Module BLesson 1

Student Survey
The survey is designed to evaluate my effectiveness in providing multiple means of representation,
action and expression and engagement. It also provides insights into whether the students feel that
their classroom community is kind, caring and inclusive.

Read each question carefully and choose the number that best describes you. There are no right
or wrong answers, simply circle 1 if the sentence does not describe you at all and 5 if the
sentence strongly describes you. If you describe yourself as somewhere in the middle rate
yourself accordingly.
