DEFINITIONS
1.1
1.2
Orientation means an information session that has been developed by the Commission,
Contractor or Employer to inform the workers of hazards and operating constraints in the
area of work; approximate duration ~ hour to 2 hours.
1.3
Training means attendance in a formal training class and passing a test. This term applies
to Subway/SRT Rule Book, Surface Rule Book and Work Protection Code Training;
duration varies.
1.4
Commission's Program includes but is nat limited to Subway Rule Book Training, Surface
Rule Book Training, Work Protection Code Training, Ladder Safety Policy, Site Service
Isolation, and Substation Awareness.
1.5
Hot Work means work that involves open flames or work that produces heat or sparks.
Such operations include, but are not limited to, welding, brazing, cutting, grinding, soldering,
thawing pipes, torch-applied roofing and cad welding.
1.6
Fire Watch is a person trained in the use of fire suppression equipment and alarm
activation. Duties include monitoring the work area by observation and inspection, during
and after the Hot Work, to identify fire hazards and respond appropriately. A Fire Watch is
required for all Hot Work. While on Fire Watch duties, no other task shall be performed.

Commission property is defined as any part of the TIC system that is in, or is ready for
revenue service.
CONSTRUCTOR
2.1
For the purpose of the Contract the term "Constructor" as defined in the Occupational
Health and Safety Act, shall mean the Contractor. The Contractor shall ensure that
provisions of acts, regulations and by-laws pertaining to the duties, obligations and safe
performance of the Work in accordance with the obligations of the "Constructor" as set out
in the Occupational Health and Safety Act are observed.
2.2
Whether the Commission or the Contractor is the Constructor, the Contractor shall provide
in writing the name of the Contractor's representative who will be named on the "Notice of
Project" as the "Supervisor in charge of the Project". The Contractor's representative shall
assume the duties of the Constructor and ensure that these responsibilities are fulfilled on
the Site.

The Contractor shall report any "Notice of Injury or Occurrence" made under Section 51, 52
and 53 of the Occupational Health Act or Section 43 "Right to Refuse Work". This shall
include incidents as described in Regulation 11 for the purpose of Section 53 of the Act.

Quality Assurance:

Supervisor's and Safety Officer's Qualifications: Supervisor and Safety Officer to have
successfully completed Supervisory Basics training or be WSIB Certified.
CONTRACTOR'S RESPONSIBILITIES
4.1
The Contractor shall ensure that the following acts and regulations are followed on the Site:

Before beginning Work at the Site each Employer shall complete Ministry of Labour form
1000. Each Employer at the Site must provide a copy to the Contractor and Commission's
Representative.
4.3
For Work on Commission property, ensure that the TIC's Site Service Isolation work
instruction {WI-45) is reviewed with all Workers and complied with on the Site.
4.4
Ensure that adequate control procedures are developed and implemented on the Site for all
sources of energy including adequate lockout procedures for the protection of the Workers.
4.5
Provide a safe workplace, using safe equipment and materials, including non-conductive
ladders.
4.6
Immediately correct unsafe conditions. Discuss weekly Tool Box Talks and implement a
permit procedure for Confined Space, Lock Out and Tagging and Hot Work.
4. 7
Establish and maintain a written Site specific safety program and Site specific plans
addressing such items as emergency procedures, contact personnel, traffic control plan
and fall protection plan.
4.8
Develop a Safe Work Method for all high risk/energy Work which varies from established
Contractor and Commission procedures.
4.9
Inspect the workplace regularly which shall be subject to site AUDIT from Commission's
Representative once a month and reviewed on a weekly basis. Both will be documented.
4.10
Investigate and report accidents and incidents, including near misses, to the Commission's
Representative.
4.11
Ensure necessary training of all Workers such as fall protection, lock and tag, first aid,
WHMIS, traffic control and propane and maintain evidence of training or recertification on
the Site.
4.12
Prior to access to Commission facilities the Contractor and Workers may be required to
participate in "Commission Programs" when required.
4.13
Ensure all Subcontractors have adequate safety programs and training for their Workers.
4.14
4.15
Establish a procedure for workers and visitors entering the Site. Ensure that workers and
visitors wear a clearly visible and proper identification. Security measures shall be Site
Specific.
4.16
Provide a site specific orientation that includes, but not limited to site safety, site specific
hazards, incident reporting, emergency procedure and reporting hazards.
4.17
Provide the Contractor's Health and Safety Program to anyone entering the Site including
new workers, Subcontractors, suppliers and visitors.

