OBJECTIVE
I am applying for the position of orthopaedic doctor of Andrews Sports Medicine and Orthopaedic Center in Alabama.

SUMMARY
Hello, my name is Hayden Klemenok. Before I go into the specifics of what qualifies me for the position of orthopaedic
doctor at your medical practice of Andrews Sports Medicine, I would like to give a bit of an overview as to why I am a
good fit for the job. Myself being only thirty years old, I view myself as having the perfect mix of experience in the field,
while at the same time still having many years ahead of me as well which could be spent at your facilities. I have been
interested in medicine ever since my early adolescence, and because of this I carry with myself an intense aspiration to
always improve and become better educated in my field of study. Also, through the years I have developed a very
likeable personality which works to my advantage for both my clients and my colleagues. These qualities, along with
many others, I feel make me more than fit for the position at your offices and is why I hope you select me for the
prestigious position you have to offer.

EXPERIENCE
Orthopaedic Physician, University of California, San Francisco Medical Center
San Francisco, California 2010-2015
I worked as one of the head orthopaedic physicians at UCSF Medical Center after finishing my fellowship at University
of California, Los Angeles. I resigned from UCSF after five years because I seeked a job specializing in the field of
orthopaedics, such as your medical center.
Accomplishments
Successfully treated patients with injuries such as ACL tears and UCL tears.
Rehabilitated eighteen athletes with career threatening injuries.
Further perfected my field of medicine while at UCSF Medical Center.

Assistant Orthopaedic Physician, David Geffen School of Medicine, UCLA Medical School
Los Angeles, California 2008-2010
After finishing medical school, I trained in the fellowship program of UCLAs medical center specializing in
orthopaedics.
Accomplishments
Lead my class of doctors in training with quickest progress and success while at fellowship.
Successfully treated my first patient as a legal orthopaedist while at fellowship.
Received highest praises by my mentors while still in training.

While in my second year of pre-med at St. Marys College I worked as a doctors assistant to both the campuss
medical facilities, and to the fellowship program as well.
Accomplishments
I obtained my first hands-on experience in the medical field.
Acquired my first medical knowledge and skills while at St. Marys.

After completing my medical school training from St. Marys and fellowship training from UCLA, I went on to work at
UCSF Medical Center for five years until resigning in 2015.

SKILLS
Through my years of practice in the field I have developed many cooperative skills that are very useful in the field of
medicine such as compensation for others ideas and work types, being able to efficiently work with other doctors, nurses,
etc., a friendly, open attitude towards others which allows me to professionally coexist with other colleagues in stressful
environments that may arise, patience toward others, and most importantly the ability to remain composed in stressful or
intense situations. Along with the social aspect of myself, I have also obtained a masters degree in human anatomy from
St. Marys College to better familiarize myself with the human body and its mechanics.

REFERENCES
Through my years of training and practice in the medical field, I have left several lasting impressions on both my
professors and mentors. One of which who may openly attest to my strict work ethic, cooperative and likeable
character, and professional skill was my head mentor while completing my fellowship, Vice Chancellor of Health
Sciences at the David Geffen School of Medicine, UCLA, Dean John C. Mazziotta M.D., Ph.D.
Along with the recommendation of Dr. Mazziotta, a colleague of mine for five years who openly agrees to the skill and
overall personal qualities I possess is the head orthopaedic surgeon of the University of San Francisco California, Dr.
Christina Allen. Both of these people would be more than welcome to recommend me for the position you have to offer,
and would agree that I am the prime candidate for the job.
