AFFIDAVIT OF OWNERSHIP
I, GERMAINE V. PINEDA, Filipino citizen, of legal age, single, a resident of
Poblacion, Taytay, Palawan, after having duly sworn in accordance with law hereby
depose and state that:
1. I am the owner of the following appliances:
a. 12 units EMAXX 2G DDR3 1600 Memory Module
Amounting to Php.725.00/ unit or 8,700 in total;
b. 11 units 18.5 LED Monitor 1360x 768 D
Amounting to Php.3,350.00/ unit or 36,850
c. 13 units WD Caviar Blue 3.5 500G SATAIII
Amounting to Php.2,050.oo/unit or 26,650
d. 10 pcs. Video Cards amounting to Php.3,200/ pc or 32,000; and
e. 1 unit printer - EPSON L220
Amounting to Php.7,800
2. That the above-described items were bought from NERD HERD Computer
Sales and Teslinque Computer Sales Center and were issued Delivery
Receipts evidencing said purchase, however, the Delivery receipt of Video
Cards cannot anymore be located, copy of the DR is herein attached;
3. That I am executing this Affidavit of Ownership to attest to the truth of the
foregoing and to support my application for loan from ECLOF MICRO
FINANCE- San Lorenzo, Poblacion Branch and for whatever legal purpose
this may serve.