Ensure construction

Ensure that the health and safety of the general public, employees of the Commission and
workers are protected in relation to the work performed on the Site.
4.21

The Contractor shall assign a full time "Competent Safety Person" to assist the Contractor's
representative in discharging of safety responsibility, at no additional cost to the
Commission.
The Commission's Representative reserves the right to review the
qualifications of the selected "Competent Safety Person".
4.23

Failure to comply with the occupational health and safety legislation or procedures detailed
in the Contractor's health and safety program including site specific procedures and
programs designed to prevent incidents may be cause for the issuance of a stop work order
until the contravention has been corrected. All costs associated with the stop work order
are at the expense of the Contractor. Claims for the delay will not be accepted by the
Commission.
4.25
The Contractor shall monitor all working areas and conditions for any harmful gasses
and/or oxygen deficiency as per applicable regulations.
PRODUCTS
5.1
Throughout the duration of the Contract, maintain on Site and make readily available to
anyone on Site, Material Safety Data Sheets (MSDS).

Install a bulletin board to be known as the "Project Safety Bulletin Board" on the project.
The "Project Safety Bulletin Board' is for the purpose of posting all information considered
mandatory posting material by the Occupational Health and Safety Act, the Regulations for
Construction Projects, Workplace Safety and Insurance Board Notices any other
information pertaining to safety associated with the Site.
Designated Assembly Area:

Designate an area on the Site as a "Designated Assembly Area" where Workers are to
assemble in the event of an emergency.

The Designated Assembly Area shall be identified with a 900 mm x 900 mm plywood sign,
painted Yellow with Red lettering at least 150 mm high with a stroke of 50 mm with the
words "Company Name - Designated Assembly Area". If the project is of a size and nature
that it requires more than one "Designated Assembly Area", the areas shall be identified in
the plan referred to above, and in addition labelled "A", "B", "C", etc., as deemed
necessary.

The "Designated Assembly Area" sign shall be mounted on a fence weighted stand, side of
a building as deemed necessary in the circumstances.
7.1
Submit completed Site Service Isolation forms ECF 0309-29 and ECF 0309-30 and
Lock outrrag-out/lsolation procedures to the Commission's Representative prior to
commencing excavation.
UTILITY LOCATES
8.1
The accuracy of the underground utilities information shown on the Contract Drawings is
not guaranteed by the Commission. Do not proceed with excavation until utility locates of
all buried or concealed services are complete .

For Work on public road allowances and on other property, arrange for a utility locate and
locate report from each utility authority that potentially has services in the vicinity of the
proposed Work unless instructed otherwise. Provide the Commission's Representative
with each locate report and follow all orders identified in the locate reports. Provide forms
ECF0309-29 and ECF0309-30 completed as appropriate.

In the event that an accurate locate cannot be provided, excavation must be done by hand
until undisturbed ground is reached prior to commencing excavation, or by other
non-destructive excavation techniques as approved in writing by the owner of the utility .

When the locates are complete, update as-built drawings as specified in Section 01 78 39
showing exposed utilities and return to Commission's Representative.

