BSN IV
Its a new beginning chapter to duty in the ward at Saint Michael as a staff nurse
without our mentors to guide us but only the staff nurse in the unit. Me, myself and I
trying to learn all in this ward as a staff nurse because someday I will be the same
for us. I assigned at district 3 and observed for the endorsement on how they
interact to the other nurse. The staff nurse duty, orient me first the daily routine in
the ward especially the charting and medication. So, I took all the vital signs to my
assigned district and charting it and recorded. I also observed for what my staff
nurse did in the duty. I did charting for two patients in a day but I never gave per
oral medications and the IVTT medications. In the end of the shift, I still observed
and listened for the endorsement.

BSN IV
I still assigned at the district 3 but another staff nurse to handle. Its another task to
adjust for the routine. The staff nurse assigned me to compute the drugs and dilute
the medications for the IVTT medication. I prepared all per oral medications for the
patients and observed to my staff nurse on how to give IVTT medications. I took all
the vital signs in the district and give medications through per oral and charting two
patient in a day. The staff nurse taught me everything about in the ward and the
priorities to be needed. I carried all the doctors order and follow up the IV dextrose.
In the end of the shift I still not yet do the endorsement I just still observing.

BSN IV
In this day, I assigned in district 1. Its another task to adjust because every person
has their own priorities at work on how they had finished it earlier. The staff nurse
assigned me not to charting because in this time our patients was modest toxic so
what I did to took all the vital signs in the district and do the charting vital signs and
recorded. I monitored the vital signs one of our patients, I also follow up the IV
dextrose and give medications through per oral, do the bedside care in every
patient, IV regulated, and do health things, but in the end of the shift I am the one
to endorse my patients to the pm shift. My staff nurse taught me on how to deal
everything in the unit.

BSN IV

I still assigned in the district 1 same staff nurse, so I took all the vital signs within
the district and prepare all the medications and carried out all doctors order, I had
two patients charting in a day and give per oral medications and assist all the
procedure, bedside care done and do the patients needed to be accomplished. If
there was for discharged I am the one to remove the peripheral intravenous cannula
without my staff nurse to have an initiative to do.
