OBJECTIVES
Establish rules and regulations to have the better understanding the obligations and
responsibilities with respect to the achievement of the objective of management system.
To create awareness that HSE is an integral part of work and that production and safety are
inseparable.
Ensure to have better understanding to response in event of any emergencies.
Ensure thorough investigations on work area accidents/ incidents and establish safeguards to
prevent reoccurrence.
Ensure the workplace is provide of the safe work place is to eliminate/reduce to the risk of
death/injury to person/damage to property.
Ensure periodical safety training given in order to equip employee with the required knowledge,
skill, and attitude that will always enable them to perform their duties in the safest manner.
Create the positive atmosphere in health and safety in consultation with employees.
Identify highly hazardous operation within the scope of work and specific integrated preventive
measures to mitigate the same.
To ensure compliance with relevant applicable legislation.

1.0

SCOPE

The scope of this HSE Manual is to describe in detail with reference and requirement of statutory
regulations and guidelines of Occupational Health & safety management (OH & S), environmental
Management system (EMS) about the control measures in all aspects in order ensure the HSE
system maintained well within UNISCO Shop/Site
2.0
PURPOSE
The Purpose of (HSE) manual is to have an established and well-controlled Health, Safety
Environment management system within UNISCO Shop/Site
3.0

HEALTH PLAN

3.1

FIRSTAID FACILITY
Competent employee shall be identified and first aid training shall be given to ensure sufficient
first aiders are available.
The name of the first aiders shall be exhibited in notice Boards.
A Complete first aid kit shall made available in prime location and it shall identified & ensure
for easy access.
Periodical check of availability/validity of medicine shall be verified.
All first aid cases shall be recorded in a first aid treatment log. [Format :UNISCO(F)/HSE-01]

3.2

MEDICAL FACILITY

Treatment for Ill health shall consulted with the In-house doctor, in the event of severe Illness
the case shall be referred below mention hospital

Minor injuries that cannot be treated by the First Aider will be immediately referred to nearest
hospital.
Medical examination shall be conducted biannually for blasters & painters

In the event of a serious injury, adequate facility will be made available to call a physician or
ambulance or contact any other agency or to transport the injured person from the workplace to

3.3

SANITATION

3.3.1

Potable Water

Adequate drinking water shall be made available.

Chemical analysis test shall be done periodically once in a year for acceptable level.

E-Coli test shall be done once in 6 months.

Water analysis report shall be recorded in Format. UNISCO(F)/HSE-44

3.3.2 Toilets
Sufficient toilet blocks shall be available according to the strength of employees
Toilet blocks shall be cleaned frequently and maintained in a good sanitary condition,
cleaning details shall be recorded. [Format :UNISCO(F)/HSE-33]
Sewage water shall be disposed as on when required
Availability of soap fluids for employees to wash their hands before taking food.

3.4 HEAT STRESS


Working in excessive atmosphere will cause heat stress. Heat stress is serious health issue and can lead
to heat stroke, which could cause death. Everyone who works outdoors in hot weather need to follow the
following precautions
Drink plenty of water, even when not thirsty. Sip small amount often
Wear light, loose fitting cotton cloth.
Take regular break in the shade.
Avoid alcohol or drinks with caffeine before and during work.
Watch co-worker for signs of heat exhaustion or heat stroke.
If you start feeling symptoms of headache, dizziness, etc stop the work immediately and inform
to co-worker/ supervisor.
If he notice co employee suffering with heat stroke, organize for immediate help from
emergency medical service.
The following are few symptoms of heat stroke

No sweating

Red or flushed, hot dry skin

Rapid pulse

Headache

Blurred vision

Dizziness or fainting

Difficulty breathing

Unusual behavior

Convulsion and collapse.

3.4.1 Heat stress Preventive and awareness Program


Awareness training shall be given on heat stress management to all supervisory employees.
All employees shall be encouraged to drink sufficient water at frequent interval to avoid
dehydration.
Sufficient stock Oral Rehydration powder shall be kept in the stores
Direct sun light works shall be suspended during the high temperature
All workers shall be advised to take short break if feel fatigue
Heat exhausted person shall be immediately attended by the first aider/foreman and shall be
referred to clinic if required.

3.5 FOOD SAFETY


3.5.1 General
The hygienic level in the kitchen/canteen & during preparation shall be well maintained in all level at all
the times. This includes following good practices in maintaining kitchen, kitchen equipment during food
preparation.

Protecting food from any kind of contamination


Preventing any organs multiply which could result in premature decomposition of food
Eleminating any harmful biological organisms(viruses, certain yeasts, spores, moulds etc)in
food by good cooking practices.
Kitchen/Canteen shall be inspected and recorded once in a month.

3.5.2 Personal hygiene practices during handling food


Kitchen/Canteen staff shall follow

Avoid touching nose, mouth hair and skin during food preparation

Do not smoke in kitchen and canteen.

Do not cough or sneeze directly onto food

Wash hands after coughing or sneezing

Wash hands after blowing nose

Use disposable tissues to wipe hands

Cover all wounds or cuts on hands or arm completely with bright colored waterproof wound
strip

Wear disposable gloves if there are wounds on hands. Change both gloves and wound strip
regularly & Wearing hair cover (net caps) and aprons.

Keeping fingernails short and clean,Change gloves every after change of task

Avoid using strong perfumes/ aftershaves

Also food handlers should be free from any illness

3.5.3 Food storage


Temperature shall be maintained for the following food

Refrigerated storage shall be 41 F or below

Frozen storage shall be -10 F or below

Fresh fruits and vegetables shall be 35 to 55 F

Food shall be stored as required in order to avoid contamination.

