PROBLEM PRESENTED

Above client is suffering from Dengue for 3 weeks, and on such

condition she is appealing and asking for help at our office that she
could be referred at Kolambugan Medical Center.
IV. Diagnosis:
Clients problem started with a high fever and rushes last July 23, 2016
. Due to this problem, she went to Dr. Mejoradas clinic Kolambugan Lanao
del Norte for a check-up. As revealed, she was advised to take antibiotics,

and other rushes relief medicines because the doctors findings is only a
severe fever. For her own welfare and comfort, she follows the doctors
advice. In fact, she already bought a lot of antibiotics and other rushes
medicines to the fact that it is only a normal fever due to over fatigue and
stress.
Client further said, that on July 28, 2016, she went to Kolambugan
Lanao del Norte Medical hospital for further check-up because her fever has
no remedy. Dr. Reyes that she has a dengue which needs immediate
treatment and medications. Thus, she is recommended for blood donor, and
daily medications, as early as possible.
V. Family Background
As stated, Pastora was born in Buru-on, Iligan City . When she was
18years old, Pastora together with her parents transferred to Purok 3,
Poblacion, Kolambugan Lanao del Norte and live permanently until to date.
Her father died last 1988 and 1989 her mother also passed away.
In October 1982, she met Enting L. Cabando who is a resident of
Poblacin, Kolambugan Lanao del Norte . He is a fisherman .They started to
know each other when Enting went to Market where Pastora is a Fish vendor.
Time had passed, their closeness ended up to marriage which was held last
September 19, 1985 . And Pastora stop working as a Fish vendor and focused
only as a housewife because her husband wants Pastora to stay only in their
house to raise their child.

IV.

RECOMMENDATION
In view of the above findings and records of the case , the undersigned
worker recommends the above mentioned client to avail the financial
assistance of DSWD charge to AICS funds.
