CLINICAL HISTORY:
69 years old male with a history of prostate ca. Now SIP RARP with PLND

DIAGNOSIS:

A. Radical prostatectomy specimen (32 grams):


- Prostate extensively involved bilaterally by prostatic adenocarcioma (Gleason's
pattern 4+3, total score 7).
- Approximately 60% of prostate slides contain tumor.
- The dominant tumor is located in right lobe and measures 1.7 cm.
- The tumor focally extends through prostatic pseudocapsule into extraprostatic soft
tissue.
- Perinerual invasion by carcinoma and high grade PIN are identified.
- Left seminal vesicle is involved by carcinoma.
- The apical, basal, pseudocapsular and soft tissue resection margins and right seminal
vesicle are free of tumor.

B. Right pelvic node - Two lymph nodes, negative for tumor (0/2).

C. Left pelvic node - Two lymph nodes, negative for tumor (0/2).

SURGICAL PATHOLOGY REPORT


PATIENT: WALDHEIM, PETER A. SPECIMEN#:

The specimen is labeled prostate, it is received in formalin and consists of a 32 g product of a radical
prostatectomy. The specimen is 4.5 ern from right to left, 3.2 cm from anterior to posterior, and 3-cm from
superior to inferior. The capsule is mottled tan'pink and shaggy with moderate fibrous adhesions. There is
cautery effed'''at the prosthetic urethral/bladder neck base. The right side is inked red and the left side is inked
black. The apex and base are coned and the specimen is sectioned from apex to base to reveal a diffuse white
tan nodular firm surface. A distinct 0.3 x 0.2 x 0.2 cm yellow tan firm nodule is identified at the left posterior
quadrant (A 10) The remainder of the specimen exhibits a minimal amount of whirled lobular tan nodularities
located peripherally to the prostatic urethra.

The right vas deferens is 3.4 x 0.5 cm and is cylindrical, pink and unremarkable. The right seminal vesicle is .r8~~
x 1.6 x 0.5 cm, lobular and pink and sectioning reveals a cryptic gray tan unremarkable surface. .

The left grey-tan deferens is 2.8 x 0.4 cm and is cylindrical and pink and unremarkable. The left seminal vesicle
is 3 x 1.5 x 0.5 ern, lobular and pink, sectioning reveals a cryptic a grey-tan unremarkable surface. .

Prostate gland is entirely submitted.
