The mother of a baby in the NICU was a


prostitute so the baby had a foster mom who
was in her 50s. I have to remember that the
family isnt always biological
One mother was on the run from her abusive
husband and was going to stay at friends houses
with her baby. This is going to impact the babys
health. Me and the charge nurse tried to help
her by giving her extra formula and diapers.
One of the mothers was so excited that she had
a baby and was so excited to take him home.
She ran back upstairs after her smoke break to
be with him

I used my knowledge of therapeutic
communication that I learned from Long Term
care to communicate with my patient to create
a professional nurse-patient relationship. During
my assessments I would explain everything I
was doing and I would tell her her vital signs and
about the health of her baby during her babys
assessment.
I used the HP lens when assessing my patient so
that I wouldnt judge my patient and perform
my assessment based on my assumption of my
patients history. When I went with a nurse to
take vitals on a patient, I judged the family by
thinking about how young they were to have a
baby and continued to judge because they
already had another daughter that looked old
enough to be sisters with her mother. I should

not judge because maybe this mother was older


than I thought she was and maybe since she is
Indian it is in their culture to start a family at a
young age.
I used my previous knowledge of how to take
Vital signs on my patient from when I learned
how to do them in Long Term Care. During my
assessments I make sure to remind the patient
not to talk or else it might impact the results. I
also use my previous knowledge by knowing the
normal ranges of vital signs.
I learned a lot of medications from NURS 2550
which was helpful to become familiar with the
drugs used on pregnant mothers and babies.
During my first assessment on a baby I noticed a
Band-Aid on her heel and I knew that it was
from the vitamin K injection.
When I was in the NICU the nurses gave a lot of
information about how to do assessments. I
made a better relationship with the nurses in
this unit by being able to talk to them about
nursing and by being able to work with them.
The amount of time you have to make a
relationship with your patient is shorter than it
was in Long Term Care. For each relationship
with my patient I make sure they trust me and
feel comfortable with me performing
assessments. I make sure they have everything
they need by asking if they need ice, pain
medication, water and if they have any
questions. A big part of the relationship is
making sure they know you know what you are
doing which will help them be more

When I was assigned a patient I had to perform
an assessment every four hours.
When performing an assessment, I had to
remember patient safety and I had to make sure
I provided my patient with privacy.
I used my patients charts to develop a plan of
care by helping myself remember what I needed
to ask her and perform during my assessment
For each patient I was assigned I went over the
pamphlets each new mom receives and made
sure they didnt have any questions. This was
important because it made them aware of
resources they can use for help and it also
helped them become more knowledgeable
about breast feeding and tummy time.
After each patient I was assigned I would reflect
on my previous assessments and think of parts
of the assessment I missed or could do better.
After performing many assessments, I realized
how important it is to the mothers to have
someone to help them by showing ways to take
care of their baby and by knowing that there is
someone there to take care of them if they
need it.
After having my clinical instructor with me the
first time while performing an assessment I now
feel comfortable and have been successful at
performing complete post partum assessments
When performing my assessments on my
patients I know what the appropriate results of
the vitals should be and I recognize any vitals
that are not normal. After taking a babys

temperature I realized it was too high so I


retook it to make sure I did it properly and I got
a better temperature.
After performing a baby bath with help from
students and my clinical instructor I am now
able to perform one on my own.
I make sure to provide my assigned patients
with compassion, commitment and care. During
one of my assessments my patients family was
present and I asked if she wanted them to leave
or for me to come back after their visit. I also
made sure to cover her while performing the
assessment to give her more privacy and I also
shut the curtain to give her privacy from other
patients in the room.
I showed professionalism by showing up to
clinical on time with clean scrubs and making
sure I am wearing my clinical badge and I am
fully prepared.
When meeting a new patient, I identify myself
as a nursing student and tell them my name.
After performing an assessment, I complete my
charting accurately and sign every page. If I have
forgotten a part of the assessment, I go back
and complete it and make sure I dont forget
that part of the assessment next time. For
example, during one assessment I forgot to ask
her her pain scale and check for bowel sounds.

Michaela, you are beginning to recognize the complexities of family centered nursing and relational practice. You have had the opportunity to
work a variety of families, which has contributed to your knowledge and understanding of the many roles of the nurse when caring for this
population. I have observed your confidence levels increase, and this is excellent. In your daily practice, reflect upon what this experience means
for the families and on how you can be in relation with them.
Continue to work on developing your leadership and initiative skills. Nurses have many roles when working with families and we must ensure
we are caring for the whole patient and family, not simply completing skills or tasks. Build upon your foundational knowledge skills from your
previous and current courses, including lab materials. You need to ensure you have a solid foundational knowledge, so that you are able to focus on
caring for the patient holistically.
From observing your practice, you strive to be professional in your interactions and are eager to help other students or staff. Please become
more familiar with the resources available for families from organizations such as the health unit. I would also like to see you integrate evidence
based practice into your care plans. You are professional in your interactions and eager to help. You practice in a non-judgmental way and strive to
ensure that your assessments are complete. I am looking forward to working with you and further developing your skills, Michaela.
