This is an investigation to determine if Dep. Moreno had the responsibility

of notifying a supervisor of the misconduct observed by Dep. Armendariz while

actions taken by patrol personnel were appropriate in handling the civil matter
and civil court paperwork. During the incident Dep. Armendariz enters the home
of one of the subjects and demands that the child be produced and threatens to
arrest the subject if she is not turned over to him. Dep. Moreno is present during
the incident and can be seen on Dep. Armendarizs camera assisting Armendariz
with deciphering the court documents and the facilitation of turning over the child
to the other party.

to directly link items of evidence to Charley Armendariz in the course of his
duties. After further review it was determined that Deputy Williams or Deputy

card
for Catherine Worley.

After further review it was determined Deputy Williams authored IR

11-118250, which is associated with Catherine Worley. Catherine
Worley was arrested in regards to this incident report. It is unknown
if Deputy Williams ever had possession of this debit card at that
time, but since he had contact with the individual, the investigation
has been initiated.

It was also found Deputy Marie Trombi authored IR 09-106931,

which is associated with Catherine Worley. Catherine Worley was
cited and released in regards to this incident report. It is unknown if

Charley Armendariz, to Charley Armendariz in the course of his
duties. After further review it was determined Deputy Ross authored
IR 08-011053, which is associated with Jesus Manuel Basoco-Vega.
Jesus Manuel Basoco-Vega was cited in regards to this incident
report. It is unknown if Deputy Ross ever had possession of this
Mexican Identification card at that time, but since he had contact
with the individual, the investigation has been initiated.

This administrative investigation was initiated after detectives were

unable to directly link a Washington State Drivers License for Pedro
Villa-Nila, which was located at the home of Charley Armendariz, to
Charley Armendariz in the course of his duties. After further review
it was determined Deputy Ross authored IR 07-00177654, which is
associated with of Pedro Villa-Nila. Pedro Villa-Nila was arrested in
regards to this incident report. It is unknown if Deputy Ross ever
had possession of this Washington State Drivers License at that
time, but since he had contact with the individual, the investigation
has been initiated.

This administrative investigation was initiated after detectives were unable

to directly link items of evidence to Charley Armendariz in the course of his
duties. After further review it was determined that Deputy Brent Komoroski and
Deputy Chris Lopez may or may not have possessed the following items: 1) a
Mexican voter card in the name of Enoc Arim Velazquez Lopez; 2) Mexican
Matricula Consular Identification for Misael Garcia-Aragon.

This administrative investigation was initiated after detectives were

unable to directly link a Mexican Identification card in the name Enoc
Arim Velazquez Lopez, which was located at the home of Charley
Armendariz, to Charley Armendariz in the course of his duties. After
further review it was determined Deputy Komoroski authored IR 09116481, which is associated with Enoc Arim Velazquez Lopez.

Lopez was arrested in regards to this incident report. It is unknown if
Deputy Lopez ever had possession of this Mexican voter card at that
time, but since he had contact with the individual, the investigation
has been initiated.

This administrative investigation was initiated after detectives were

unable to directly link a Mexican Matricula Consular Identification for
Misael Garcia-Aragon, which was located at the home of Charley
Armendariz, to Charley Armendariz in the course of his duties. After
further review it was determined Deputy Komoroski authored IR 09068806, which is associated with of Misael Garcia-Aragon. Misael
Garcia-Aragon was arrested in regards to this incident report. It is
unknown if Deputy Komoroski ever had possession of this Mexican
Matricula Consular Identification at that time, but since he had
contact with the individual, the investigation has been initiated.

duties. After further review it was determined that Deputy Ortega may or may not

Casimiro Flores; 2) a Mexico Drivers License for Hermino Casimiro Flores; 3) a
Mexican Voter card for Jesus Mota-Mesa.

unable to directly link a Mexican Voter card for Hermino Casimiro
Flores, a Mexico Drivers License for Hermino Casimiro Flores, and
a Mexican Voter card for Jesus Mota-Mesa, which was located at
the home of Charley Armendariz, to Charley Armendariz in the
course of his duties. After further review it was determined Deputy
Ortega authored IR 10-042666, which is associated with the listed
names. Hermino Casimiro Flores was arrested in regards to this
incident and Jesus Mota-Mesa was listed as being a passenger in a

individual, the investigation has been initiated.

