While I have been working for Durham County Council, I worked on a Dementia care unit with 10 people.
During the time working on this unit, I supported the people with various activities including memory test
activities and sorting out old memorabilia such as post cards, pictures and books to allow the people to
recollect their past. Stories of old were also used to help the people I worked with remember aspects of
their past. Other duties included taking food orders, keeping the people safe within the building,
escorting the people to their relevant transport.

My current role involves assisting with risk assessments and working with other community groups such
as parents and carers whom i have created an excellent relationship with. I work in a person centred
way and encourage choice and independence by interacting with the service user and understanding
what their needs are, and how they want to achieve their goals. In my role as a community coach, I
managed a football team consisting of service users who live in and around the Consett area. The team
have entered into a learning disabilities Abilility Counts League and played around the north east one
sunday every month, which allows myself work flexable to the organisational demands and to meet the

needs of the service user. The service users also trained at consett Belle Vue leisure centre on a
Tuesday afternoon.
I have succesfully supported people to access public transport around the North East, giving them
confidence in developing their own social network and promoting social inclusion. I have also
encouraged the service user to help me in situations to enable empowerment for the service user and
also independence from my support.
I have developed myself a large social and educational network around the Derwentside area,these are
community centres, leisure centres, college, this has helped me start new activities and courses for the
people which whom I work with and has proved very effective in their learning. These activities and
courses are fun and has an educational value. I have encouraged physical exercise to stay fit and
healthy, and also to teach the service user new and exciting opportunities that has allowed the service
user enjoy what he/she has done.
I have also helped reintroduce some service users back into the service who have been away for some
time and that lost the confidence to interact with people they once knew.
I am always willing to take the opportunity to learn knew aspects of my role and to do extra duties that is
asked of me. I organise my work load well and keep up to date of my paperwork. I gather information
and share with my colleagues and other agencies but keep confidentiality.
Part of my role is to be key worker to to an individual person who's review meeting's I have attended with
their parents or carers and their social workers. In these meeting I have discussed their person centred
plan and how this is working with the activities they do.
I am approachable , friendly and professional and work as a team player, this reflects how I have been
seen to do my job well in the community and praised by otside agencies.

Starting 12th of january 2009 i was given role of acting senior community coach. I maintained this role
Iuntil january 2010. In this role, I managed a small unit based in Consett Methodist church consisting of
four staff and over twenty service users. My duties included organising rota's, dealing with petty cash,
dealing with any problems with outside agencies and also parents & carers, making sure all files and
paper work are up to date, dealing with transport money, keeping working area safe and tidy, dealing
with any staffing issues. These duties are not exhaused.
I worked in Apopka, Florida, U.S.A. on the Camp America program, working with people with learning
and phisical difficulties on a summer camp. Duties included assisting with personal hygiene of the
resident, assisting with the daily activities which included swimming, canoeing, sports and games,

nature, and arts and crafts and helping the resident make costumes or songs for the night time activities.
When summer camp had ended I was offered a place working in one of the group homes owned by the
company quest who owned the summer camp in which I worked. I worked with some of the residents
Who were on the summer camp for the 12 week duration. This included feeding, personal hygeine,
laundrey duties, cleaning and report writing.
I have very good organisational skill and able to communicate effectivly. I have a great knowledge of
computer packages such as word and excel and use the internet with great effect. Since i have joined
Durham county council, I have aquired numerous qualifications including NVQ level 2 and level 3 in
health & social care. I have completed a certificate in dementia Awareness and I am always willing to
learn more.
