AFFIDAVIT OF QUITCLAIM WITH

INDEMNITY UNDERTAKING

We, Prudencia Salaguste Bacalso, wife, of legal age, Filipino, Marlyn

Salagoste Bacalso , daughter, single of legal age, Filipino; Aniceta Bacalso Balistoy,
daughter, married of legal age, Filipino, and Elisa Bacalso Cartalla, daughter, married
of legal age, all residents of Upper Lucimba Poblacion Pardo Cebu City, Philippines,
subscribing under oath,depose and state:
1. That I Prudencia Salaguste Bacalso, am the legally married spouse of Jaime
Cabalona Bacalso , who died intestate on June 11, 2015, at Vicente Sotto Memorial Medical
Center, per Certificate of Death No. 20131749, a copy of which is hereto attached as integral part
hereof;
2. That we: Aniceta Bacalso Balistoy , Marlyn Salagoste Bacalso,
Elisa
Bacalso Cartalla are the legitimate children of Prudencia Salaguste Bacalso and the deceased.
Jaime Cabalona Bacalso;
3. That at the time of his death, Jaime Cabalona Bacalso left certain personal
properties consisting of bank deposits at METRO BANK Borromeo Branch Located at
Borromeo St. Cebu City Philippines under Savings Account No. 4193419114864;
4.That as a matter of law, the said bank deposit is Absolute Community Property of which
one-half (1/2) thereof belongs to me Prudencia Salaguste Bacalso and the other one-half ()
belongs to my husband Wilfredo Borbajo Saberon.
5.That being the surviving heirs of Wilfredo Borbajo Saberon, WE are the only persons
legally entitled to inherit the one-half (1/2) share of Wilfredo Borbajo Saberon in Savings Account No.
100570118625 at Union Bank Cebu-Mandaue Branch.
6. That to the best of our knowledge, no other person is rightfully entitled to the money
under the said savings account of Wilfredo Borbajo Saberonexcept US, being the surviving spouse and
legitimate children of thedecedent.
7.That upon receipt of the proceeds of said savings account, WE undertake to render the
Bank free and harmless against any and all claims by third parties whatsoever.
8.Wherefore, for and in consideration of the foregoing premises, and upon OUR actual
receipt of the proceeds of Savings Account Number100570118625, WE hereby freely and voluntarily
RENOUNCE, WAIVE, QUITCLAIM, and FOREVER RELEASE any and all claims and/or causes of
action which WE may have or hereafter have, against the Bank, its officers and agents, over Savings
Account Number100570118625, in a manner that is absolute and irrevocable.