This administrative investigation was initiated after detectives were

unable to directly link packet number 915, a Mexico Drivers License
for Andres Fragoso-Perez, which was located at the home of
Charley Armendariz, to Charley Armendariz in the course of his
duties. After further review it was determined that Deputy Rangel
was the case agent in IR 10-091977, which is associated with
Fragoso-Perez. Fragoso-Perez was booked into MCSO jail in
reference to this incident report. It is unknown if Deputy Rangel ever
had possession of this Mexican Voter Registration Identification card
at that time, but since he had contact with the individual, the
investigation has been initiated. Item 915 was not listed among the
items put into inmate property for Fragoso-Perez.

This administrative investigation was initiated after detectives were

unable to directly link internal tracking number 77, a Mexican ID card
for Omar Sandoval-Arellano, which was located at the home of
Charley Armendariz, to Charley Armendariz in the course of his
duties. After further review it was determined that Deputy Rangel
was the case agent in IR 10-075516, which is associated with
Sandoval-Arellano. Sandoval-Arellano was turned over to ICE in
regards to this incident report. It is unknown if Deputy Rangel ever
had possession of this Mexican Identification card at that time, but
since he had contact with the individual, the investigation has been
initiated.

This administrative investigation was initiated after detectives were

unable to directly link item number 887, a Mexican Federal Electoral
card in the name Rene Hernandez De Jesus DOB:

Metricula Consular Identification in the name Rene HernandezDejesus DOB:
, both located at the home of Charley
Armendariz, to Charley Armendariz in the course of his duties. After
further review it was determined that Deputy Rangel was the case
agent in IR 11-044315, which is associated with Sandoval-Arellano.
Sandoval-Arellano was arrested in regards to this incident report. It
is unknown if Deputy Rangel ever had possession of this Mexican
Identification card at that time, but since he had contact with the
individual, the investigation has been initiated.

unable to directly link a Wisconsin Driver License for Juan Manuel

further review it was determined that Deputy Rangel was the case
agent in IR 10-060510, which is associated with Juan Manuel
Sanchez Alejandro. Juan Manuel Sanchez Alejandro was arrested
in regards to this incident report. It is unknown if Deputy Rangel
ever had possession of this driver license at that time, but since he
had contact with the individual, the investigation has been initiated.

unable to directly link an ID card for Miguel Vargas-Lucas, which
was located at the home of Charley Armendariz, to Charley
Armendariz in the course of his duties. After further review it was
determined that Deputy Rangel was the case agent in IR 11-032421,
which is associated with Miguel Vargas-Lucas. Miguel Vargas-Lucas
was arrested in regards to this incident report. It is unknown if
Deputy Rangel ever had possession of this ID card at that time, but
since he had contact with the individual, the investigation has been

; 2) Identification card for Gregorio Rodriguez
Jr.; 3) Mexico Drivers License for Juan Gutierrez-Fajardo; 4) Identification card

; 6) a Mexican voter card for Jose Mendoza-Mendoza DOB

unable to directly link an Arizona ID card for Luis Jesus Hernandez,
which was located at the home of Charley Armendariz, to Charley
Armendariz in the course of his duties. After further review it was
determined that Deputy Joya was the case agent in IR 11-053759,
which is associated with Luis Jesus Hernandez. Luis Jesus
Hernandez was arrested in regards to this incident report. It is
unknown if Deputy Joya ever had possession of this ID card at that
time, but since he had contact with the individual, the investigation
has been initiated.

