TO THE HONORABLE JUDGE OF SAID COURT:

respectfully petitions the Court for an Occupational Drivers License under the Provisions of Section 23A
and 25, Article 6687b of Vernons Texas Civil Statutes, and would show unto the Court as follows:

OCCUPATIONAL DRIVERS LICENSE


Rules for County Court at Law, Fort Bend County, Texas, are as follows as Wit:
1. Verified Petition must have attached as exhibits:
a. copy of court judgment suspending drivers license or DPS letter suspending drivers license
b. affidavit of Defendant/Petitioners employer or supervisor as to type of work performed, hours
and place of employment, and necessity to drive a motor vehicle
c. proof of SR-22 form.
2. An information copy of the Petition is to be delivered to Criminal District Attorney in Fort Bend
County at time of filing, along with Petitioners date of birth and Texas Drivers license number.
3. Attorney for Defendant/Petitioner is to prepare conformed copies of Petition and Order to be certified
by clerk.

ORDER

WHEREAS, on _______________________, _______, Defendant/Petitioner,


__________________________________, was convicted of the Offense of driving while intoxicated
upon a public highway in Fort Bend County, Texas, and his/her Drivers License No.
__________________ was automatically suspended, and said drivers license has been surrendered to
Clerk of this Court.

3. Copy of Judgment of Conviction of this Court
4. Proof of liability of insurance in accordance with provisions of Texas Motor Vehicle Safety
Responsibility Act as provided in amended Article 670lh of Revised Civil Statutes of Texas by
furnishing a true and correct copy of Form SR-22
5. A certified copy of Defendant/Petitioner Petition for an occupational license with all exhibits
attached hereto
6. A certified copy of this Order