In the event that an accurate locate cannot be provided, use hand excavation or other

commencing drilling activities .
When exposing any concealed utility within 3 m of its estimated exterior surface or
specified by the Utility Authority who performed the locate .
If there is another utility within 3 m of the utility being worked on, use hand excavation
or other non-destructive excavation techniques until undisturbed ground is reached
prior to commencing drilling activities .
If Work in the vicinity of the locate does not occur by the expiry date in the locate
report, arrange to acquire a new locate.
9.1
Within ten (10) days of receiving the forms and utility locate report (if applicable), the
Commission's Representative will arrange an on-Site meeting with the Contractor and
Commission's staff to identify and verify the service(s) requested to be isolated.
9.2
When all isolations are complete the Commission's staff will Lock-out/Tag-out the service(s)
by installing a multi-lock hasp and sign-off on the Site Service Isolation form.
9.3
Once the Contractor installs the locks and tags and the Site is deemed safe, the
Commission's Representative can authorize the Work to proceed.

10.1
When the isolation is no longer required and the Contractor deems it is safe to restore the
isolation, the Contractor shall remove the locks/tags and signs and shall surrender the Site
Service Isolation form to the Commission's Representative.
10.2
Once the Site Service Isolation form has been surrendered, the service shall be considered
live.
10.3
The Contractor shall contact the Commission's Representative to restore the services; the
Construction Inspector shall sign the restoration order.
10.4
Commission's staff shall restore the services according to the TIC procedures and shall
sign the restoration order.
10.5
Forward the original documents to the Commission's Representative for filing and archiving.

HOTWORK
11.1
Prior to performing any Hot Work, in an existing Commission facility, notify the
Commission's Representative minimum five (5) days in advance to obtain a Hot Work
Permit.

Prior to issuance of a Hot Work Permit, complete and submit the following forms to the
Commission's Representative:

The use of propane, natural gas, or gasoline fuelled engines underground is prohibited.
12.2
Use diesel engines only with appropriate scrubbers and exhaust controls.
12.3
In excavation, unless provision is made to ensure that exhaust gases will not accumulate in
excavation .

Unless exhaust gases and fumes from engines are discharged directly outside
building or structure to point sufficiently remote to prevent return of gases .
Unless there is an adequate supply of air for combustion and adequate natural or
mechanical ventilation to ensure exhaust gases will not accumulate .
Unless continuous air monitoring for carbon monoxide and nitrogen dioxide is
performed for the duration of the Work.

FIRE PROTECTION
13.1

In enclosed spaces, supply and install heat detectors, wiring and end of line resistors on the
construction side of the hoarding and dust-tight partitions. The number of heat detectors
shall be as required by the code. Interconnect the heat detectors and the end of the line
resistors to the existing fire alarm panel in each station. After the Work is completed,
remove the heat detectors, end of line resistors and its wiring. Install heat detectors in
accordance with CAN/ULC S524-M, Standard for the Installation of Fire Alarm Systems.

Bulk storage of flammable liquids and other hazardous materials is not allowed on Site.
-Supplies for daily use only are permitted. Flammable liquids must be handled in approved
containers that are labelled in accordance with WHMIS and maintained in good order
complete with all spouts, caps and gaskets. Keep containers for flammable liquids closed
at all times when not directly in use. Empty containers are dangerous and must be kept
with their caps, spouts and gaskets in place.

The bringing in, use and disposal of gasoline, benzene or other flammable materials shall
be handled with good and safe practice as required by authorities having jurisdiction.
13.6
Provide fire extinguishers of the non-freezing chemical type in each temporary building,
enclosure and trailer. Every fire extinguisher shall be inspected for defects or deterioration
at least once a month by a competent worker who shall record the date of inspection on a
tag attached to the extinguisher.

Subway Rule Book Training and annual Retraining is mandatory; duration one (1) day.

Persons attending Subway/SRT Rule Book and Surface Rule Book Training must present a
photo ID for admission to the course. They must pass a written test for certification. The
course and test are given in English only. Anyone failing any test or an unauthorized
absence will not be permitted to rewrite for two (2) years .

Work Protection Code Training (for isolation of work within the vicinity of equipment
operating at 750 volts AC or above). For workers who may be required to be present
where work protection is present or used; Work Group Member Training, duration half (1/2)
a day. Retraining required every three (3) years .

Testing and Commissioning Rules and Procedures Training; duration three (3) hours .

