AFFIDAVIT OF DISCREPANCY
I, DAISY LACSINA PARAGILE, of legal age, married to FERNANDO P.
PARAGILE, citizen of the REPUBLIC OF THE PHILIPPINES and presently
residing at Forget me not St., Bansalan Davao del Sur, after having been
duly sworn to in accordance with law, do hereby depose and say:
That my name is DESIE LACINA, as stated and registered in my birth
certificate issued by the local civil registrar of Bansalan, Davao del Sur,
Republic of the Philippines, the country of my birth;
That during my childhood and adult years, I have been accustomed to
the use of the name DAISY LACSINA;
That I assumed the name DAISY LACSINA, when I signed official
documents;
That I am executing this Affidavit to clear any discrepancy which may
arise from official documents bearing either name DESIE LACINA or
DAISY LACSINA ;

JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS
We, CLEOFE GIL ENAJE and LILIOSA CAPUYAN CAA, both of legal
age, Filipinos, both married and residents of Poblacion Uno, Bansalan Davao
del Sur, after having been duly sworn to in accordance with law, hereby
depose and say that:
1. We are the name and identical persons who caused the execution of
this instrument;
2. We personally know DAISY LACSINA PARAGILE who was born on
May 26, 1967 at Bansalan, Davao del Sur to the spouses PEDRO
ROMANO LACSINA and FEDILA LAURENTE OBAS;
3. The names DAISY LACSINA PARAGILE per Voters ID and DAISY
LACSINA PARAGILE per SSS ID are the same person;
4. That we know of the aforementioned facts because we are
neighbours for a long time;
5. That we execute this Affidavit to inform the authorities concerned of
the aforementioned facts and for whatever legal purposes this may
serve;
6. That DAISY LACSINA PARAGILE is her true and correct name.