Non-food items shall be stored separately.

3.5.4 When to wash and sanitize of equipment & dishes

Equipment shall be cleaned generally. The food contact surfaces of equipment & utensils shall
cleaned thoroughly

Before each use with a different type of raw food

3.5.5 Hand washing lavatory

Easily accessible

Not to be used for any other purpose

Must have liquid / powder soap (no multi use hand towels)

3.5.6 General cleanliness


Cleanliness should be ensured on the following by cleaning at regular intervals

Non-food contact surfaces shall be cleaned at regular intervals.

Stoves, grill and deep fryer surfaces

Floor areas underneath food preparation areas; walk in freezers, refrigerators and around cooking
areas.

Condiment containers

Dry storage areas.

3.5.7

Control of pests

Conduct regular inspection on the premises.

Ensure kitchen garbage area shall be maintained & garbage is removed regularly.

3.5.8 Waste disposal


The kitchen wastage shall store in identified placed, it shall be dispose immediate as on when required.

SAFETY PLAN

4.1

RESPONSIBILITIES, ACCOUNTABILITIES & AUTHORITIES

4.1.1

General Manager-Projects

Responsible and authority for the effective implementation of HSE manual & ensure the employees
at all levels are clearly understood & following the same.
Accountable to the Managing Director related to health, safety & environment and welfare of
employees and who shall be affected by the companys activities.
Ensure all HSE issues been analyzed thoroughly & the required correction / corrective action &
preventive action are implemented.
To ensure responsible for the availability of the required infrastructure & resources at all levels.
Shall chair safety management/committee meetings.
4.1.2

Site / Production Manager

Responsible for his personal safety and all employees under his authority, including sub contractors/
visitor who shall be affected by the companys activities.

Responsible for the implementation of HSE manual & ensure well maintained at all levels

Accountable to the General Manager related to health, safety & environment for safe operations.

Implements the HSE manual and other referenced requirements.

Ensure that all Engineers are aware and carry out the requirements of the HSE Manual.

Ensure the engineers, supervisors, foremans & employees understand the HSE requirement and
follow the same.

Maintains close liaison with the Safety Engineer/Officer.

Ensure all HSE issues have been discussed in safety committee meetings and initiate for the
required correction / corrective and preventive actions Ref.HSE Management flow chart
UNISCO (F) HSE-08 and Management program for significant requirements of OSH & E shall
be recorded in [Format. UNISCO(F)HSE-38]

Intimation of any changes in Organizational Structure, Activities, Machinery, Procedure,


Technology, Product etc... shall be discussed in safety committee meetings and recorded in
format. UNISCO (F) HSE-31.

Ensure active participation of employees during Safety Tool Box meetings

Ensure the availability of first aid kit & first aid personnel.

Ensure the Statutory Notices, the IMS Policy, the names of appointed First Aiders/ Fire Wardens
are displayed and maintained in prominent locations.

4.1.3 Safety Engineer/Officer:

Safety Engineer/Officer directly reporting to General Manager and co ordinate with Site/
Production Managers, Site/ / Production Engineers, supervisors and foremans.

Accountable to the General Manager for all health, safety & environment requirements.

Responsible to establish the HSE manual at all levels.

Ensure & Monitor the safe working conditions and safety practices.

Ensure the availability of first aid kit & first aid trained personnel.

Ensure the subcontractors, are complying with the UNISCO HSE requirements.

Ensure personal protective equipment is worn by employees as appropriate.

Conduct weekly safety inspection or co-ordination with the Site/Production Manager / &
Site/Production Engineer.

Safety Engineer / Officer shall conduct daily inspection to identify any safety deviation & initiate
report deficiency and arrange immediate rectification.

Conduct safety observation and Preparation of daily safety observation report. Format.
UNISCO(F)/HSE-03]

Preparation of weekly safety report. [Format. UNISCO(F)HSE-04]

Preparation of monthly safety report. [Format. UNISCO(F)HSE-05]

Preparation of monthly HSE activity plan [Format. UNISCO(F)HSE-11]

Preparation of HSE statistics [Format. UNISCO(F)HSE-41]

Stop work notice shall be issued in case of imminent hazard/risk to employee/equipment

Co-ordinate with site/ production Managers/Engineers while conducting of risk assessment for
implementation.

Educate the employees awareness on applicable legal & other requirement

Investigate & analyze root cause in all accident/ incident and report to General Manager.

Analyze and report HSE performance & recommend areas of improvement.

Conduct the safety toolbox talk every week.

4.1.4 Site /Production Engineer


Ensure for the Implements the HSE manual and accountable to the Site /Production Manager
for all health, safety & environment requirements
Responsible for his personal safety and all employees under his authority
Participate actively in HSE activities to establish the system is well maintained.
Encourage employees to work in safe manner.
Ensure availability of risk & impact assessment for the activity and implement the control
measure as required.
Maintain the safe work environment, tool, equipment and other resources.
Conduct safety toolbox along with supervisors/foremans.
Ensure the appropriate PPE is used for the activities monitored proper for the usage

4.1.4 Maintenance Engineer (Electrical /Mechanical)

Ensure for the Implements of HSE manual and accountable to the General Manager for all
health, safety & environment requirements

Responsible for his personal safety and all employees under his authority.

Ensure availability of risk & Impact assessment for the activity, implementation of the
control measure.

