Please read the following carefully as you will be required to sign it to say that you agree to these terms.
All candidates except those taking KET for schools, PET for schools & FCE for schools must sign this declaration.
- If you are 17, or under 17 years of age, 2 signatures will be required, one from you and one from your parent or legal
guardian
- If you are 18, or over 18 years of age only your signature will be required

Declaration by the parent of legal guardian: (French and Dutch translations available below)
English:
- I am the parent/legal guardian of the candidate named on this form and the candidate and I give consent to this
person taking the Cambridge English exam.
- I understand that all individuals who want to take a Cambridge English exam are required to agree to all the Terms
and Conditions. I confirm that I have carefully reviewed the Terms and Conditions mentioned on the British Council
website, including but not limited to, the provision of a valid photo ID for the exam day and that a photo will be
taken of the candidate on the day of the test and will be stored on the Cambridge English secure Results
Verification website.
- Additionally, if the candidate is to use their exam result for a visa/immigration application (UK Border Agency) : I
agree for the exam result to be available on other sites shown at http://www.cambridgeenglish.org/cambridgeenglish-for/visas-and-immigration/ if I give my consent on behalf of the candidate. The photo shall only be available
to organisations/individuals that I agree to give the candidates details to or authorise to view the candidates result.
- I consent to and agree that the candidate listed on this form will comply with all the Terms and Conditions of the
Cambridge English exam at this centre.

consentement sa participation aux examens du Cambridge English.

- Je confirme que j'ai lu attentivement le rglement repris sur le site du British Council y compris, entre autre, la
prsentation d'une pice d'identit valide le jour de l'examen et le fait qu'une photo du candidat soit prise le jour de
l'preuve et sera stocke sur le site web scuris de Cambridge English de vrification des rsultats.
- Si le candidat compte utiliser les rsultats de son examen pour obtenir un visa / faire une demande d'immigration
(UK Border Agency) : je consens ce que le rsultat de l'examen soit disponible sur d'autres sites figurant sur
http://www.cambridgeenglish.org/cambridge-english-for/visas-and-immigration/ si je donne mon accord au nom du
candidat. Je comprends et accepte que la photo ne sera disponible que pour les organisations / individus qui je
donne ses coordonnes. Je consens ce que ces organisations / individus utilisent ces informations pour vrifier
le rsultat d'examen du candidat.
- En signant ce formulaire, je consens et accepte que le candidat dont le nom figure sur ce formulaire doive se
conformer toutes les conditions du rglement du Cambridge English dans ce centre.

Ik bevestig hierbij dat ik zorgvuldig de algemene voorwaarden, vermeld op de British Council website, heb
doorgenomen met inbegrip van, onder andere, het verstrekken van een geldig identiteitsbewijs op de dag van het
examen alsook het feit dat er een foto zal worden genomen van de kandidaat op de dag van de test. Ik erken
tevens dat deze foto zal worden opgeslagen op een beveiligde website van Cambridge English ter verificatie van
resultaten.
Als de kandidaat van plan is om zijn/haar examenresultaat te gebruiken voor een visum / immigratie-applicatie (UK
Border Agency) : Ga ik akkoord dat het examen resultaat ook ter beschikking gesteld wordt op andere sites die
vermeldt staan op http://www.cambridgeenglish.org/cambridge-english-for/visas-and-immigration/ ,enkel indien ik
mijn toestemming hiervoor heb gegeven namens de kandidaat. De foto is slechts beschikbaar voor organisaties /
personen waartoe ik mijn akkoord heb gegeven om de test resultaten te bekijken.
Bij ondertekening van dit formulier, ga ik akkoord dat de kandidaat vermeld op dit formulier en die het Cambridge
English wil afleggen in dit centrum, moet voldoen aan alle algemene voorwaarden opgelegd door Cambridge
English.

- I will bring a valid ID with me on the test day, and I consent to have my photo taken by the centre on the day of the
Speaking test and /or the written papers. I agree to this photo to be held on the secure Cambridge English Results
Verification site.
- Additionally, if I use my exam result for a visa/immigration application (UK Border Agency): I agree for the exam
result to be available on other sites shown at http://www.cambridgeenglish.org/cambridge-english-for/visas-andimmigration/ if I give my consent on the application form. The photo shall only be available to
organisations/individuals that I give my details to or that I authorise to view my result.
- I have informed British Council Brussels if I have special needs of any kind. I understand that I cannot withdraw
from the examination and cannot change to another session.
- By signing this form, I declare that I am aware of and agree to comply with the Terms and Conditions of this
examination.
