Job Summary:

To work as part of a multi-disciplinary Community Hospital @ Home Service


to provide nursing and re-enablement support to patients. This includes
assisting in the ongoing assessment of the care needs and reporting all
changes in status to the senior member of staff.

1.

Main duties and responsibilities


To contribute to the provision, development and review of care plans, by supporting an
interagency approach to delivering individualised care. Implementing care plans and following
programmes set up by Physiotherapists, nurses, and other health professionals to provide
comprehensive integrated care and rehabilitation programmes.
To work as part of the multi-disciplinary team that includes Nurses, and Therapists.
Liaising with clients, colleagues, family and other professionals in ensuring integrated quality
care is provided. This will include assisting clients to learn new skills that promote
independence.
To empower clients and adopt a problem solving approach by encouraging and giving clients
the confidence and skills to become independent.
To respond effectively and actively to complaints following departmental policies by reporting
to person in charge.
To assist professional staff in care and treatment and participate in the review process.
To promote equality for all, including the maintenance of confidentiality, supporting individual
customs, values and spiritual beliefs and ensuring that anti discriminatory practices are
demonstrated.
To support carers in their role of carer. This may include communication and liaison with other
agencies, including partnership agencies.

On occasion to provide practical / personal support for people identified as needing support
until their treatment is completed.
To support clients with their personal and emotional care needs, whilst respecting their dignity
and maximising their independence.
To support the service with some administrative tasks such as inputting data.

Communication
To actively participate in multi-disciplinary meetings and virtual ward rounds.
To support effective communication both internally and externally.
To evaluate, assess and monitor clients progress in relation to their goals and care plans and
to record accurately then report back any changes or concerns to person in charge.
To empower clients, families and carers by ensuring that they have an understanding of the
care and treatment offered.
To maintain accurate documentation as required by the Sussex Community NHS Foundation
Trust Information Governance.

Team working
To respect and acknowledge professionals skills and boundaries, taking opportunities to
develop self knowledge, contributing to effective Team Work.

4.

General duties
To use re-enabling and rehabilitation techniques in activities such as washing and dressing
practice, mobility practice, kitchen skills and prescribed exercise programmes.
To work closely with the Nurses to contribute to an efficient nursing service for patients with
acute to chronic disease.

Flexibility
This job description is intended to provide a broad outline of the role. The postholder may be
required to carry out other duties including patient support and attends to some administrative
duties commensurate with their banding and competence.

Policies and Procedures


The postholder is required to familiarise themselves with all Trust policies and procedures and
to comply with these at all times.

Confidentiality and Data Protection

The postholder must maintain the confidentiality of information about patients, staff and other
health service business and meet the requirements of the Data Protection Act (1998) at all
times. The postholder must comply with all Trust information and data protection policies at all
times. The work of a NHS Trust is of a confidential nature and any information gained by the
postholder in their role must not be communicated to other persons except where required in
the recognised course of duty.
7.

Health and Safety


Employees must be aware of the responsibilities placed on them under the Health and Safety
at Work Act (1974) and must follow these in full at all times, including ensuring that they act in
line with all agreed procedures at all times in order to maintain a safe environment for
patients, visitors and colleagues.

8.

Use of Technology
The Trust is making increased use of computer technology. The majority of employees (both
clinical and non-clinical) should expect to use automated information systems in their work in
order to improve quality and co-ordination of services, and to enable faster and more certain
communication within the Trust. Necessary training will be provided.

10. No Smoking Policy


Sussex Community NHS Trust operates a no-smoking policy, in line with Government
legislation. This applies to all staff, visitors and patients. It is a condition of employment for
staff that they do not smoke whilst on duty or in uniform or anywhere on Trust premises.
Whilst we do not discriminate against employing smokers, they are expected to adhere to this
policy and all prospective employees should be aware of this.

11.

Professional Registration
All employees who are required to be a member of a professional body are required to hold
relevant registration and must comply with the standards and guidelines of their professional
practice, education and conduct and must act at all times within the boundary of the code of
conduct.

12.

Infection Prevention and Control


Infection Prevention and Control is everybody's responsibility. All staff, both clinical and non
clinical are required to adhere to the Trusts Infection Prevention and Control policy and
make every effort to maintain high standards of infection prevention and control at all times
thereby reducing the burden of Health Care Associated Infections.
