Valid NBI or Police Clearance of the prospective adopter/s.
Barangay Certification as proof of residence.
Certificate of Employment with amount of monthly or gross income indicated, or any proof of
financial capacity.
8. Certificate of no pending action issued by Municipal Trial Court having territorial jurisdiction
over your residential address.
9. Certificate of no pending action by Regional Trial Court having territorial jurisdiction over
your residential address.
10. Affidavit of Consent from the biological parents of the prospective adoptee.
11. Valid government-issued ID of the biological parents of the prospective adoptee.

For the purpose of drafting and filing of the petition for adoption, we only need for now,
the scanned/photocopy of the documentary evidence mentioned in numbers 1 to 7, 10
and 11.
The requirements on numbers 8 and 9 are needed after filing of the petition and
during the course of the trial so you have to prepare and secure the original of the same
as soon as possible.
