Standard Operating Procedure: Effective Date: 28 August 2014

Performing Blood Glucose Measurements with the LifeScan Select Simple Device (v2.0)

Gloves are in one package with strips and lancet (GALAU test package) for each patient.

Preparation Materials:
1. A pair of plastic/latex/nitrile gloves
2. LifeScan Simple Select Electrochemical SMBG Device
3. LifeScan Simple Select Electrochemical SMBG Test Strips (the code must be same with
device code)
4. Soap/Alcohol swab
5. Single Unit of Use Lancet
6. Biohazard Waste Container

Use plastic/latex/nitrile gloves for both hands.


Ask patient to wash hands first. When it is not possible to do, clean patients finger with
alcohol swab and wait until it dry.
Take a test strip and immediately close the vial cap to keep the other test strips in a dry
condition.
Ask the patient to wash their hands with soap and water. Hands should then be dried
completely. Another option can be to use an alcohol swab to clean fingertips prior to
lancing, but make sure that the patient has a COMPLETELY dry finger prior to blood
sampling. Alcohol may dilute blood samples leading to artificially low and inaccurate
results.

Gently squeeze the patients finger with slight pressure for collecting blood to fingertips.
Then ask the patient to position the fingers pointing downwards for 30 seconds.

Use a disposable single unit of use lancet to take a patient's blood. Avoid the middle
finger area (see red image in the illustration below) as this will cause unnecessary pain.
Instead target the sides of the finger for lancing which will reduce patient pain and
discomfort since fewer nerve endings are present in this area. (see green image in the
illustration below).

Discard the first drop of blood that comes out with a dry cotton ball.

Take a blood sample to the strip by touching the sample inlet of the test strip to the drop
of blood. Do not hold the machine and test strip under the trickle of blood as it may cause
excess blood to enter the test strip port and can damage the machine.
If the display shows Er5 on the screen, it means that blood drop sample has not
completely filled the confirmation window.
The test strip reaction chamber must be completely filled with blood. When the blood
sample is too small and does not completely fill the reaction chamber, DO NOT wiggle
the test strip, scrape another small drop of blood onto the test strip, or apply a larger
second drop of blood since this can result in incorrect measurement values. Repeat
measuring with a new strip AND a new finger puncture. DO NOT USE THE SAME
ORIGINAL FINGER PUNCTURE AND SQUEEZE OUT A SECOND SAMPLE AS A
FALSE TEST RESULT MAY LIKELY OCCUR. The concentration of each sample will
be different due to evaporation effects and dilution due to interstitial fluid being squeezed
into the second blood sample.
Remove and throw away the used test strip into the biohazard container and repeat the
measurement.
A cotton ball may be offered to the patient to stop bleeding from the fingertip sample
collection site. The blood glucose testing device will automatically begin counting down
and start measuring the blood glucose level. The time it takes to display the result is 5
seconds. When the number reaches zero "0", the results will appear on the screen.
The screen will display a countdown then display the test results after the countdown.
Record the measurement results on the health card PMR patients and pharmacys medical
records.

Discard the used single unit of use lancet, used test strip and cotton ball that has been
used into a biohazard container.
Disinfect the machine using the SOP for disinfection of blood glucose machine.

Remove the gloves and dispose of properly into a biohazard container.


Wash the hands according to SOP on hand washing.

Note:

The OneTouch SelectSimple Test Strips should be stored at a temperature < 30C

The OneTouch SelectSimple System components should not be refrigerated

The OneTouch SelectSimple has an operating temperature range of 10-44C

The OneTouch SelectSimple has a hematocrit range of 30-55%

The OneTouch SelectSimple is a 5-second test

Be sure not move the test strip during the measuring procedure
Device will automatically turn off in 2 minutes if no blood sample has been applied.
