CONSEQUENCES OF NOT MEETING OUR EXPECTATIONS:
Verbal warning from teacher.
Removal from participation to fill out a Think Sheet. Student will rejoin class after student and
teacher have discussed the problem and agree on terms to ensure that the negative behavior
changes.
Parent contact and/or meeting to create a plan of action to ensure that the negative behavior
changes. This meeting could include student, teachers, parent/guardian, and/or school
administration.
Locker Rooms:
Students will be supplied with a combination lock the first week of Physical Education class. The students
will return the lock during the last week of class. If the lock is damaged beyond normal wear and tear or
lost, an $8.00 fee will be assessed to replace the lock. Students will have approximately 4 minutes at the
beginning and end of each class to change into the appropriate clothes. (They will still have their normal 4
minutes between classes for passing time.) Students will remain in the locker room until the teacher (or
Locker Room Supervisor) takes everyone to the gym. \
PE Clothes:
Lake Orion middle school students will change into proper athletic attire for Physical Education classes.
This is to allow for easier and better movement and help with personal hygiene. Students should be
prepared to change from the clothes they wore to school into their P.E. top and P.E. bottom on a daily
basis. Athletic shoes must be tied tightly to ensure they provide stability to the foot/ankle as well as stay
on the students foot.
PE Top: T-shirt (short or long sleeved) or sweatshirt. Tank tops are not allowed.
PE Bottom: Shorts, sweat pants, warm up pants.
Personal Hygiene:
Showers are highly recommended, but not mandatory. I will allow a few extra minutes at the end of class
if students would like to take a shower. Students may bring a bathing suit if they feel more comfortable.
This is a privilege -if students use this time unwisely, the privilege will be taken away from them.
GRADES:
A students final report card grade is figured by averaging the standards. These standards are important in
reporting a students physical education experience. Within each standard students will be evaluated
through a variety of assessments (observations and written assignments). These standards will be
reported by using a 4-point scale Evaluations of these standards are an ongoing process throughout
each quarter.

I understand that there is an inherent risk of injury in participating in this class. I have read and understand the
physical education syllabus. I am aware that my child will be walking/jogging/biking to the high school facilities for
pool, tennis and bike days (some neighborhoods). Student must walk/ride through crosswalks, subdivision roads and
parking lots. Students may not cross without permission.
