AFFIDAVIT OF DISCREPANCY
I, Yolanda Holgado Soriano Filipino, of legal age, with address at 17 G.
Raymundo St. Malinao Pasig City, after having duly sworn to in accordance with law,
hereby depose and say that:

1. I was truly astounded and dismayed when I learned that my Birth Certificate,
issued by the Nastional Statistics Office contained an error which created
confusion and doubt as to my true identity and filiation;
2. That the discrepancy in my Birth Certificate pertains to the my last name and
appearing thereon which is contrary to reality and can only be ascribed as
gross inadvertence or oversight.
3. In my Baptismal Certificate the first name registered is OLGADO whereas the
proper and correct spelling should appear as HOLGADO.
4. As evidence of the foregoing I have hereunto attached a copy of my Marriage
Contract issued by the National Statistics Office.
5. This affidavit is executed for the purpose of establishing the above-stated
facts and to inform the authorities concerned and not for any illegal purpose.