Testing and Commissioning Rules and Procedures will be in force following energization of
the traction power rails. Contractor personnel on-site after this date will require this
training.
14.2
The Contractor shall notify the Commission's Representative with the requirements for
training, retraining, awareness, orientation and familiarization twenty-eight (28) calendar
days in advance of such requirements. Include the Employer, worker name and Union
affiliation on the submission. The Commission will issue confirmation of such requirements
fourteen (14) days in advance of the Training, Retraining, Awareness, Orientation and
Familiarization dates.
Re-scheduling dates will be subject to the Commission's
Representative's discretion.
14.3
Re-scheduling dates without a minimum of one (1) weeks' prior notice will be subject to a
re-scheduling fee of $250.00, payable to the Commission. Unauthorized absence will be
considered a failure and the trainee will not be permitted to rewrite for a period of two (2)
years.

15
DUE DILIGENCE
15.1
If in the opinion of the Commission, the health and safety of a person or persons is
endangered or the system put in place to ensure the health and safety of Workers on Site is
not being implemented, the Commission may take action as it deems necessary and
appropriate in the circumstance, including but without limiting to, the following:

Require the Site to be shut down in whole or in part until such time as condition has been
remedied .
.3
Remedy the problem and the Commission will back-charge the Contractor for the cost of
remedial work, together with an appropriate overhead charge as determined by the
Commission in its sole discretion, and
.4
The Commission may terminate the Contract without further liability in the event the
Contractor fails to comply with these provisions.
16
16.1
Contractor shall establish the operation of the Joint Health and Safety Committee as
required by the Occupational Health and Safety Act. The Commissission's Representative
reserves the right to issue the minutes to the participants.

The scope of this document outlines the steps to be taken by Construction Management when
arranging for the isolation of services prior to work (surface penetration, excavation, coring,
cutting, drilling etc.) being performed. The document covers the process of utility locates and
isolation of services during the commission of work.
The purpose of this work instruction is to minimize the risk of accidental or unexpected contact
or exposure to hazardous energy, conformance to the requirements of the applicable
procedures, and ensure that affected parties are consulted during the isolation and restoration
of services.
Isolation of services is performed by the Operations Branch in accordance with that
Departments' procedures.
The location and isolation of concealed or surface mounted services must be performed prior to
any work commencing.

WORK STEPS
UTILITY LOCATE
3.1.1 Prior to this process the Constructor will provide to the TTC the Constructor's Lockout/Tag-out/Isolation procedures. The TTC Construction Inspector will supply the Contractor
with blank Site Service Isolation forms.
If there are no surface penetrations (excavating, drilling, cutting, coring, etc.), the
Constructor may request a Service Isolation of services near the work area; continue at
step 3.2.1.
3.1.2 The Constructor contacts the Construction Inspector requesting a utility locate on TTC
properties and provides the following information to the Construction Inspector:
3.1.3 The Construction Inspector contacts the Locate Service provider and arranges an on-site
meeting with the Locate Service representative and the Constructor. The Construction Inspector
forwards the information received from the Constructor to the Locate Service representative for
review prior to the meeting.
3.1.4 The Construction Inspector, Constructor and Locate Service representative meet at the
work site to review the relevant drawings and scope of work.
3.1.5 When the utilities have been clearly marked and locates completed, the Locate Service
provider produces a report and reviews the findings with the Constructor and the Construction
Inspector; the Locate Report is then signed, and a copy retained, by the Constructor and the
Construction Inspector. The locate report must be renewed every 30 days.
3.2
SERVICE ISOLATION
3.2.1 When the Constructor is ready, they complete the Site Service Isolation form and forward
it to the Construction Inspector with a copy of the Locate Report (if applicable).
3.2.2 The Construction Inspector contacts the appropriate Operations Branch contact and
arranges an on-site meeting, giving 10 days notice, with the Constructor and appropriate
Operations Branch personnel.
3.2.3 The Construction Inspector, Constructor and appropriate Operations Branch personnel
meet at the work site. The scope of work, extent of service isolation and identification of the
service(s) to be isolated are discussed and verified.
3.2.4 The Operations Branch personnel perform the Service Isolations (as indicated by the
Constructor on the Service Isolation Form) according to the appropriate Departments
Procedures.