Conduct safety toolbox along with supervisors/foremans.

Ensure qualified employees shall be allowed to work near exposed, energized electrical
equipment & machineries.

Ensure provision of PPE required for the activities monitor its proper use and maintenance.

Ensure the color coding for the equipment and machineries including cables/ cords and leads

Ensure and maintain the minimum approach distance for energized electrical equipment

Implementation of preventive maintenance for all the equipment and machinery in safety
aspects.

4.1.5 Supervisor / Foreman


Accountable to the Site /Production Engineer for health, safety & environment with respect to
safe practices and shall be responsible for his personal safety and all employees under his
authority.
Shall exercise close supervision over those employees who report or shall be assigned to him. He
shall ensure that employees working under him perform the work safely. He must enforce safe
rules and procedures.
Must ensure that Safety regulations are clearly understood and that the means of escape routes,
assembly points are thoroughly known to all his employees.
Ensure that all tools, machinery, and equipment shall be regular and systematic in inspection,
shall be in safe operating condition.
Co-operate with Safety engineer/ officer and follows instructions given.
Conduct toolbox talk to the employees before starting a new task.

4.1.6 Employees
Carry out assigned tasks and duties in a safe manner, in accordance with instructions, and to
comply with safety rules/procedures, regulations and accountable to the Supervisor / Foreman
for safe practices.
Consult with foreman if any doubt in safe work practices.
Obtain and use the correct tools/equipment for the work and not to use any that are unsafe or
damaged.
All tools, equipment, and personal protective equipment must be stored in the proper place after
use.
Ensure that all guards are securely fixed in rotating equipments.
Not to operate any equipment/ machineries unless authorized.
To report any accident, near miss dangerous occurrence or dangerous condition to
forman/supervisor/engineer
To avoid unplanned arrangements and suggest safe ways of eliminating hazards.
Not to participate in horseplay or place fellow employees in danger by their actions.

4.2

COMMUNICATION, PARTICIPATION, & CONSULTATION

UNISCOs Employee participation and communication shall ensure through weekly & daily
toolbox meeting, HSE posters, notice board etc.
Employees shall be encouraged to provide suggestions and report concerns regarding health, safety,
and environment.
Safety Induction (Internal & External)
All New UNISCO employees shall be given safety induction prior assign for work. Format
UNISCO[F]AD 01
All sub contractors shall given safety induction on UNISCO HSE requirement prior assign for
work. [ Format UNISCO(F)HSE-23 /A]
Specific safety orientation training shall be given when such kind of job being executed.

All visitors shall be given an induction on UNISCO HSE requirements,accompanied with UNISCO
employee at all times during the visit [Format UNISCO(F)HSE-23/B]
Visitors shall be providing with PPE (hardhat, safety shoes, safety glasses, earplug etc.)
4.2.1

Tool Box Meeting

The concerned Supervisor / Foreman shall give daily toolbox meeting at the starting of task. [
Format UNISCO(F)HSE-22 ]

Safety Engineer/ Officer shall conduct weekly safety toolbox meeting prior to commence any
activity along with the site/ production engineer [Format UNISCO(F)HSE-10 ]

4.2.2

HSE Meeting
HSE meeting shall be conducted frequently through participation of HSE committee members to
analyze on the safety reports recorded.

4.2.3

Participation & Consultation


Safety Engineer/ Officer shall ensure participations of safety committee members in OHS & E
management system by; Safety Committee members attached in Annexure 2.
Safety Committee involved in activities such as

Appropriate involvement in risk assessment

Appropriate involvement in incident investigation

Involvement in the development and review of OH&S policies and objectives

Consultation where there are changes that affect their OH&S, and Representatives on OH&S

4.2.4

Fire Extinguisher Training

Competent employee shall be identified and fire warden training shall be given to ensure sufficient
firewatchers are available.
Internal training shall be given to all employees in the awareness of handling fire-fighting
equipments in the event of any fire.
4.2.5

Mock Drill

The mock drill shall be planned [Format UNISCO (F) HSE-24] and conducted by safety
engineer/ officer/ Site/production Managers.
Mock drill shall be conducted in accordance with UNISCOs emergency procedure clause 6.0
for the effective awareness
Mock drill shall be conducted once in three months on the following order to increase the
awareness with the employees in event of any emergency.
Corrective action will be implemented on defective actions by providing the employees with
more training and information, as required.
Emergency mock drill report shall record and maintained. [Format. UNISCO (F)HSE-15]
Awareness for employees shall be given to contact ROP/ Civil defense in case of any major
accident in work place during the mock drill.

4.3

INSPECTION

Regular inspection shall be carried out as follows in order to identify deficiencies unsafe practices
etc in order to ensure the effective implementation of HSE requirement

The Safety Engineer/ Officer shall conduct daily inspections, report deficiencies, and organize
immediate corrective/preventive action. Any default/defaulters shall be issued default notice.

The Safety Engineer/ Officer shall conduct regular monthly inspection on the site/workshop
activities. Deficiencies found during inspections shall recorded and organize for
corrective/preventive action [Format UNISCO(F)HSE-18]

Fire extinguishers shall be inspected on a regular monthly basis.

4.4

ACCIDENT / INCIDENT REPORT


Each event, which results an occupational injury, shall be reported and investigated.
Accident/Incident investigation shall be carried out by Site/Production Manager/Engineer
and Safety Engineer/ Officer.

Investigate the immediate Engineer/Supervisor/Foreman/Victim for finding the root cause


and initiate the corrective/preventive action & shall be recorded.

