I, ALEX H. MANAHAN, of legal age, Filipino, married, with residence address at B2
L6 Cluster 14, 96 Kaliraya St., Tatalon, Quezon City, after having duly sworn to in accordance
with law hereby depose and say:
1. That, I am the father of SHIER JOHN MANAHAN, who graduated from High
School last schoolyear;
2. That, my aforesaid son is applying for a college scholarship at Don Bosco
Technical School and one of the requirements for such application is the
presentation of Income Tax Return (ITR) of her parent, which I have none,
hence, this affidavit;
3. That I am not gainfully employed with any private or public entity, thus, I am
not filing any Income Tax Return with the Bureau of Internal Revenue;
4. That I am exempted in doing so;
5. That I have executed this Affidavit to attest to the truth of all the foregoing and
to satisfy the inquiry or requirements of any person or office in support of my
sons application for a college scholarship and for other legal intents and
purposes this may serve.

ADVERTISEMENT PLACEMENT CONTRACT

KNOW ALL MEN BY THESE PRESENTS:
This Agreement is made and entered into by and between:
VISIBLE VIDEO PRODUCTIONS
A registered single proprietorship with postal address at #86 Country Club Rd.,
Cityland Townhouses, Oranbo, Pasig City, represented herein by Leonardo Manalansan,
Jr. Producer, hereinafter referred to as MEDIA.
-ANDPHILIPPINE CHARITY SWEEPSTAKES OFFICE (PCSO), a government
corporation, duly organized and existing under and by virtue of Republic Act No. 1169, as
amended with min office located at E. Rodriguez Sr. Avenue, Quezon City, represented
herein in her official capacity as General Manager, ROSARIO C. URIARTE, hereinafter
referred to as PCSO.
W I T N E S S E T H T H A T:
WHEREAS, the MEDIA is the producer of the Television program ISLA aired
over NBN Channel 4 from 12 noon to 1 pm every Monday to Friday.
WHEREAS, the PCSO agree to place the following media value/s of One (1) 30
second spot/day from Monday to Friday within the program ISLA aired over NBN
Channel 4, with an equivalent amount of Php 11,000.00/spot.
NOW THEREFORE, for and in consideration of the foregoing premises, and the
term and condition set forth hereunder, the parties have agreed as follows:
1.

This contract shall be effective for a period of three (3) months which shall
commence from July 01, 2004 and shall expire on September 30, 2004,
unless sooner terminated by PCSO upon written notice thirty (30) days
prior to pre-termination of this agreement;

2.

The MEDIA will cause the placement/airing of the following media value/s
one (1) 30 second spot/day aired over NBN Channel 4 every Monday to
Friday from 12 noon to 1 pm with an equivalent amount of Php
11,000/spot.

3.

The MEDIA will bill PCSO the total amount of Php11,000.00/spot for the
placement one for the placement of one spot/day. Billing will be done in
monthly basis (Php 11,000.00/spot x no. of broadcast days/month);

4.

The MEDIA shall not sub-contract this agreement without the consent of
PCSO;

5.

The MEDIA shall not engage in any activity which may be prejudicial,
inimical, or in any manner adverse to the interest of the PCXSO and/or
Office of the President during the effectivity of the contract;

6.

The MEDIA will submit all duly notarized documentation requirements

(Certificate of Performance, Billing Statement, etc.) to PCSO for
documentations purpose;

7.

The PCSO guarantees payment of the advertising placements after

MEDIA submits certificate and billing statement.