Armendariz in the course of his duties. After further review it was
determined that Deputy Joya was the case agent in IR 10-085124,
which is associated with Gregorio Rodriguez Jr. Gregorio Rodriguez
Jr. was arrested in regards to this incident report. It is unknown if
Deputy Joya ever had possession of this ID card at that time, but
since he had contact with the individual, the investigation has been
initiated.

unable to directly link a Mexican Driver License for Juan GutierrezFajardo, which was located at the home of Charley Armendariz, to
Charley Armendariz in the course of his duties. After further review it
was determined that Deputy Joya was the case agent in IR 11032749, which is associated with Juan Gutierrez-Fajardo. Juan
Gutierrez-Fajardo was arrested in regards to this incident report. It
is unknown if Deputy Joya ever had possession of this driver license
at that time, but since he had contact with the individual, the
investigation has been initiated.

unable to directly link an identification card for Jose Luis MartinezMichaca, which was located at the home of Charley Armendariz, to
Charley Armendariz in the course of his duties. After further review
it was determined that Deputy Joya was the case agent in IR 11008715, which is associated with Jose Luis Martinez-Michaca. Jose
Luis Martinez-Michaca was arrested in regards to this incident
report. It is unknown if Deputy Joya ever had possession of this
driver license at that time, but since he had contact with the
individual, the investigation has been initiated.

This administrative investigation was initiated after detectives were

unable to directly link a license for Elizabeth Bishop, which was
located at the home of Charley Armendariz, to Charley Armendariz
in the course of his duties. After further review it was determined
that Deputy Joya was the case agent in IR 09-106931, which is
associated with Elizabeth Bishop. Elizabeth Bishop was arrested in
regards to this incident report. It is unknown if Deputy Joya ever had
possession of this driver license at that time, but since he had
contact with the individual, the investigation has been initiated.

Charley Armendariz in the course of his duties. After further review it
was determined that Deputy Joya was the case agent in IR 09106931, which is associated with Jose Mendoza-Mendoza. Jose
Mendoza-Mendoza was arrested in regards to this incident report. It
is unknown if Deputy Joya ever had possession of this Mexico Voter
card at that time, but since he had contact with the individual, the
investigation has been initiated.

Principal: Deputy Jesus Cosme S1501

This administrative investigation was initiated after detectives were unable

to directly link items of evidence to Charley Armendariz in the course of his
duties. After further review it was determined that Deputy Jesus Cosme may or
may not have possessed the following items; 1) a Mexican Voter Registration
card for Humberto Morales-Caballero; 2) a Mexican Matricula Consular
identification for Marco Antonio Ojeda-Sotelo.

This administrative investigation was initiated after detectives were

unable to directly link a Mexican Voter Registration card for
Humberto Morales-Caballero, which was located at the home of
Charley Armendariz, to Charley Armendariz in the course of his
duties. After further review it was determined Humberto MoralesCaballero is listed in report IR 11-035404. Documentation shows
Humberto Morales-Caballero was arrested by Deputy Jesus Cosme.
It is unknown if Deputy Jesus Cosme ever had possession of the
Mexican Voter Registration card at that time, but since he had
contact with the individual, the investigation has been initiated.

2.

This administrative investigation was initiated after detectives were

unable to directly link a Mexican Matricula Consular identification for
Marco Antonio Ojeda-Sotelo, which was located at the home of
Charley Armendariz, to Charley Armendariz in the course of his
duties. After further review it was determined Marco Antonio OjedaSotelo is listed in MCSO reports 2010-044136 and 2010-043845.
Deputy R. Armendariz S1764 and Deputy Jesus Cosme were the
arresting deputies and Case Agents for both incidents. It is
unknown if Deputy Jesus Cosme ever had possession of the

duties. After further review it was determined that Deputy Ernest Quintero may

determined Elias Dionicio-Garduna is listed in MCSO report 2011030758 authored by Deputy Ernest Quintero. It is unknown if
Deputy Ernest Quintero ever had possession of the Mexican Voter
card at that time, but since he had contact with the individual, the
investigation has been initiated.

