ON THE DAY OF THE EXAMINATION, the Examinee MUST bring the following documents and materials:
1. NMAT Identification Form (To be submitted to the Examiner. Please read the guidelines below.)
1.1. with signature of School Head if Examinee is a graduating college student, or
1.2. with photocopy of Transcript of Records if Examinee is already a college graduate
2. One (1) valid Identification Document (ID) (To be presented to the Examiner for validation)
2.1. A valid ID MUST be original and MUST have the following features:
2.1.1. Examinees name that exactly matches with the name in the NMAT Identification Form
2.1.2. Picture that matches the Examinees face, and
2.1.3. Examinees signature.
2.2. Some examples of a Valid ID that may be presented, subject for verification, are official School ID, Drivers
License, Passport, PRC ID, NBI Clearance, Police Clearance, etc.

A photocopy of the Examinees Transcript of Records
(TOR) or Certified True Copy of Grades (if newly graduate
and TOR is still unavailable) should be submitted.
FOR EXAMINEES WHO MAY NOT BE ABLE TO COMPLY WITH THE DOCUMENTARY REQUIREMENTS ON THE TEST DATE
If you were unable to present a valid ID, your personal appearance and presentation of a valid ID is required at the CEM Test Center
office where you took the test, whereas, document(s) duly accomplished NMAT ID Form and/or Transcript of Records, should be
sent to the NMAT Secretariat through a courier. The cutoff day for submission of documents is every Thursday. CEM will update the
database of test results every Friday and release your online copy of results and official NMAT Examinee Report Form by Monday.
Please be advised that your NMAT answer sheets will still be checked and scored together with all the other examinees. However,
the release of your test results (i.e., Online/Electronic Copy and Hard Copy/Official NMAT Examinee Report Form) will be withheld
indefinitely until you comply with the documentary requirements.
