APPLICATION FORM FOR MESS / ACTIVITIES / SPORTS TRAINING

FOR MILLENNIUM NATIONAL SCHOOL & JR.COLLEGE

FOR THE YEAR 2015 - 2016
I, the parent / guardian of the student, hereby undertake as following:
All rights of amenities provided are reserved with Edjunct Educational Services,
hereinafter referred to as the amenities provider.
1. I understand that the amenities provider offers an integrated program, in which
all facilities like mess, sports training and activities are provided in a single
package. I accept the total program, and understand that specific amenities
cannot be demanded as separate packages.
2. I wish to avail of the facilities of the integrated program provided by the
amenities provider at my own free will.
3. I have read the charges structure for the program and agree to avail of all the
amenities as per the integrated program and pay charges for the same.
4. I understand that the submission of this admission form does not guarantee
amenity to the integrated program.
5. I understand that Educational ancillary services of mess / activities / sports
training are exempted from service tax as of today. However, if the service tax
becomes applicable during the financial year then the same will be borne by me.
This even applies to any advance charges paid too. The above tax or any changes
therein, shall be borne by me entirely. This will be over and above the charges
that are charged, unless otherwise specifically mentioned.
6. I understand that Educational ancillary services of mess are NOT exempted
from the Maharashtra Value Added Tax. There shall be a government specified
levy on the amenities being provided to the students. If taxation rules change
after the date of signing this undertaking, the charges shall be borne by me
entirely. This will be over and above the charges that are charged, unless
otherwise specifically mentioned.

7. I understand that Educational ancillary services of sports training might be

exempted from the Maharashtra Value Added Tax. In case of any policy changes, I
agree to bear the entire charges for the same.
8. I understand that any defaulting of payments will result in either late charges
being levied or immediate discontinuation of the amenities being provided under
this program. This is independent from the functioning of the school. On
discontinuation due to default in payment, this service might not be offered in the
future too. The decision of the amenities provider will be final in all cases.
9. Charges paid are not refundable or transferable. In case of any absenteeism
(long or short), amounts will NOT be adjusted. Full payment will have to be made
in such cases.
10. Non-payment of charges will result in discontinuation of the amenities being
provided to the child. The decision of resumption of the amenities being provided
to the child, in case of non-payment of charges, lies entirely with the amenities
provider.
11. I understand that strict action will be taken for damaging the property of the
amenities provider.
12. While utmost safety precautions are taken and regulations are followed,
mishaps may occur in some cases. I understand and accept the risks involved.
13. The decision of the amenities provider will be final in all cases.