This administrative investigation was initiated after detectives were unable

to directly link items of evidence to Charley Armendariz in the course of his
duties. After further review it was determined that Deputy Daniel Gandara may
or may not have possessed the following items: 1) a Mexican Voter Registration
card for Juan Carlos Balboa-Balboa.

This administrative investigation was initiated after detectives were

unable to directly link a Mexican Voter Registration card for Juan
Carlos Balboa-Balboa, which was located at the home of Charley
Armendariz, to Charley Armendariz in the course of his duties. After
further review it was determined Juan Carlos Balboa-Balboa is listed
in MCSO report 2011-005621 authored by Deputy Daniel Gandara.
Per the report Juan Carlos Balboa-Balboa was arrested during this
incident. It is unknown if Deputy Daniel Gandara ever had
possession of the Mexico Voter Registration card at that time, but

to directly link items of evidence to Charley Armendariz in the course of his
duties. After further review it was determined that Deputy Cisco Perez may or
may not have possessed the following items: 1) a Mexican Voter card for Antonia
Bobatilla-Franco; 2) Mexico Drivers License for Guadalupe Delgado Perez.

This administrative investigation was initiated after detectives were

unable to directly link a Mexican Voter card for Antonia BobatillaFranco, which was located at the home of Charley Armendariz, to
Charley Armendariz in the course of his duties. After further review
it was determined Antonia Bobatilla-Franco is listed in MCSO report
2010-033310 authored by Deputy Cisco Perez. Per the report
Antonia Babatilla-Franco was arrested during this incident. It is
unknown if Deputy Cisco Perez ever had possession of the Mexico
Voter card at that time, but since he had contact with the individual,
the investigation has been initiated.

unable to directly link a Mexico Drivers License for Guadalupe
Delgado Perez, which was located at the home of Charley
Armendariz, to Charley Armendariz in the course of his duties. After
further review it was determined Guadalupe Delgado Perez is listed
in MCSO report 2010-187229 authored by Deputy Cisco Perez. Per
the report Guadalupe Delgado Perez was arrested during this
incident. It is unknown if Deputy Cisco Perez ever had possession of
the Mexico Drivers License at that time, but since he had contact
with the individual, the investigation has been initiated.

This administrative investigation was initiated after detectives were unable

to directly link items of evidence to Charley Armendariz in the course of his
duties. After further review it was determined that Deputy Amie Duong may or
may not have possessed the following item: 1) Arizona license plate 693GYJ.
This administrative investigation was initiated after detectives contacted
Manuela Ruiz Hernandez who is listed as being registered to Arizona license
plate 693GYJ. Hernandez reported approximately two years ago she had been
stopped by MCSO deputies. During that stop deputies took the plate stating the
plate was not valid due to no insurance.
Hernandez further reported an unknown female deputy asked her if she
was . . . a citizen, a Mexican, are you illegal, and are you a wetback? CAD
records show the stop in question was conducted by Deputy Amie Duong.

*PSB investigated this case before the Courts Order. PSB finished its
investigation before the October filing that listed all of the investigations.

This internal complaint involves the investigation and completion of an

administrative investigation, IA#14-0142 (a division level investigation on Charley
Armendariz). That complaint, IA#2014-0142, was investigated by Sgt. Jeremy
Blain S1361 of District II. The complainant, Amy Rogers, reported that Deputy
Armendariz S1764 used excessive force and was unprofessional in his conduct
during a traffic stop. She had also retained a lawyer and was suing the Sheriffs
Office for her treatment and damages to her motorcycle. The incident occurred
on March 12, 2014 and the completed investigation was not received by the
Professional Standards Bureau until August 1, 2014. None of the required forms
were completed, none of the interviews were recorded and there was no
documentation of the injuries or damages. Concerns of various policy violations
were identified and were subsequently investigated by the Professional
Standards Bureau.
