January 1-31, 2010

I. ASSESSMENT:
• I was able to established rapport with my patients and their relatives and dealt them as much as I
could.
• Obtaining history with my patients is very important since I would know how to deal with their
complaints and render care to them efficiently.
• Performing physical assessment with my patients under my care was very important since it would
indicate the possible complications that would happen to my patients.
• Identifying the subjective and objective cues of my patients under my care would be very
important and I would know how to deal with them accordingly.
• Basing on the cues and laboratory results I could make a nursing diagnosis on my patients.

II. PLANNING
• Since there were so many patients in my ward, I was able to prioritize who were the important
patients to be given most important attention and rendered my service to them accordingly.

III. IMPLEMENTATION
• Carrying doctor’s order for the patient’s individual care and implementing nursing care based on
the assessment I gathered.
• Administered the prescribed medications and treatment for the patient accurately.
• Giving health teachings and discharge instructions to the patients for their health promotion.

IV. EVALUATION
• Evaluating the care I rendered if it was effective and assessing the condition of my patients.
• Notifies my superior and physician for any untoward reactions of the patients under my care and
dealt with them accordingly.

V. RECORDING
• Recorded the important nursing interventions and other necessary important data.
