APPLICATION FOR ADMISSION

The Starmark Academy for Professional Dog Trainers, Inc. (the Academy) is dedicated to
developing professionals in the area of dog training. While the course of study is rigorous and
demanding, these characteristics are a prerequisite for the Academy to reach its goal of providing
its graduates with the skills needed for a successful career in the dog training industry.
GENERAL INFORMATION
The Academy is owned and operated by Triple Crown Dog Academy, Inc., a Texas corporation,
and is located at 200 County Road 197, Hutto, Texas.
The purpose of this Application is to provide the Academy with certain information concerning
your qualifications and career aspirations, to provide information regarding admission
procedures and requirements, and to provide the terms and conditions of admission into the
Academy.
The Academy reserves the right at any time to add to or delete from programs of study, to update
classes, to make faculty changes, to change textbooks and training and teaching methods. The
Academy specifically reserves the right to adopt, update or amend any policies, rules or
regulations as the Academy may deem appropriate in its sole discretion. Upon adoption or
amendment, each student agrees to be bound by, and to comply with, all policies, rules and
regulations of the Academy.

affiliation, or beliefs in administration of its admissions policies, educational policies and
programs.

ADMISSION PROCEDURES
Each student must be 18 years of age, and must be a high school graduate or hold a General
Education Development (GED) certificate. Each prospective student must provide the Academy
with either a copy of his or her high school or college transcript, or an official copy of his or her
GED certificate. To ensure the safety of Academy staff and students, each prospective student
must also provide the Academy with an official NCIC Criminal History. The typical cost for a
criminal history check is $18 and can be obtained by acquiring a set of fingerprints at your local
law enforcement agency and submitting them to the FBI. For more information on obtaining

Each prospective student is required to
(i)
Complete an application on this form or other such form as may from time to
time be required by the Academy,
(ii)
Submit an $50 application fee and $500 refundable tuition deposit,
(iii)
Submit other required information and materials (including his or her high
school or college transcript or GED certificate; entrance essay, NCIC Criminal
History, two letters of reference that are work or character related from
someone other than a friend or family member), and
(iv)
Participate in a phone interview with a representative of the Academy.
Upon completion of the foregoing a prospective student will be considered for admission. Each
prospective student will be notified as to whether he or she has been accepted for admission to
the Academy.
The Academy reserves the right to reject an applicant for any reason.
Former students who wish to re-enter the Academy (including those who have withdrawn,
whether for good cause or no cause, or who have been terminated) must submit an updated
application for admission along with a new application fee and all other required information. All
re-entry students must have his or her previous Academy transcript updated and evaluated to
determine which program components have been successfully completed and will apply to his or
her current program of study.
Academy students who have completed a program of study are allowed to continue their
Academy education for the purpose of completing other programs of study upon submission of
an updated application, an application fee for the new program of study, transcripts or certificates
evidencing completion of prior programs of study, and all other required information. An
interview may be required, in the discretion of the Director.
