ANEMIA HEMOLITIK AUTOIMUN PADA WANITA USIA 40 TAHUN
DENGAN RETIKULOSIT NORMAL
Laporan Kasus Manora N,1 Herliani DPH,1 Felix L,1Nabilah,1 Fatmawati2 1 Dokter Umum Rumah Sakit Awal Bros, Pekanbaru 2 Spesialis Patologi Klinik Rumah Sakit Awal Bros, Pekanbaru ABSTRAK PENDAHULUAN Anemia hemolitik autoimun (AHA) merupakan anemia yang disebabkan oleh pemendekan umur eritrosit akibat autoantibodi yang menyebabkan lisisnya eritrosit itu sendiri dengan didukung hasil Coombs test positif. . KASUS Seorang wanita berusia 40 tahun datang dengan keluhan badan lemas. Pemeriksaan fisik ditemukan konjungtiva anemis. Pemeriksaan laboratorium didapatkan Hb 4,8 g/dL, MCH 32,4 fL, MCV 92,4 fL, dan hitung retikulosit 0,28%. Pada gambaran darah tepi, ditemukan eritrosit normositik normokrom, anisopoikilositosis, sel target, sferosit, dan fragmentosit disertai hasil Coombs test direk positif. PEMBAHASAN Diagnosis anemia hemolitik autoimun ditegakkan pada pasien berdasarkan gambaran darah tepi anemia normositik normokrom serta ditemukan sferosit, fragmentosit dan didukung oleh hasil Coombs test direk positif. Hasil positif tersebut menandakan adanya antibodi yang melekat pada permukaan eritrosit. Hitung retikulosit yang normal pada kasus ini mengarah pada kemungkinan adanya autoantibodi bereaksi dengan antigen pada retikulosit atau gangguan produksi sel darah di sumsum tulang. SIMPULAN Diagnosis anemia hemolitik autoimun ditegakkan berdasarkan anamnesis, pemeriksaan fisik, dan pemeriksaan laboratorium yaitu Coombs test direk positif. Oleh karena hasil retikulosit normal, maka diperlukan penilaian morfologi sumsum tulang melalui bone marrow puncture. KATA KUNCI Anemia hemolitik autoimun, retikulosit

RETICULOCYTE IN A 40 YEARS OLD WOMAN Case Report 1 Manora N, Herliani DPH,1 Felix L,1Nabilah,1 Fatmawati2 1 General practitioner in Awal Bros Pekanbaru Hospital 2 Clinical pathology specialist in Awal Bros Pekanbaru Hospital ABSTRACT INTRODUCTION Autoimmune hemolytic anemia (AIHA) is characterized by the shortening of red blood cell (RBC) survival and the presence of autoantibodies directed against autologous RBC. A positive Coombs test is the hallmark of diagnosis for AIHA. CASE A 40 years old woman presents with fatigue. Conjunctiva pallor was found in physical examination. Laboratory tests showed Hb 4.8 g/dL, MCH 32.4 fL, MCV 92.4 fL, reticulocyte 0.28%. Meanwhile, peripheral blood smear showed normocytic normochromic erythrocyte, anisopoikilocystosis, target cells, spherocytes, fragmentocytes and supported by positive Coombs test. DISCUSSION Diagnosis of AIHA in this patient was based on the finding of normocytic normochromic erythrocyte, spherocytes, and fragmentocytes in peripheral blood smear. The diagnosis was also confirmed by a positive direct Coombs test. A positive direct Coombs test implies the presence of antibody which is stuck to the surface of red blood cells.. The normal reticulocyte count in this case leads to the possibility of the presence of autoantibodies react with antigens on reticulocytes or disorder of blood cell production in the bone marrow. CONCLUSION The diagnosis of AIHA was based on history taking, physical examination, and lab exam with positive direct Coombs test. Despite of normal reticulocyte count, we need to assess the morphology of bone marrow through bone marrow puncture. KEYWORD Autoimmune hemolytic anemia, reticulocyte