The learning value of the accident investigation report shall be Implemented and cascaded
to all employees.
4.5
PERSONNEL PROTECTIVE EQUIPMENT (PPE)
Hazards shall exist in workplace in many different type: sharp edges, falling objects, flying sparks,
chemicals, noise and a innumerable of other potentially dangerous situations. Occupational Health &
safety management (OH & S) shall requires that employers protect employees from workplace hazards
that shall cause injury.
Controlling a hazard at its source shall be best method to protect employees. Depending on the hazard
or workplace conditions, UNISCO urge the use of Personal protective equipment to manage or eliminate
hazards.
Personal protective equipment, commonly referred to as "PPE", is equipment worn to minimize
exposure to a variety of hazards.
The PPE availability shall be verified & recorded monthly basis. [Format UNISCO (F) HSE-13]
4.5.1 Safety Helmets and Safety shoe
Safety Helmets helmets shall do the following:

Resist penetration by objects.


Absorb the shock of a blow.
Be water-resistant and slow burning.
Proper adjustment and replacement of the suspension and headband.

4.5.2 Clothing and Gloves


Protective clothing and gloves shall wear to protect the body and limbs from hazards of,
chemical liquids and vapors, heat, electricity, radiation
Welders shall wear leather aprons, hand sleeves, leg guard, leather gloves etc
Cotton gloves with leather or suitable palms shall worn for general protection purpose,
(handling materials)
Rubber gloves should wear whilst working on live, low- voltage electrical switchgears.
4.5.3 Eye Protection

Welders shall use welding shield with appropriate filters (shades) during welding.

Goggles, safety glasses, face shield shall be worn during the chipping, grinding, & gas
cutting.

4.5.4 Hearing Protection


Hearing protectors is a very personal one and depends on a number of factors including level of
noise, comfort, suitability of the hearing protectors for both the worker and the environment and
compatibility.
Public environment noise level shall not exceed 70 dB.
Noise monitoring record shall be maintained and recorded .Once in a month

4.5.5 Respiratory protection


Disposable dust mask shall be used in the presence of airborne dust or particles
Shall wear half face piece respirator during spray painting and full-face respirator during manual
blasting.

Work Procedures:

Manual Handling:
During lifting back shall be straight, load shall be close to the body, the leg muscle shall used for
lifting, no twist in upper body while lifting.
Object shall be inspected for any sharp edges, nails any other thing that may cause injury.
Gloves shall be used during handling rough sharp material.
During lifting objects, weight above 50 pounds power equipment shall be used.
During two or more people carrying any heavy object prearrange signaling shall be ensured
while lifting lowering/ dropping and releasing the load.
Ensure proper PPE shall used during the task

4.7

Mobile Equipment Operation:

Prior to moving the equipment, the movement path shall be ensured free of employees and any
objects.

The equipment shall be move in a safe speed (10km/hr) in company premises.

The permitted operator shall drive the truck, forklift, and any other equipment.

Load shall not be raised or lowered during handling & during travelling.

The load on the truck shall be firmly fastened in order to avoid any tipping or falling

Unsafe attachment shall never be used.

4.8 Lifting operation & Equipment


All lifting gears shall be checked for the fitness and certified by the third party inspector and it
shall be color coded accordingly.
All materials handling equipment cranes, forklift shall be checked for the fitness and certified
by the third party inspector
The weight shall be assessed properly in order to engage suitable lifting equipment.
The pipes, beams, plates shall be stacked properly with wooden sleepers/ steel poles
Loads shall be controlled by tag lines.
Only qualified riggers shall be engaged in any rigging activity.
The rigger shall be present during the loading, unloading, and any other handling by lifting
equipment.
No alteration, repairs modification shall be done in any lifting gears and equipment.
Never cross/ walk/ work under or near the suspended load.
Lifting tools & tackles shall be visually inspected on a regular basis, and third party
inspection shall be done once in 6 months and lifting equipments once in a year.

4.9 Hand Tools:


Defective tools shall not be used.
Do not carry sharp hand tools in clothing.
While using the hammer eye protection shall be worn.
While using the screwdriver, the correct size, type of screwdriver shall be used.

4.10 Power Tools:


Ensure all the electrical tools are acceptable safe condition and shall have necessary safety
devices.
Ensure all the metal parts, which do not carry currents (handle, housing etc), shall be grounded
when connected to a power source.
Ensure the equipment is disconnected while making adjustments or repairs.
Do not use electrical tools in areas when there is a possibility of flammable dust, gases, or
vapor.
Do not use extension cords except when necessary.
All the tools shall be guard/ shields installed.
Ensure all the tools meet safety standards & good condition.
Faulty tools shall be labeled as unsafe or under maintenance.

4.11 Compressed gas cylinders:


Ensure cylinders are not drop, expose to extremes temperature.
Ensure the valve cap always in place except when in use.
Ensure the valve or valve cap is not used to lift the cylinder.
Compressed gas cylinders shall not roll in horizontal position.
Never tamper with safety relief devices of cylinder valves. Always use proper fittings on gauges
suitable for the particular gas being used.
Contents shall be properly marked on all cylinders.
Compressed gas cylinder (whether full or empty) shall be transferred in upright position secure
them with chains at cradle.
Ensure oxygen cylinders are separated from combustible materials & acetylene cylinders by
minimum distance of 3 meters during storage.
Flammable gas cylinders shall be stored in proper designated and safeguarded area.
Cylinders shall be never placed where they might become part of an electrical circuit.
No smoking, naked lights or sources of ignition shall be allowed within or near cylinder stores.
Fire extinguishers shall be located outside the store entrance
4.12

FIRE PREVENTION
The main precautions to prevent fire include-

Keep fuel, heat, and oxygen apart.

