Assessment / Introductory Phase


Good day everyone! Im Janne Chlarisse Vega Gonzales, but known by the name Jane,
17 years old with a height of 158 centimeters, waist line of 32 inches and a weight of 55
kilograms. Im a college student from Pamantasan ng Lungsod ng Maynila, College of Nursing.
I have so many things to change in terms of my lifestyle. I always want to eat, I dont
have much exercise before, but now, I do exercise like swimming and chess. I do my leisure
exercise when I got my free time. Since, I just have not enough free time, my body tends to
become big and fatty. I always sit, and I always want to study. My preparation for doing this
wellness journal, I prepared a lot of books and websites that will help me complete and
accomplish my wellness project, because I know that this plan will not just for my academic
thing, for my first goal, it will help my body become fit and health. For my second goal, this will
improve my study habits. As I imagine the result, this keeps me motivated that I will become
physically and mentally fit.
