Thesearepathognomonicphysicalsigns.Ifapatientwithapatentductusarteriosus
(PDA)developsEisenmengerssyndrometherewillbearteriovenousmixingcausing
deoxygenatedblood.CruciallythisaffectsthelowerlimbsexclusivelyasthePDA
connectspulmonaryarterytoaorticarchdistaltotheleftsubclavianartery.Theexact
mechanismbywhichhypoxaemiaresultsinclubbingisnotfullyunderstood,butis
thoughttoinvolvegrowthfactorsinvascularendotheliumandplatelets(Seethis
articletolearnmoreaboutthemechanism).Henceasthelowerlimbsexperience
morehypoxaemia,theydevelopmoreclubbingandcyanosis.

Clinicaltips:
OnceEisemengersdevelopsthereisnoflowmurmuracrossPDAasthepressurein
rightandleftheartareequal.ThismeansasingleloudS2,whichmaybepalpable
andsignsofRVhypertrophy.Oftentheremaybeassociatedflowmurmurs,butnot
thecharacteristiccontinuousmachinerymurmurofaPDA.