Keep heat away from combustible materials.

Prevent flammable vapors mixing with air. Understand how a fire occurs, the dangers of heat
and smoke and how to control the fire.

Fire extinguishers of suitable type, capacity and numbers shall be provided

All fire extinguishers shall be inspected regularly and calibrated periodically.

All fire pumps and fire hose reels shall be inspected weekly and record shall be maintained

All fire alarm system & emergency lights shall be tested monthly [Format - UNISCO(F)HSE32]

4.13 SCAFFOLDING
Safety engineer /Officer shall inspect all erected scaffolds. Safety Engineer/ Officer will issue Scaff
Tag permit for the fitness.
4.13.1 Mobile Scaffold Towers
Mobile towers constructed from scaffold tube and fittings or cup lock must only erected by a
competent person who has sufficient knowledge and experience of erection techniques and the load
factor, which is likely to be imposed on the mobile tower when put to proper use,
A ladder shall be provided to gain access to the working platform and shall be secured.

The recommended maximum height of a mobile tower is 9.6 meters although this shall
be increased to 12 meters if tied to a structure when is in use.

Working platform shall be secure, fully covered using boards at least 38mm thick.
Supported with transoms at not less than 1.5-meter intervals.

A main guardrail shall be minimum of 910mm if the tower is above 2 meters height

Sufficient numbers of intermediate guardrails or suitable alternatives shall be done; the


gap does not exceed 470 mm.

Toe boards shall be at least 150mm high, fixed with proper clamps.

Wheels shall be secured to the standards of the scaffold tower and fitted with safety
locks.

Mobile scaffolds shall be used on ground, which is firm and level.

The working platform shall be clear of men of material before attempting to move.

4.13.2 Independent Tied scaffolds


Scaffolds over 2 meters high shall be fitted with guardrails and toe boards.
Base plates shall be fitted into the bottom of standards, which shall be rest firmly and evenly
on the timber sole plates.
Scaffolds shall be erected on ground, which is both firm and level using timber sole plates
of adequate size to spread the weight of the structure evenly between two standards.
Material including scaffold components shall not be dropped or thrown from scaffolds but
lowered to ground level.
Access to working platforms shall be achieved by a suitable, secured ladder built into
scaffold structure, with intermediate landing places at not more than 9-meter intervals.
Where the scaffold has been designed to store material the load shall be spread evenly over
the working platforms, ensure adequate bracing and ties are considered.
Materials shall not be stacked higher than the toe board unless additional arrangements are
provided.
Independent scaffold shall be erected higher than 30 meters unless it is properly designed
for that purpose with design calculations.

4.14 Safety Rules When Using Ladders


Certain safety rules shall be observed while using ladders: Ensure that footwear is not greasy, muddy, or slippery and has a good grip on the rungs.
Face the ladder and hold on with both hands.
Use platforms or attached trays for tools while working on a ladder. Alternatively, a bucket
lashed to a rung can be used as a tool holder.
Hold on with at least one hand. If the use of both hands is necessary to do a job, a safety
harness shall be worn.
Overreaching or leaning sideways may cause overbalancing and fall from the ladder.
Do not climb higher than the third rung from the top.
Portable ladders shall not be used in high winds.

Correct placing and securing of a ladder following guidelines shall be followed:

The ladder shall be set on a firm, level, and non-slippery base.

On soft ground, solid footing shall be provided to prevent the ladder from sinking. On firm
earth or soil the ladder should be set in prepared, scraped- out grooves.

Ensure the ladder shall be secured effectively by lashing it as near as possible to its upper
resting place to prevent sideways movement. This is particularly important with long ladders,
where it is easy to overbalance it by even slight movement of the body.

Correct positioning of the ladder is 1meter out at the base for every 4 meter height. (75
degree)

FALL PROTECTION (WORK AT HEIGHT)


Prior to allowing employee to work at heights, the Supervisor/ foreman shall obtain WAH permit
[Format. UNISCO (F) HSE-26] & confirm their fitness to work and that they do not suffer from
vertigo.
To avoid fall accidents during working at height, sufficient fall protection systems shall be in
place. The following types of prevention system shall be used wherever applicable.

1. Safety harness
2. Scaffolding platform
3. Working platform.(MEWP)
Safety harness:
All employees working above 2 meters shall use safety harness, secure properly with lifeline
or to firm support.
All employees working above 6 meters shall wear safety harness with lanyard attached with
shock absorber, employees working below 6 meters shall not use lanyard attached with shock
absorber.
Working Platform (MEWP): Mechanical Elevated Work Platform
Competent & trained employee shall operate the Mobile elevated working platform .
HSE MEWP inspection shall be done on monthly basis [Format. UNISCO (F) HSE-47]
MEWP shall be inspected by third party & certified for fitness.
Refer manufacturer operation and safety manual prior to use the equipment.
4.16

WELDING

Employees shall wear proper PPEs (welding helmet / welding visor, leather gloves , aprons
,hand sleeve, leg guard ,filter mask, ear plug )in good condition shall be used during the
welding task.
Ensure the flammable /combustible materials cleared prior to work.
Protective glass shall be used when knocking up slag etc.
Welding arc shall be screened to avoid other employees being affected or expose by radiation.
Welding equipment shall be switched off when not in use.

