AFFIDAVIT COMPLAINT
I, BEA MALONZO, of legal age, Filipino, single and a resident of Brgy.
Dalawinon Magsaysay Davao Del Sur, Philippines, after having been sworn to law
deposes and says:
1. That I am the same person who executed the foregoing affidavit complaint against
Raja Salvador;
2. That sometime on august 1, 2016 at 4:00 p.m, I was in my house doing a
household chores, upon a sudden the respondent surreptitiously entered in my
house, boxed me, kicked me and slapped me on the face, uttering words against
me as KABIT KA, PUTANG INA KA or (YOU MISTRESS, YOUR SON OF
A BITCH") she continued her act, until one of our neighbor came to the rescue;
3. After the incident I immediately go to our Barangay, to report said incident. The
Barangay Blotter is attached hereto as ANNEX A.
4. On the same day, I went to Paulino Hospital in Digos City to have a medical
examination for the injuries I sustained; the doctor said that I suffered minor
contusion and bruises on my right upper arm. A copy of medical certificate is
attached hereto as ANNEX B.
5. That on august 5, 2016, I filed a complaint to the Barangay and after several
hearings, no amicable settlement was reached between the parties. The certificate
to file action is attached hereto as ANNEX C.
6. That I am executing this affidavit to attest the foregoing statement and for the
purpose of charging the respondent Raja Salvador for the Crime of Slight Physical
Injuries, Trespass To Dwelling and Slander By Deed, penalized under the Revised
Penal Code.
IN WITNESSES WHEREOF, I have hereunto affixed my signature this 28th day
of august 2016, in the municipality of Magsaysay, Davao Del Sur, Philippines.
