The Contest is organised by Measat Broadcast Networks Systems Sdn Bhd (”the Organiser”). The Terms and Conditions of this
Contest are attached herewith or can be obtained from www.astro.com.my. Participants are required to have read and understood
the Contest Rules and Regulations before signing this Application Form.

This Application Form need to be submitted to the organizer upon completing and signing prior participating in the contest. By the
submission of entries into this Contest, the participants shall be deemed to have unconditionally accepted the Terms and Conditions
of the Contest.

Have you ever participated in any contest organized by Measat Broadcast Network Systems Sdn. Bhd.
(“Astro”) and/or other broadcasting television stations and/or participated and/or performed with respect
to any public event or such other performances? If yes, please provide details as to the name of the
programme/event/performance and the date and your participation in the same (i.e. whether you sang,
danced, acted etc).

Are you contracted to any recording company, management company and/or such other entertainment
related companies. If yes, please provide details pertaining to the name of the company(ies) that you
are contracted to and the duration of your contract with such company(ies) and whether there are any
restrictions in your agreement(s) with such company(ies) preventing and/or restricting you from (i)
competing in the Contest; (ii) entering into any service agreement with Astro and/or any of its affiliated

I certify and confirm that I have read and understood all the contents of this Application Form and the Rules and Regulations and
the entry and eligibility requirements for the Contest and by signing this Application Form I hereby signify my acceptance and
agreement to abide by all the Contest Terms and Conditions provided herein and all the Contest Rules and Regulations. I agree
and consent to the interview, photograph, audio and/or visual recording by the Organizer (collectively ”Recording”). I confirm that
all details, particulars and personal data given by me to the Organizer are true, accurate and complete in all respect and shall
remain true, and I have not willfully or knowingly omitted and/or misrepresented any information. I agree that any failure to attend
the Recording at such date and time specified by the Organiser would result in my disqualification from the Contest. If I am
notified by the Organizer that I have been disqualified at any time during the Contest Period, I agree to immediately withdraw
myself from the Contest and/or the production of the Programme and will not challenge or question such decision.
