AFFIDAVIT OF CONSENT
I, Solita V. Albano, of legal age, married, Filipino
citizens and residents of Brgy. 4 Bacarra, Ilocos Norte,
Philippines, after having been duly sworn to in accordance
with law do hereby voluntarily depose and state the following:
That I am the legal gurdian of the minor Michael Sean
Corpuz, who was born on JULY 18, 2008 at Gov. Roque B. Ablan
Sr., Mem. Hospital, Laoag City, Philippines;
That our said child will travel to Davao City, Davao del
sur;
That I cannot personally travel with our child due to
work;
That I am giving my full consent to the travel of the
aforementioned child to Davao City and to be accompanied by
Michael V. Albano;
That I am executing this affidavit to attest to the truth
of the foregoing facts and for whatever legal purposes it may
serve.
IN TRUTH WHEREOF, we have hereunto set our hands this