The electrical supply shall be switched off when the welding cable is being pulled from one
place to another.
Welding cable shall not lay on gas cylinders, oil containers or through wet areas etc.

Precautions shall be taken to prevent adjacent objects from catching fire due to welding

4.16.1 Grinding
All grinding machines shall have guards in place.
Grinder RPM shall be less than wheel RPM.
Double face shield like Safety glass and face shield shall wear.
Ensure defective or malfunctions grinder to be immediately removed from service.
Ensure flammable /combustible materials are cleared prior to work.
Cutting wheel shall not be used for grinding activity and grinding wheel shall not be used for
cutting activity.
Wheels shall not be over tighten & shall not use damaged/crack wheels.
Ensure the firefighting equipment nearby.
Grinder shall be plugged off when changing the wheel.
Do not use ornaments and loose garments during the operations
Proper align key shall be used to fastening the wheel
Cables shall be clear of pathways, stairs, and ladders
Grinding machine shall be operated, repaired, serviced and handled by authorized and
experience employee.

4.16.2 Gas Cutting


Ensure hoses are checked thoroughly for any defects & greasy or oil soaked hoses shall be
replaced
Regulators and torch shall be check thoroughly and cleaned before starting the job.
Flash back arrestors shall be equipped in oxygen as well as acetylene.
Only flint lighters shall light the torches.
Cylinders shall never be allowed to come in contact with live electrical wire and shall be
kept away from sparks, flames, or slag.
Cylinder valves shall be closed when work is completed or stopped.
Cylinders shall be always being kept in upright position secured with chain.
Hoses shall be clear of pathways, stairs, and ladders.
No cutting shall be performed on used drums, barrels, tanks or other containers that were used
for holding flammable/ combustible materials until they have been cleaned.
Cleaning must remove all flammable materials present, as well as any substances such as grease,
acids, or other materials that might produce flammable or toxic vapors when subjected to heat.
Gas cutting kit shall be inspected on a regular monthly basis. [Format. UNISCO(F)HSE-28]
4.17

Handling/Storage of Chemicals
Material Safety Data Sheets shall be referred.
Container label shall be studied prior the task.
Barricades shall be used in leakage areas.
Chemical containers shall not be kept near equipment.
All employees shall have information of hazardous materials / liquids available.
Ensure employees aware eyes wash areas.
Do not eat, drink, or smoke while handling chemicals.
Cigarette lighters or matches shall not be taken in to any hazardous area.

Spillage shall be mopped immediately.

Paint containers shall not be stored more than 3 nos in column.


Temperature shall be maintained below the flash point.

Legal permit shall be obtained for storage of chemical and records shall be maintained in
format UNISCO (F) HSE-40.

4.18

PAINTING
All painting processes/type/method shall be in accordance with UNISCOs approved
procedures where operation involving paints which produces flammable vapors

Care shall be taken when mixing and transferring paints.

Naked flames and lighted cigarettes shall not be permitted

All paint spillage shall be mopped up immediately.

Any waste rags or other combustible material contaminated with paint shall be disposed

Firefighting equipment shall be readily available.

When working with paint, which may contain lead, special precautions, shall be followed to
prevent absorption through skin, swallowing or inhalation of the fumes.

When working in confined spaces, where ventilation is restricted, respiratory protection and
the provision of breathing apparatus shall be provided.

Adequate washing facilities including hand cleansers shall be made available.

Ensure where the nearest eye wash areas.

Do not eat, drink or smoke while handling chemicals

Spraying shall not be conducted outside the predetermined spraying areas.

All spraying areas shall be kept free from the accumulation of deposits of combustible
residues as practicable, by cleaning.

Scrapers, or other tools used for cleaning purposes shall be of non-sparking material.

4.18.1 BLASTING

Blasting equipment and ventilation system shall be checked prior to use.

Blaster hood shall be ventilated by clean cool air through regulator / filter

Dead man control shall be fitted at the nozzle end of the abrasive blasting hose

The rated working pressure of a blast hose shall not exceeded

Blasting operation shall not take place with a coiled hose

Hose whip arrestor or hose coupling safety locks or both shall be fitted to hoses

Pinholes shall not tape in the blast hose. The hole will enlarge quickly and will cause a blow-out

Blasting hoses shall be coil and stored away from water, oil, and chemicals to prevent rotting.

Sufficient Illumination shall be maintained.

Appropriate PPE shall be used.

RADIOGRAPHY TEST

UNISCOs approved subcontractor shall carry out all radiographic tests as per the applicable client /
Code requirements.
Radioactive work shall be carried out end of shift or required distance and proper protection
shall maintain.
All classified employees shall have adequate training and qualifications for their job.
All radiation instruments shall have a valid test / calibration certificate. Recalibration and recertification shall done annually.
Appropriate personal protective equipment before entering a controlled area.
When radiation work is conducted in an operating area where other employees shall be
exposed to ionizing, will be barricade and signboard shall be placed like DANGER, Keep
Away, and Radiography test is in process.

HYDRO TEST & PNEUMATIC TEST


All hydro tests shall carry out in accordance with UNISCOs approved procedures. [Internal
work permits Format. UNISCO (F) HSE-21]

The surrounding areas of the vessel/tank under pressure test shall be barricaded.

Sign board showing vessel/tank under pressure test shall be placed near the location.

The pressure gauges shall be verified for proper functioning and the Pressure gauge pressure
range shall be ensured to withstand the test pressure.

