LESSON SUBMITTAL INSTRUCTIONS - PLEASE READ!
ATTENTION STUDENTS! Our database system requires that lessons be SUBMITTED

IN CORRECT ORDER (see below). Each lesson has a specific Lesson Identification
Form.
LESSONS SUBMITTED OUT OF SEQUENTIAL ORDER OR WITHOUT PROPER
LESSON I.D. FORMS WILL BE RETURNED.
By following these procedures, our office can efficiently expedite the processing of
your lessons. If you have questions or have misplaced your forms, contact our office at

INSTRUCTIONS FOR SUBMITTING LESSONS

Each Lesson/Assignment has its own Lesson Identification Form. Be sure to attach the correct form to each
assignment. Incorrectly identified Lessons will be returned to students ungraded. Keep a COPY of each
assignment for your records. No photo copies of Lesson I.D. Forms will be accepted.
NOTE: Do NOT staple or attach Exam Request forms to lessons being submitted.
Send lessons by mail: 1701 Broadway BE1140, Seattle WA 98122, or

Bring to Distance education office, room be1140, or
E-mail FOLLOW INSTRUCTIONS IN YOUR COURSE GUIDE
Do the following BEFORE submitting lessons via EMAIL or FAX:

1. Write your NAME on the top and stick personal Barcode labels to ALL Lesson ID Forms (at Stick
Barcode Here)
2a. Mail ALL original Lesson I.D. Forms TOGETHER to our office before Emailing lessons.

2b. Scan your Lesson I.D. Form and email with your lessons.
OR
2c. Fax (206) 934-5562 your lessons (be sure to use the Lesson I.D. Form as a cover sheet).
