all LWs
to open their bank account. Ms.Sunita Sharma,MS,ICDS participated in the meeting and share sector
meeting plan with the participants.

Participation in Community Health Day:Cluster Coordinators observed the 12 Community Health Days in the month of Dec,2011 to
March,2012.Vaccination of different vaccine was done by Health Visiters.IFA was distributed to the
preganant women.Health and Nutrition counselling was given by HV to pregnant women.
Link Workers Training:Link Workers three days training was organised on 28-30/12/2011.We have built capacity of LWs
regarding goal,objectives of HUP program.Ms.Pushpa Mahaur built their capacity on MAS and health
resource map.19 LWs and 3 CCs participated in the training.House hold survey format.Birth spacing survey
format,Pregnant and Infant survey formats were discussed in the training.

We have completed listing and Mapping of the all 25 slums in the month of November,2011.We have listed
8252 structures in these slums.Mapping of these slums has been completed.Listing and mapping was carried
out with the help of Link Workers.Slum boundries were decided with the stakeholders and local residents.
Mapping in process.
Catchment area mapping and defining:HUP staff had been given orientation and training in Agra organized by PFI in the month of November,
2011.They were oriented in the training on the goals and objectives of HUP program. The field visit
organized in the training was very fruitful as we got good understanding on listing and mapping, house hold
survey and health recourse mapping. Field exposure during the training was very effective to understand the
listing and mapping, HH survey to be carried out in the respective cities. We have marked the boundaries of
the slums and their catchment area.
House Hold Survey:We have surveyed 8252 structures and found population of 50475 in the all 25 slums.We found 9284
families,489 pregnant women,1051 0-1 yr children and 5768 eligible couples in the slums.The survey was
carried out in the month of December,11 and January,12.

We have selected all 20 link workers in the month of December,2011 and they have started their activities in
their respective slums.We have divided them in three clusters.Two clusters have 7 link workers each and one
cluster have 6 link workers.
LWs selection process :- We are selecting the LWs through following steps :- Organise a community
meeting and gave introduction of the project .
Identify some volunteers to be LW through community meeting.
Short list the potential voluteers to be LW.
Finalise the volunteer as LW through community vote.

Stakeholder identification and Sector Meeting:We have identified all Health visitors and AWWs in our project areas.We have introduce our LW s to the
stake holders.Cluster coordinators have participated in the sector meetings. They have discussed about
Swasthya Diwas and listed the fix day of Immunization in the meeting.31 AWWs and LWs were present in
the meeting.Ms Sunita Naag,Sector Supervisor,ICDS was heading the meeting.
Project staff identified the stakeholders during their field visit.25 AWCs, 5 ward members are in the project
area. We have visited local health service providers, Aagan Wadi Centres, Ward members and other local
leaders to seek their support in the program. We visited DTHC Hari Parwat to develop linkages with the
govt. Counterparts.

We have participated in MAS meeting and built their capacity on high risk pregnancy, Ante Natal Care
and identification of pregnancy and calculations of EDD, 5 Cleans during emergency, danger sign during
delivery, post natal care, birth planning, danger sign during pregnancy. We have built their capacity in all 20
MAS in our intervention area.

Community Meetings
We have organized 135 community meetings in the intervention area during the quarter. We discussed
MCHN and WATSAN issues with the community women. We discussed about new born care and Care
during pregnancy and ANC, PNC and INC.Participants in the meetings were app. 3390.

Workshop on Nutrition status in the community:-100 Participants from the different stakeholders AWWs
and community people& Link Workers participated in the workshop. Members from the Mahila Arogya
Samiti (MAS) participated in the workshop.

Film Show: - Films regarding breast feeding and nutrition status & nutritional practices were shown in the
event. The message of the movie was we can prevent malnutrion in the mother and child by taking
preventives measures and promote breast feeding.
Another film Child Crawling was shown to address their misperception regarding early breast
feeding and colostrum feeding.
Quiz Competition:We organized quiz competitions in the above slums. 10 Questions related with breast feeding to be answered
by the participants. Participants were from MAS members and community women.10 winners were selected
from the each competition. Gifts were given to them.
Rewarding of the winners: - We asked questions in the competition to the audience and appropriate
answers were gifted with prizes. We asked 10 questions in the each event and gifted 10 participants for the
right answer. Total 40 women were rewarded in the event.
We made aware app 500 people from the intervention area regarding breast feeding and exclusive breast
feeding.
Health Camp (Private) :We have organized 8 health camps in the quarter on 9th of every month to provide health care and Ante Natal
Care to the pregnant women of the community. The camp was organized with the support of Rainbow
Hospital Pvt.Ltd, Agra. The camp was organized at Rainbow Hospital, Sikandra, Agra. The camp was
started at 11:00 am. The camp was end up at 1:00 pm.
Ante Natal Checkups(ANC)
Dr. Narendra Malhotra, Professor and Dr.Parul Mittal, MD, diagnosed the pregnant women. 19
pregnant women provided ANC.

Pathological Services
Pathologists examine the pregnant women for BMD.BMI and Hb testing. Weighting of the pregnant
women was taken at the pathology counter. Blood Pressure of the pregnant women was taken at the
counters.

Health and Nutrition counselling was given by HV to pregnant women and 0-1 yr child.
Adolescent girls were also vaccinated for TT.

