Final Clinical Evaluation Summary
Student Comments: For noticing, I was able to notice subtle signs of electrolyte imbalance by monitoring my patients trending lab values and gathering subjective data during my assessment to develop a plan of care to treat my patient accordingly. For interpreting, I was able to prioritize my plan for the day when caring for two patients. When comparing data of two patients, I chose to assess the patient who was post op day 7 for pneumothorax and complaining of 7/10 sharp pain to old chest tube site first, before assessing a patient post op day 2 for adjustment of knee arthroplasty and had no complaints of pain. I understand that for my patient post op for pneumothorax, I did a focused respiratory assessment since my patients breathing was a priority. I also was able to develop my understanding of why my patient was receiving certain medications. For responding, I was able to assume responsibility by delegating nurse assistants to obtain vital signs if I had other priorities that needed to be done first. After performing my morning assessment, I was able to develop a care plan specific to my patient and prioritize what interventions needed to be completed first. I was able to communicate effectively and use therapeutic communication to calm and reassure an anxious patient. By practicing my IV therapy skills in resource lab, I was able to perform IV medication administration in the clinical setting efficiently. For reflecting, everyday after clinical, I wrote down notes to put in my weekly self evaluation such as what I had trouble with during the day and any feedback that I received from my instructor and staff. By doing this, I was able to understand what I needed to improve and plan on how I would improve. I understand that communication is important when delivering care such as providing medications on time. I understand the importance of communication, and by responding to feedback from my clinical instructor, I was able to improve on my communication skills and ensure that I provided clear communication during my clinical rotation to my patients, my patients families, primary nurses, and clinical instructor. I feel my strength was responding well to feedback and providing quality patientcentered care. Self Evaluation: Anthony Paguyo
