CONTRACT OF SERVICES

KNOW ALL MEN BY THESE PRESENTS:

This contract made and entered between:
The Jawili Integrated School with office address at Jawili, Tangalan, Aklan,
herein represented by POLICARPIO T. TIBAR, Principal 1 hereinafter referred to as
the FIRST PARTY
-andWENNY LAHUT, of legal age, Filipino, married, a legal age, Filipino, married, a
resident of Jawili, Tangalan, Aklan, herein under referred to as SECOND PARTY;
WITHNESSETH:
1. That the FIRST PARTY is in need of the services of the SECOND PARTY as
Caretaker/Janitor at Jawili Integrated School, who shall perform work not performed
by regular personnel of the SECOND PARTY;
2. That the SECOND PARTY has signified his intention, to which the FIRST PARTY has
accepted to provide the service needed by the latter;
3. That the SECOND PARTY hereby possesses the education, experience and skills
required to perform the job as a Caretaker/Janitor;
4. That the SECOND PARTY hereby attest that he is not related within the fourth
degree of consanguinity or affinity to the hiring authority and/or representative of
the FIRST PARTY; that he has not been previously dismissed from government
service by reason of administrative offense; that he has not reached the compulsory
retirement age of sixty-five (65);
5. That in view hereof; the SECOND PARTY is hereby contracted as Caretaker/Janitor
for the period of twelve (12) months, in consideration of the monthly rate of THREE
THUSAND (3,000.00) to be paid on a monthly basis chargeable to the school
Maintenance Operation and Other Expenses;
6. That as Caretaker/Janitor, the SECOND PARTY is expected to perform the
following functions;
Perform actual cleaning of school premises, securing school properties and
does other related works/duties assigned by immediate and or next higher
supervisor;
7. That the SECOND PARTY shall perform work at a time and schedules to be agreed
by both parties;
8. That the SECOND PARTY shall report to the School Principal or Head Teacher of
Jawili Integrated School, Jawili, Tangalan, Aklan;
9. That the term of this contract shall commence on April 1, 2016 and expire on
April 1, 2017, but as case maybe, it will end up any time of the year when the
SECOND PARTY resigned;
10. That it is understood that this contract does not create an employer-employee
relationship between the FIRST PARTY and the SECOND PARTY, that the services
rendered hereunder are not considered and will not be accredited as government
service; and that the latter is not entitled to benefits enjoyed by the regular
personnel of the FIRST PARTY.

IN WITNESS WHEREOF, both parties hereunder set their hands this 1 st Day of
April 2016 at Tangalan, Aklan.