Verify that all supply lines and hose connections are secure with retaining devices before and
during the test.

4.21

Adequate lighting shall be available throughout testing operations.


MACHINERY AND EQUIPMENTS

All machinery and equipments in the shop / site shall be free from defects, with necessary guards
in place and to safe for its intended use.
All mobile and overhead cranes used inside the premises shall undergo regular maintenance and
periodical load test certification.
All rotating parts shall be guarded, including pedestrian fans covered with nets.
Safety devices shall not manipulated.
Safety glasses with side shields and applicable PPES shall be worn during machining time.
No attempt shall be made lubricate, clean, adjust, repair while the machine running.
Compressed air shall not use clean human body/Machine.
Machining chips shall be removed with steel wire brush.
Proper clamping of the work piece on the machine shall be ensured prior to start the machine.
All machinery and equipments shall be inspected every quarterly with color code.

ELECTRICAL SAFETY
ELCB (Earth Leak Circuit Breaker) shall be used with all electrical equipment and tools.
Equipment grounding conductors to installed.

ELCB (Earth Leak Circuit Breaker) shall be inspected & tested on a regular weekly basis.

Equipment connected by cords/leads and a plug shall be visually inspected for external evidence

certified safe for use by maintenance department.

CONFINED SPACE

Confined space activities shall be undertaken under a permit to work system


[Format. UNISCO (F) HSE-27].
Following points shall be considered

Presence of inflammable liquids / gases.

Presence of toxic liquids / gases.

Limited access (making entry, exit, and movement of materials difficult).

Enclosure (outsiders do not know often what is going on inside confined space).

Limited working area

Oxygen concentration shall be not less than <19.5% or >23.5%.

Continuous gas monitoring shall be carried out when found necessary.

Multi gas detector meter shall be calibrated and details shall be recorded in format UNISCO (F)
HSE-43.

A stand-by worker trained in emergency response and the use of & equipped with self-contained
breathing apparatus/ escape set shall be stationed outside the confined space and must maintain
permanent contact by predetermined means. Escape set shall be calibrated and details shall be
recorded in format UNISCO (F) HSE-43.

Care shall be taken to ensure that no flammable or toxic vapours are drawn into the confined
space i.e. vapours from adjacent facilities, vehicle or compressor exhausts, etc. while working
inside.

Adequate illumination shall be ensured in the confined spaces. Temporary lighting shall be
provided by hand held battery units, which are essentially safe. Air shall be supplied and
exhausted through separate inlet and outlet valves maintaining adequate level of oxygen and air
movement where required.

Compressed cylinders shall not be kept inside any confined space.

HOUSE KEEPING

Ensure that good housekeeping practices followed.

Work areas, passageways and all other areas shall be kept free of obstruction of debris and
materials.

Temporary extension leads, wires, electrical cables etc. shall be kept off the walking
surfaces and routed in an orderly manner.

EMERGENCY RESPONSE PLAN :The entire employee shall be trained to react in the event of an emergency scenario like fire or
any other accident. Safety engineer/ officer & Site/ production Managers shall ensure the
implementation of emergency procedures and responsible to prepare the employee to act
appropriately in the event of any emergency. The emergency procedures shall be strictly
adhered.
Call ROP /Civil defense emergencies no.9999 and describe the details of accident in case of
major accident.

The areas designated by HSE department at front and rear gates, which are safe
meeting point and easy vehicles boarding points.

Emergency vehicle: Emergency vehicle shall be stationed in all the facilities for entire shift hours.
First aider:

The group of employees trained in first aid and they shall be responsible for
giving first aid in case of any medical emergency and take care until the
emergency vehicle or ambulance arrives for further treatment.

Fire warden:

The group of employees trained in fire warden and they shall be responsible for
communicate with and obtain necessary instruction from emergency response
team leader and facilitate the assembled employees in the designated assembly

proceed.
Initial response:

Make alarm, upon hearing the emergency alarm, the emergency response team will
lead the entire employee to the emergency exit to nearest assembly point.

Evacuation decision: Based on the severity of the emergency situation, the emergency team leader
decides if an evacuation of all employees is required, if he deems an evacuation
necessary he instructs to sound the alarm (siren) and all employee in accord with
the evacuation action plan presented later in this document. Periodic emergency
evacuation drill will be conducted to evaluate the response time.
Emergency team duties:
Upon notification of an emergency, the emergency response team is responsible for
performing the following duties.
The emergency response team members must stop all the work and proceed directly
to the emergency assembly point. If any of the members is operating crane, forklift
or other movable machinery, he has to stop it assemble in nearest assembly point.
Subsequent to the emergency gathering at assembly point, the emergency response
team members must follow the instruction issued by the team leader/ coordinator
and carry out the assigned duties.

Assembly point:

Do follow the evacuation & emergency procedure.

Assembly points shall be located in front and rear gates of our units.
Upon hearing of evacuation siren all employee as directed by their
foreman/supervisor must switch off / secure all machines, power equipment,
tools, crane, vehicles, generators and compressors, gas cylinders or
electrical appliances and proceed directly to their assigned relevant
assembly point using the shortest and safest route. Foreman/ supervisor

count their employees and then report the total absentees to the emergency
team leader.
The emergency response team leader will ensure all office staffs report to
their designated assembly points and perform head counts of the office
employee then report total absentees to the emergency team coordinator
furthermore ensure all visitor report at a designated assembly point.
Emergency team coordinator to set feedback of head count and he had to
decide what action to be taken search and rescue of missing employee.
Ensure all employees are aware that they may not leave the assembly points
unless otherwise instructed in emergency or evacuations.

