PARENTAL RELEASE, ASSUMPTION OF RISK AND INDEMNIFICATION AGREEMENT

We hereby understand and agree that there are certain risks specifically related to The ISGOC Youth Group 5/28
Camping Trip, that our Child/Ward will take, and that all travel can be dangerous and involves risks of serious
injury and even death. We also understand and agree that although the Trip Leader(s) and trip chaperones will
chaperone The ISGOC Youth Group 5/28 Camping Trip our Child/Ward may be unsupervised at times during
his/her participation in The ISGOC Youth Group 5/28 Camping Trip. In consideration of our Child/Ward being
permitted to participate in The ISGOC Youth Group 5/28 Camping Trip, we agree, on our own behalf and that of
our Child/Ward, to forever release, acquit, discharge, covenant to hold harmless and covenant not to sue ISGOC,
its trustees, employees, representatives, agents, volunteers, all related to or associated with ISGOC, and all other
individuals and organizations assisting or participating in The ISGOC Youth Group 5/28 Camping Trip (all collectively
referred to herein as the Releasees), from any and all claims, suits, liabilities, actions and causes of action,
including, but not limited to, claims of negligence on the part of the Releasees, which we or our Child/Ward or our
heirs, legal representatives, successors, conservators and assigns may have, now or in the future, which arise
directly or indirectly out of our Child/Wards participation in The ISGOC Youth Group 5/28 Camping Trip. We
hereby also agree, on our own behalf and on behalf of our Child/Ward, to indemnify the Releasees from and
against any and all claims, suits, actions, causes of action, including, but not limited to, claims of negligence on the
part of the Releasees, and liabilities, including attorneys fees, by any person (including without limitation our
Child/Ward and either of us) resulting directly or indirectly from our Child/Wards participation in The ISGOC Youth
Group 5/28 Camping Trip, including, but not limited to, injury of any person caused by our Child/Ward or for
damages to or destruction of any property caused by our Child/Ward. We have reviewed the plans for The ISGOC
Youth Group 5/28 Camping Trip and recognize and appreciate the risks and danger associated with our
Child/Wards participation in the trip, including, but not limited to, risks associated with air travel, surface travel,
the potential negligence of the Releasees, and participating in the daily activities of The ISGOC Youth Group 5/28
Camping Trip. We hereby assume the risk of any and all losses and damages, including, but not limited to, personal
injury which our Child/Ward may incur as a result of participating in The ISGOC Youth Group 5/28 Camping Trip.
We recognize that general social and political conditions and circumstances may affect The ISGOC Youth Group
5/28 Camping Trip and are outside of ISGOCs control and have agreed, with knowledge of those conditions and
circumstances, to allow our Child/Ward to participate in The ISGOC Youth Group 5/28 Camping Trip and to assume
the risks that we are assuming by this agreement. The release, assumption of risk and indemnity provisions
contained above include any property or personal loss or damage, or other loss or damage caused or alleged to be
caused, in whole or in part, by the ordinary negligence (but not gross negligence) of the Releasees. We, the
undersigned, do hereby solemnly swear that we have the legal custody of __________________ [insert name of
Child/Ward]. We have read this Form in its entirety and we have satisfied ourselves that we understand what it
means. We hereby expressly agree that the provisions contained in this Form are intended to be as broad and
inclusive as permitted by the laws of the State of Oklahoma and that if any portion hereof is held invalid or
unenforceable, we agree that the balance shall continue in full legal force and effect. By signing this Form, we
affirm that we have decided to allow our Child/Ward to participate in The ISGOC Youth Group 5/28 Camping Trip
with full knowledge that the Releasees will not be liable to anyone for any personal injuries or property damage
our Child/Ward may suffer while participating in the trip.
