Purpose.
The purpose of this procedure is to provide detailed information, instruction and
guidance on the methods to be used and the standards required for all activities involving
the use of scaffolding on site during the construction.

Scope.
This procedure is directly applicable to CV. Dira Utama Sejahtera operations in Indonesia.
The contents of this procedure will apply to all scaffolding activities on site irrespective of
whether they are being carried out by the Contractor, Subcontractor, and Owner or
Vendors Personnel.
1.6
Responsibility.
DUS assigned supervision is responsible for the Health, Safety and Environmental (HSE) in
their assigned area; this includes Project Manager/Project Coordinator, Area
Coordinator/ Site Coordinator and assigned Supervisors.

Procedure.

All scaffolding constructions will comply atleast with current industry practices and
standards and this procedure. Special attention will be paid to a number of minimal
technical specifications, which are specified below.
All scaffolding shall be tagged at each access point (ladder), indicating whether it can be
accessed or not. The tag will be red and will indicate that the scaffold is unsafe for use
until the scaffold has been inspected and passed safe for use. When it is safe and
inspected, a green tag card will be inserted, indicating the date, name and signature of
the authorized, competent inspector. The correct quality of the scaffolding is under the
complete responsibility of the scaffolding competent person whether it is constructed by
a DUS Contractors or Subcontractor.

In addition all scaffolding shall be re-inspected and confirmed as safe to use after bad
weather or other incident that could affect its safety, e.g. high winds. All scaffolding will
be built on a stable base. If necessary, special measures have to be used, such as spread
beams under the supports.
The standard scaffolding platforms will be able to carry 200 kg/m2. If scaffolding will be
used to lift up heavy materials, a calculation note, proving the construction is strong
enough will be made and given to the Senior Safety Officer. After the approval, the tag
card can be inserted, indicating safe access. In all cases, it is the responsibility of the
Subcontractor to identify which scaffolds require calculation notes, and to seek the
approval of Site Coordinator and the DUS HSE Department as necessary.
All work floors will have double handrail and toe board. This means a toe board of a
height of minimum 15 cm (no openings between the toe board and the working
platform), a handrail at a height between 1 and 1.2 m, and a handrail in between (height
about 50 cm) in the working platforms will be finished with a maximum opening of 20
mm.
All accesses to platforms will be made in a safe way by means of ladders or stairs (inside
or outside the scaffolding). No access is allowed by climbing over handrails. The use of
safety bars or gates is compulsory. Safety bars should be erected so that they always
close after use. Open safety bars always should be avoided. When raising or lowering
materials, Scaffolders should be clipped on at all times or on a safe handling platform
with a double guardrail provided. Neither equipment nor material shall be thrown up or
down.

Scaffolds shall be erected, moved, dismantled, or altered only under the supervision and
direction of a competent person.
Do not store excess material on a scaffold and keep all tools and material secure from
falling. If necessary, barricade below to warn other workers. Check to ensure for any
nearby energized electrical lines and maintain at least 4 meters feet distance.

Preparation
The work should be assessed to ensure that all health and safety risks are
identified and suitably controlled. Supervision should aim to organize, plan
and manage their work to minimize any risk, with a goal of zero
accidents/incidents.
Scaffolders should be selected who have appropriate attitude, aptitude,
fitness, training and experience for the particular task to be completed, and
have received appropriate and recorded training.
Supervision should be provided that is appropriate to the work situation and
the number and skills of the work team. Additional safety precautions are to
be incorporated where there is a need to prevent others entering an area or
protecting from falling objects. Any precautions should be appropriate to
the situation; methods include, securing tools, exclusion zones, etc.
All scaffolders must wear and use fall arrest equipment, which has been
inspected prior to use.

Alteration
One single guard-rail remains to ensure that scaffolders are protected when
carrying out alteration works. Scaffolders should be working off a minimum
three-board platform (3 X1 95 mm boards) or more for wider structures
when carrying out these operations in the working platforms will be finished
with a maximum opening of 20 mm.

stairs (inside or outside the scaffolding). No access is allowed by climbing

always should be avoided.

times or on a safe handling platform with a double guardrail provided.
Dismantling
All dismantling activities should be carried out progressively, reversing the
erection process. Therefore Scaffolders should work along the elevation
removing the single guard-rail and then lowering the boards from that
section of guardrail to the lift below. Scaffolders MUST NOT remove the
single guardrail from the whole elevation before lowering the boards.
Ladder Access
Ladders for use by Scaffolders should be included as early as possible in to the
erection process and removed as late as possible during dismantling, reducing the
need for Scaffolders to climb the scaffold structure.
It is recommended that a ladder bay is constructed and that ladders are
incorporated in this bay from top to bottom (i.e. for the entire height) of the
scaffolding structure.
Employees shall be instructed and required to ascend/descend ladders in. the
proper manner that is, facing the ladders and holding the-side rails with both hands.
Material shall be raised or lowered with line hoisting equipment and not carried in
one hand while ascending or descending. When a gin wheel isused, the support
frame shall be scaffold tagged.

Decking
Scaffolders should use as a minimum a 600mm wide platform placed centrally (e.g. 3
x 195mm scaffold boards). Boards used for decking should always be adequately
supported by transoms and bearers i.e. every 1.5m or 1.2m as appropriate.
All work floors ideally should be made of metal, which is fixed. However wooden
boards may only be used with the specific permission of PST Site Coordinator and
the HSE Department. Suspended scaffold systems shall be able to support six times
the maximum intended load.
When working on a suspended scaffold, employees must wear a harness and be tied
off to an independent life line which is not part of the scaffold support and is
capable of supporting 2.5 tons static load per person.
When welding on a suspended scaffold, check with the Site Safety Officer or Project
Superintendent for special precautions and rigging requirements protection.
Full body harnesses must be worn with a lanyard secured to adjacent structures or a
lifeline when employees are working 1.8 meters or more above ground or adjacent
elevation and a guardrail system is not provided. (100% Fall Protection).
Employees are allowed to tie-off to a guardrail system when the guardrail meets the
required standards for guardrails.
Guardrails are required (for construction) on any open sided floors, excavations, and
holes that are 1.8 meters or more above the next level, unless another type of fall
protection is used.
