On 11-12-16, I Officer J Hasenstab P#13801 was contacted by Detective Salisbury P#8264 in reference to a traffic
collision involving 2 fatalities, at the intersection of Rainbow/Yorktown. Willie Worthams was identified by listed witnesses
as being the driver of the above 2010 blue chevrolet camaro at the time of the collision. At the time of this report, Fatal
Detail Detective Salisbury was investigating the traffic collision to determined fault. Worthams was transported from the
accident scene to UMC Trauma by Medical Personnel.
Officer S Killeen P#9675 and I were dispatched to the hospital to evaluate Wortham for signs of impairment. While
Bedside with Wortham, Officer Killeen administered HGN test. Nystagmus was present in both eyes. Lack of
convergence was present in Wortham's left eye. While administering HGN, Officer Killeen, noted a strong odor of an
unknown alcohol coming from Worthams as he spoke. Worthams also had wattery, glassy, and bloodshot eyes.
Based on the above circumstances, Officer Killeen obtained a telephonic search warrant from Judge Sullivan to obtain
evedentiary blood samples from Worthams.

On 11-12-16, I Officer J Hasen stab P#13801 was contacted by Detective Salisbury P#8264 in reference to a traffic
collision involving 2 confirmed fatalities, at the intersection of Rainbow/Yorktown. Willie Worthams was identified by listed
witnesses as being the driver of the above 2010 blue Chevrolet Camaro at the time of the collision. Witnesses stated that
the Camaro, driven by Worthams, was racing a dark colored pickup truck, southbound on Rainbow, at a high rate of
speed. While traveling southbound, a grey Hyundai sonata, bearing NV plate
pulled out from the curb, in front of
the Camaro. The Camaro collided with the Hyundai. The two passengers of the Hyundai, who at the time of this report
were not identified, were pronounced deceased at the scene. At the time of this report, Fatal Detail Detective Salisbury
was investigating the traffic collision to determine fault. Worthams was transported from the accident scene to UMC
Trauma by Medical Personnel.
Officer S Killeen P#9675 and I while operating as marked traffic units T3101 and T3102 were dispatched to UMC hospital
to evaluate Worthams for signs of impairment. Officer S Killeen, who is a certified drug recognition expert (DRE#022415)
evaluated Worthams, while Worthams was lying in a hospital bed. While Bedside with Worthams, Officer Killeen
administered the HGN test. Nystagmus was present in both eyes. Lack of convergence was present in Worthams left eye.
While administering HGN, Officer Killeen, noted a strong odor of an unknown alcohol coming from Worthams as he
spoke. Worthams also had watery, glassy, and bloodshot eyes.
Based on the above circumstances, it was determined that Worthams was in physical control of a motor vehicle when he
was involved in a collision involving the death of two people. Based on Officer Killeen's evaluation, and observations,
probable cause exists to believe that Worthams was impaired at the time of the collision.
Officer Killeen obtained a telephonic search warrant from Judge Sullivan to obtain evidentiary blood samples from
Worthams. The warrant was approved at 2219 hours. The first blood sample was obtained at 2242 hours. The second
blood draw was obtained at 2337 hours. Both samples were stored in the secure refrigerator at CCDC.
Detective Salisbury's investigation revealed that Wortham's vehicle was traveling more than double the posted speed limit
of 35 MPH at the time of the collision. This excessive speed is a willful, wanton disregard for public safety, and property,
and it resulted in the death of two victims. Detective Salisbury's investigation is ongoing and the full report will be available
at a later date.
Worthams was cleared and released from UMC by medical staff. Worthams was transported to CCDC where he was
booked accordingly.
