Narrative of Offense Committed
On or about the above stated date and time, the Defendant did

operate a motor vehicle within the state while license has been barred for being an habitual offender under section 321.555 and 321.556 of
the Code of Iowa

I, the undersigned, being duly sworn, state that all facts contained in this Complaint and Affidavit, known by me or told to me by other reliable persons form the basis for my
belief that the defendant committed this crime
State all facts and persons relied upon supporting elements of alleged crime

DRIVER WAS OBSERVED SOUTHBOUND IN A COMMERCIAL VEHICLE PAST THE EARLY WEIGH STATION WITHOUT STOPPING AS
REQUIRED. OFFICER WAS ABLE TO GET THE VEHICLE STOPPED AT JACKSON AVENUE OFF HIGHWAY 20. AS I APPROACHED THE
DRIVER, I REALIZED IT WAS A DRIVER I HAD PREVIOUSLY ARRESTED FOR DRIVING WHILE BARRED. WHEN I RAN HIS LICENSE THROUGH
STATE RADIO, THEY CONFIRMED HIS LICENSE STATUS WAS BARRED.
