JOINT AFFIDAVIT OF TWO

DISINTERSTED PERSON
We, JOSELITO MACAM and WILLIAM ANG both Filipinos, of legal age and residing
respectively at 735 MADRID STREET BINONDO, MANILA and 322 P. CARREON BINONDO,
MANILA, after having been duly sworn in accordance with law, hereby depose and state that:
1. We personally know CARLOS E. DE JESUS, whose name and circumstances of birth appear in his
Certificate of Live Birth, issued by the National Statistics Office.
A copy of the aforesaid Certificate of Live Birth is attached to this Joint Affidavit.
2. However, in the records of the Social Security System (SSS), CARLOS ESCOTO DE JESUSs
middle name was not registered.
His middle name is ESCOTO, as correctly stated in the aforesaid Certificate of Live Birth.
3. To the best of our knowledge and information, CARLOS ESCOTO DE JESUS and the person
referred to in the records of the SSS as CARLOS DE JESUS are one and the same person.
4. We are not related to CARLOS ESCOTO DE JESUS, either by affinity or consanguinity. However,
we have knowledge of the foregoing facts by reason of our close friendship with his family, who have
been our friend for more than 3 years.
5. We are executing this joint affidavit for the purpose of attesting to the foregoing declarations and for
all other lawful purposes that it may serve.