3.2.5 The Constructor and the Construction Inspector must witness any tests that confirm the
required services have been isolated.
3.2.6 When all isolations are completed the Operations Branch personnel Lock-out/Tag-out the
service by installing a multi-lock hasp and sign-off on the Site Service Isolation form which is
retained by the Constructor and the Construction Inspector.
3.2.7 The Constructor installs locks and tags as per their own submitted procedures.
NOTE: the Constructor may only install their locks/tags aher the Operations Branch
locks/tags have been installed; however, the Constructor may be present and witness
the installation of the Operations Branch locks/tags.
3.2.8 The Constructor authorizes work to proceed.
3.3
RESTORATION of SERVICES
3.3.1 When the isolation is no longer required and the Constructor deems it is safe to restore
the isolation, the Constructor removes their locks/tags and signs and surrenders the Site
Service Isolation form to the Construction Inspector.
NOTE: Once the Site Service Isolation form has been surrendered, the service is to be
considered live.
3.3.2 The Constructor contacts the Construction Inspector to restore the services; the
Construction Inspector signs the restoration order.
3.3.3 Operations Branch personnel restore the services according to the Operations Branch
procedures and sign the restoration order.
3.3.4 The original documents are forwarded to the Field Office Clerk for filing and archiving.

Cutting and welding equipment has been inspected to make sure bottles are secure.
Cylinders have been supported or located in such a way that they cannot be knocked over.

or ensure the work is moved to a

Where combustible materials, such as paper clippings, wood shavings, or textile fibres, are
on the floor, the floor has been swept clean for a radius of 15 metres. Combustible floors have
been kept wet, covered with damp sand, or protected by non-combustible or fire-retardant
shields. Where floors have been wet down, personnel operating arc welding or cutting equipment
are being protected from possible electric shock.
8.
All combustibles have been relocated at least 15 metres horizontally from the work site.
Where relocation is impractical, combustibles have been protected with fire-retardant covers or
otherwise shielded with metal or fire-retardant guards or curtains. Edges of covers at the floor

Openings or cracks in walls, floors, under locked doors, ducts or drains within 15 metres of
the site have been tightly covered with fire-retardant or non-combustible material to prevent the
passage of sparks to adjacent areas.
10. Where welding and cutting is done near a wall, partition, ceiling, or roof of combustible
construction, fire-retardant shields or guards have been provided to prevent ignition.
11. Where welding and cutting is to be done on a wall, partition, ceiling, or roof, precautions
have been taken to prevent ignition of combustibles on the other side by relocating combustibles.
Where it is impractical to relocate combustibles, a fire watch on the opposite side from the work
has been provided as described in {15).
12. Welding and cutting will not be attempted on a partition, wall, ceiling, or roof that has a
combustible covering or insulation, or on walls or partitions of combustible sandwich-type panel
construction.
13. Welding and cutting that is performed on pipes or other metal that is in contact with a
combustible wall, partition, ceiling, roof, or other combustibles will not be undertaken if the work is
close enough to cause ignition by conduction.
14. Fully charged and operable fire extinguisher{s) that are appropriate for the type of potential
fire are available immediately at the work area. A small water or garden type hose may be used
as a substitute for a class "A" fire extinguisher.
15. A FW is supplied as per the requirements stated in the Contract Documents. More than one
Fire Watch person will be supplied, if combustible materials that could be ignited cannot be
directly observed by the primary FW person.
16. Where heavy smoke is generated, additional measures will be taken to prevent it from
preading, such as venting it to the outside or using a smoke "eating" machine. If heavy smoke
cannot be avoided then the work will be performed when the building is not occupied.
Note: If necessary, request isolation of the fire alarm detection zone, fire alarm system, or,
sprinkler systems in the immediate area to prevent false alarms. If it is determined by TTC that
this is not possible or is impractical, the contractor shall use alternate methods as agreed with the
FOP.
