l'dme vertbral post-traumatique sans fracture est voquer dans un
contexte traumatique ;
rarement, le diagnostic est celui de dysplasie fibreuse, au cours de laquelle on
note une sclrose priphrique vocatrice.
Pour diffrencier ces diverses tiologies, le contexte est primordial. Les lsions multiples sont souvent secondaires. L'aspect micronodulaire mouchet est vocateur
de mylome. Un hypersignal vertbral en diffusion se voit surtout dans les lsions
infectieuses.
Lsions en hypersignal T1 et T2
Il s'agit le plus souvent d'angiome banal (fig. 13.5). La dgnrescence graisseuse est
frquente chez le sujet g ; elle est htrogne. L'angiolipome est une tumeur rare
comportant des composantes tissulaires et graisseuses.
L'involution graisseuse post-radique a un aspect caractristique : hypersignal T1 et
T2 homogne de plusieurs vertbres adjacentes, en regard de la zone d'irradiation
(fig. 13.6).

Lsions en hyposignal T1 et T2
Ces lsions correspondent une ostocondensation de : un lot condensant
bnin, une mtastase avant tout prostatique ou une dysplasie fibreuse. Un hyposignal trs marqu peut se rencontrer aprs vertbroplastie. Dans la maladie de Paget,
la vertbre peut tre condense ; elle est en gnral augmente de volume.
L'ostochondrome est rare dans cette localisation.
ANOMALIES DE MORPHOLOGIE
Tassements vertbraux
La fracture est rcente (< 3 mois) : la vertbre tasse est en hyposignal T1, hypersignal T2 et surtout T2 STIR, et prend souvent le contraste. Il peut s'agir d'une fracture
tassement sur vertbre normale ou ostoporotique (fig. 13.7A et B, et voir fig. 9.5) ou
sur vertbre pathologique, notamment mtastatique.
Quelques lments diagnostiques sont proposs ci-dessous.
Arguments tiologiques devant un tassement vertbral
En faveur d'un tassement sur vertbre tumorale :
notion de cancer primitif ;
topographie cervicale ou dorsale haute ;
lsions multiples ;
bombement du mur vertbral postrieur pidurite ;
atteinte associe du pdicule ou de l'arc postrieur ;
hyposignal T1 nodulaire ou global de la vertbre ;
envahissement des tissus mous adjacents ;
hypersignal diffusion avec ADC bas (souvent < 1,103 mm2/s).
En faveur d'un tassement bnin :
terrain ostoporotique ;
topographie lombaire ou dorsale basse ;
lsion unique (ou autres tassements ostoporotiques associs) ;
fracture du mur vertbral postrieur ;
aspect en bande horizontale de l'hyposignal en T1 ;
absence de lyse osseuse ;
absence de lsion des parties molles adjacentes ;
hyposignal diffusion avec ADC lev (souvent > 1,103 mm2/s).
Il s'agit d'un tassement ancien (> 3 mois), essentiellement ostoporotique ou posttraumatique : le signal vertbral est normal (fig. 13.7C et 13.8).
Lorsque la vertbre est compltement tasse (vertebra plana), outre les causes prcdentes, des tiologies particulires sont discuter, en particulier un tassement sur
granulome osinophile ou hmangiome.