First aid team actions:

During evacuation, first aiders shall be stationed at the assembly point. They
shall be responsible for ensuring all medical/ first aid kit is available and
attending the victim and providing necessary first aid.
If any victim needs to transfer for emergency medical attention, an
emergency vehicle shall be used for this purpose. According to the severity
of the injury, the Site/Production Manager (emergency response team
coordinator) will contact ambulance for further treatment.

Important notes to be followed:


1. Conduct emergency/ evacuation drills to check the readiness of the employees and take prompt
corrective action in cases where employees alertness is deemed unsuitable.
2.

Ensure disabled employees are assisted during such emergencies or evacuation

3. Ensure all new employees and sub contractors are trained in safety induction
4. Ensure all visitors know the assembly point.
5. Conduct periodic inspection to check to readiness of operation such as fire fighting system, fire
alarm system, emergency exit routs, and first aid facilities etc.
6. Ensure all emergency response team are ready to adequately evacuate from their assigned work areas
to the designated assembly points.
7. Ensure all employees are aware that they may not leave the assembly points, unless otherwise
instructed by emergency response team.

ELECTRICAL ACCIDENT
In the event of an electrical accident, there shall be one or more causalities due to
electrocution
Generally Causalities will be unconscious / or might have mental shock

Should not try to rescue the casualties before de -energize the electrical supply, even if
the supply has already tripped. In the mean time, casualty shall be separated from the
electric wire by means of non-conductor such as wood, stick or nylon rope

In case of mental shock, but conscious, make him comfortable by talking to him

Remove the casualty to the nearest clinic/hospital as fast as possible with the available
transport.

In the event of flood or high wind expected, the following actions shall be taken:

Avoid nearby that may ignite the dust.
Ensure- No smoking to prevent any fire.
Suspend the activities in consult with Site /Site/Production Manager and evacuate the employees
to Assembly Point.

INCENTIVE SCHEME:
Safety Man of the Month:

An employee shall be identified for their best effort on implementing and following safety
procedures from the guidelines of UNISCO HSE for the month. Format : UNISCO[F]HSE-45

Site/Production manager/ Engineer will identify assess the person capability & efforts with the
help of Safety Engineer/Officer to evaluate the same.

Three candidates would be selected and among them, the best safety person would be chosen. A
cash prize of OMR 10.00 shall be awarded with certificate as an appreciation.

8.0 HAZARD/RISK & ASPECT/IMPACT REGISTER & LEGAL REQUIREMENT REGISTER:


PURPOSE:
To carry out aspect/impact & hazard/risk study & to identify significant aspects/hazards and its
impacts/risks with reference to procedure UNISCO [P] MS020 & UNISCO [P] MS021,records
maintained in risk register UNISCO (F) HSE- 36 and aspect register UNISCO (F) HSE- 37

Legal requirements are identified and listed with reference to procedure UNISCO [P] MS022;
records are maintained in legal requirement register UNISCO (F) HSE-25. Permit register
maintained in format UNISCO (F) HSE-40

9.0 MONITORING METHODOLOGY

The main areas of subcontracting activities are

Metal & Wooden scrap removal.

Proof loading of overhead cranes.


All the subcontractors shall be made aware about UNISCOs HSE requirements & Standards to be
adhered with. During all the stages of subcontracting activities, HSE requirements shall be
highlighted & before parties are allowed for actual quoting, site conditions are shown compulsorily,
& the prevailing safety culture in UNISCO is manifested.
Where ever applicable the specific standard operating procedures as per UNISCO s quality
manual, segment of HSE manual copy, required for the subcontracting activity to be complied with
are handed over to the subcontractor.

All the necessary documentation records generated during the course of contract execution shall be
preserved for future references, if required. Throughout the job progress, sub-contracting activity is
monitored & necessary co-ordination/directions are given to comply with the HSE manual
requirements. First aid cover, portable water supply is always ensured. If applicable as per
contractual obligation, food provisions and lodging facilities at camp are also provided.

Disciplinary actions, if violations made are applied to the sub-contractors in the same guidelines as
it is imposed on UNISCO employees.

If subcontracting activities are of longer duration, HSE plan may be asked for submitting & reviews
shall be made about the effectiveness of mitigating measures on the hazards generated by the
proposed activities. Approval shall be entrusted once the manual addresses all the issues of
probable and possible hazards generation.

The main motto is to coordinate for carrying out the subcontracting jobs without any obstacles &
also without causing any hindrance to UNISCOs activities thus attaining
ZERO ACCIDENT AIM.

Definitions:
Near Miss:
A Near Miss is an unplanned event, which under slightly different circumstances could have resulted
in harm.
First Aid Case (FAC):
Work related injury that requires one time treatment and subsequent observation (for example minor
scratches, burns, cut, which do not ordinarily require medical care) and does not result in a Medical
Treatment Case (MTC).
Lost Time Injury (LTI):
Any work related injury which results in a person being unfit for work for a period of 72 hours or more
immediately following the occurrence of the occupational injury The period of 72 hours includes rest
days, weekend days, leave days, public holidays
Medical Treatment Case (MTC):
Medical treatment case is not LTI but requiring medical treatment from a professional physician or
qualified medical person and MTC case is more than a First Aid.
Risk:
The likelihood that an event will occur (i.e. the evaluation of a hazard)
Hazard:
Anything that has the potential to cause harm or injury, or damage to human, property or the
