Narrative of Offense Committed
On or about the above stated date and time, the Defendant did

operate a motor vehicle by one or more of the following means:

a. While under the influence of an alcoholic beverage or drugs or a combination of
such substances; c. while any amount of a controlled substance is present in the person as measured in the person's blood or urine; and having been
convicted of/received a deferred judgment for the crime once before; the said defendant having been convicted of/received a deferred judgment for the
crime of Operating While Intoxicated in Buena Vista County, Iowa

AFFIDAVIT

I, the undersigned, being duly sworn, state that all facts contained in this Complaint and Affidavit, known by me or told to me by other reliable persons form the basis for my
belief that the defendant committed this crime
State all facts and persons relied upon supporting elements of alleged crime

On February 25, 2015 at approximately 10:03PM I was on routine patrol when I observed a red Dodge Intrepid bearing IA plate BBS793 traveling west
on D42. While traveling behind the vehicle I observed the vehicle cross the center line on 4 different occasions. The vehicle also traveled at a speed
between 45-50 miles an hour. I then activated my emergency lights at this time the vehicle pulled over on the north side of the road. I then ran the
Tommy Hammond through vehicle field sobriety tests. I then contacted a DRE who conducted more tests on Tommy. Tommy was placed under arrest
for operating while under the influence.
