PETITION
COMES NOW the petitioner through the undersigned counsel, and
unto the Honorable Court most respectfully alleges and avers:
1. That Petitioner is of legal age, single and a resident of Afternoon
Glory, Talon-Talon, Zamboanga City, where she may be served
with summons and other court processes;
2. That Public Respondent is the office in-charge of the registration
and correction of entries where the fact of birth of the petitioner
was registered, which registrar is located at Ipil, Zamboanga
Sibugay, where it may be served with summons and other court
processes;
3. That Petitioner was born on January 2, 1994 at Ipil, Zamboanga
Sibugay as evidenced by a photocopy of a Certificate of Live
Birth hereto attached as Annex A;

4. That at the time of Petitioners birth and up until at present, her

Certificate of Live birth was issued bearing the name VALERIE
ANNE PALMA SALES by the Civil Registry of Zamboanga City;
5. That in the said Certificate of Live Birth of herein Petitioner, what
was entered and what appeared therein as regard her first name was
VALERIE ANNE instead of VALE ANNA her true and
correct first name;
6. That petitioner has been known by her parents, friends,
acquaintances, relatives, neighbors in the community, school, and
in any other places as VALE ANNA and not VALERIE
ANNE as shown in her Certificate of Baptism and Official
Transcript of Record hereto attached as ANNEXES B and C
respectively;
7. That it was only last December of 2014 that petitioner learned and
discovered the wrong entries as to her First Name as appearing in
her Certificate of Live Birth, when she secured a copy of the same
from the National Statistics Office;
8. That petitioner has likewise secured pertinent clearances from the
Philippine National Police, the Office of the City Prosecutor,
the Office of the Regional Trial Court Clerk of Court and the
Office of the Municipal Trial Court Clerk of Court showing that
she has no civil, criminal or any administrative case or any pending
case for that matter, copies of said clearances are hereto attached
and marked as ANNEXES D, E F and G, respectively.
To further prove that indeed she has no pending cases in the whole
country, a photocopy of a Record Clearance from the National
Bureau of Investigation is likewise attached in this petition as
ANNEX H;
9. That in order to avoid confusion as to the Petitioners first name,
she has therefore filed this petition, for the correction of the entry
as appearing in her Certificate of Live Birth as to her First Name
from VALERIE ANNE to VALE ANNA.
P R AY E R
WHEREFORE, it is most respectfully prayed of this Honorable
Court, that after due notice, publication and hearing, an Order be issued
ordering the Office of the City Civil Registrar of Zamboanga, to effect the
correction of the entry as appearing in the petitioners Certificate of Live
Birth as to her first name from VALERIE ANNE to VALE ANNA and
to issue to the petitioner a new Certificate of Live Birth with the said
correction.

Petitioner prays for such other reliefs that are just and equitable under
the premises.

am the Petitioner in this case; I have personally caused the preparation of the
foregoing Petition; I have read and understood all the allegations therein; all
the contents thereof are true and correct of my own knowledge and
information; I have not commenced any other action or proceedings
involving the same issues subject of this case in the Supreme Court, Court of
Appeals, or any other tribunals or agency, and, to the best of my knowledge,
no such action or proceeding is pending in the Supreme Court, Court of
Appeals, or any other tribunal or agency; and that if I should hereafter learn
that a similar action or proceedings has been filed with or is pending before
the Supreme Court, Court of Appeals, or any other tribunal or agency, I
undertake to report said fact to this Honorable Court within five (5) days
from knowledge thereof.
