I, JOSELITO T. LIM, of legal age, Filipino, married and with business
address at Near Petron Gasoline Station, Talon-Talon, Zamboanga City, Philippines,
after being duly sworn to an oath in accordance with law, do hereby depose:
1 That I am the owner/operator of D JAZZ MARKETING under
business ID No. 1545456;
2 That I was issued a Business Permit Number 15-18714R by the
Permits and License Division of the Office of the City Mayor of
Zamboanga City on January 29, 2015 and the same will expire
on December 31, 2015;
3 That said business had not yet started its operation from the day
of the issuance of the business permit due to the ongoing
process of its application for importation from the Office of
Bureau of Internal Revenue (BIR) and Bureau of Customs;
4 That I am executing this affidavit to attest the veracity of the
foregoing facts and for ITR purposes.

with business address at Megaland, Don Alfaro St., Tetuan, Zamboanga City,
Philippines, after being duly sworn to an oath in accordance with law, do hereby
depose:
5 That I am the owner/operator of DONNA BELLA TRADING
under business ID No. 1546340;
6 That I was issued a Business Permit Number 15-19212R by the
Permits and License Division of the Office of the City Mayor of
Zamboanga City on May 19, 2015 and the same will expire on
December 31, 2015;
7 That the said business had been operating without income and
considered and declared as insolvent, hence, the operation
ceased on August 26, 2015;
8 That I am executing this affidavit for the attestation of the
truthfulness and veracity of the foregoing facts and for the
purpose of terminating said business permit.

under business ID No. 1030630;
10 That I was issued a Business Permit Number 08-09061R by the
Permits and License Division of the Office of the City Mayor of
Zamboanga City on March 09, 2015 and the same will expire on
December 31, 2015;
11 That the said business had been operating without income and
considered and declared as bankrupt;
12 That I am executing this affidavit for the attestation of the
truthfulness and veracity of the foregoing facts and for the
purpose of terminating said business permit.
