Concerning the Board of Law Examiners and the State Bar of Michigan, a certification of graduation must be
received by the State Board of Law Examiners in furtherance of my application.

I request immediately upon my graduation that you certify the degree conferred upon me and the date my

LAW SCHOOL CHARACTER AND FITNESS CERTIFICATION

I am an applicant for admission to the State Bar of Michigan. Under the Michigan Supreme Court Rules
Concerning the Board of Law Examiners and the State Bar of Michigan, a certification of my character and fitness
must be received by the State Bar of Michigan Character & Fitness Department in furtherance of my application.
I request that you now direct a letter:*
1. Certifying I was not subject to discipline (other than for scholastic performance) during the period of my
enrollment or, if I was so disciplined, describing the extent of discipline imposed upon me and the facts upon
which the discipline was predicated; and
2. Certifying there are no facts reflected in my law school records or within your knowledge to negate the
conclusion that I have the requisite character and fitness to practice law or, if adverse information does exist,
describing its precise nature and the available evidence by which it may be substantiated.
I consent to the release of any information necessary to respond to this request and I agree to release and
discharge you, the school, its representatives, members of the Character and Fitness Committee, District
Committees thereof, the State Bar staff and, members of the State Board of Law Examiners and its staff,
individually and in their representative capacities, from any liability for the release of information pursuant to this
request.