Three days training was organized at Moti Palace,Agra on Nutrition for Link Workers on January,
10-12,2013. Dr.Anupam Bhasker,District Immunization Officer inaugurated the training with lightening
the lamps before Lord Budhha. Dr. Bhasker gave details of the upcoming measles campaign and appreciated
our LWs and CCs efforts for improving immunization coverage in the intervention area. Mr. Rajeev
Kumar facilitated session on concept of Health, Food, Nutrition and Nutrient. He elaborates Health Health
is a state of complete physical, mental and social well being and not merely the absence of disease and
infirmity. He gave details on classification of food.
Stakeholder Sensitization
We have organized a stakeholder sensitization meeting at ICDS office of Agra city, Project on May,
24, 2012.We discussed regarding AWW one day orientation and selection of AWC for Aan Prasan and God
Bharai celebration. Five AWCs were selected for the event. The AWCs were Kishorepura, Nagla Ajita,
RamNagar, Satnam Nagar and Nai Awadi Bodla. The celebration will be organized on every IIIrd Thursday
of the month.

AWW orientation
We have organized one day orientation of AWWs on May, 30, 2012 at Awadh Banquet hall, Sanjay Place,
Agra.Chief Guest to inaugurate the orientation was Dr.Anupam Bhasker; District Immunization Officer
(DIO).Guest of Honor was Ms.Kamlesh Gupta, District Programme Office (DPO), ICDS.We gave
introduction of the program and SNBS.

Film Show
We have 25 slums in our intervention area so we planned 8 film shows in these slums. We have
covered 3 slums in a show therefore we have covered 24 slums of the intervention area in these shows. The
shows were started since April, 2012 and end up on September, 2012.One Show was planned in one day.
The show started at 11:00 am. The film was based on Pregnant and new born care. Different messages were
given in film. Total 82 film shows were organized in the year. We asked questions in between the film show
to the audience and appropriate answers were gifted with prizes. We asked questions in the all film shows
and gifted 5 women each show for the right answer. Total 410 women were rewarded in the event.

Healthy Baby Show:LWs, AWW and MAS members identified the participants for the show. Distt Health and Family welfare
department and ICDS were coordinated to provide support during the show.
9 AWWs named Ms. Anita, Madhupesh, Savitri, Rajeswari, Tulsi, Lalitesh, Anita, Shiva and Premlata
of the areas were participated in the show and facilitated the contestants. We have weighing scales for
weighing of the contestants. Contestants were screened at slum level by LWs in primary stage. Then
shortlisted were come to the show. All 21 contestants were weighted.
Criteria for the identification of healthy baby:We had laid down 3 criteria for the contestants.The criterias were:
Total 21 children participate in the show. The immunization cards were checked for assessing their
immunization status. Three children were selected as per criteria and weight as per age.
The committee of Dr. Rakesh Agnihotri, Ms. Harpyari, Ms.Dulari, HV and Ms.Anita, AWW decided the
winners of the show.

Exposure visit:Jaipur HUP city team had exposure visit in our intervention area on 20-21,September,2012.They visited
Saraswati MAS,Nagla Ajita and Dr.B.R.Ambedkar MAS,Jagdishpura during the visit to understand risk
pooling carried out by the MAS.

Healthy baby show was organized in the celebration to aware the community regarding infant care.
Criteria for the identification of healthy baby:We had laid down 3 criteria for the contestants.The criterias were:-

We have organized 5 UHND sessions with support of Deptt. Of Social and Preventive Medicine,

MAS sammelan
MAS sammelan was organized on March, 08, 2013.The sammelan will be organized at Panchsheel
Community Hall, Nagla Ajita. We planned the event two weeks in advance of the day. Govt.
stakeholders were informed regarding the event. Integrated Child Development Services schemes
(ICDS), Distt. Health and Family Welfare Deptt. and others were informed through letters prior to the
event. Programme of the activities was shared with CMO, ACMO, DPMU, DPO, CDPO and other
government officials. They instruct their staff as per plan. We invite all the MAS members to participate in
the sammelan. All the 24 slums of the intervention area were covered in the event. The sammelan will be
organized from 10:00 am to 4:00 pm. The participants were 280.
A rally was organized to generate awareness among the community regarding women empowerment and
roles and responsibilities of MAS. 5 slums of Agra city would be covered by the rally namely are Nagla

Ajita, Ram Nagar, Nagla Ber, Kishorepura and Jagdishpura. MAS members will participate in the rally. The
rally had app. 250 participants. The rally started at 8:30am and end at 10:00am.
MAS, performing well were rewarded in the sammelan. Excellent performing link workers were rewarded in
the sammelan as well. Gifts were distributed as per their performance.3 MAS and 3 Link Workers were
rewarded in the event. We honored 5 women i.e. Dr. Nirmla Yadav, CMO,Dr. Renu Agarwal, Assit.
Professor, SPM, Ms. Harpyari and Ms. Dulari, Health Visitor and Ms. Sharda Vimal for their efforts of
women empowerment.

We have organized 35 ceremonies in the year in slums of the Agra City. The slums covered were Ram

BBSR team of 17 persons had exposure visit in our intervention slum Kishorepura and Ram Nagar. UHND

the Valakappu is the equivalent of the God Bharai. In
Hindi God Bharai literally means to fill the lap with
abundance.GOD BARAI is doing When a pregnant
woman has completed seven months of gestation
period, the baby is assumed to be viable and in a safe
phase. So, the celebration is usually at the end of the
seventh month or in the eighth month to welcome the
little one to the family and literally fill the motherto-bes lap with gifts and fruits to bless her with the
abundant joys of motherhood.

its all staff and Women Health Samities has organized the Health camps, awareness programs to
spread this message.
The overall impact of Awareness Program was very positive. People have decided to go to the
hospitals for regular antenatal checkups and delivery, after attending the awareness programs.
